How to Get a Fertility Test and What to Expect

Getting a fertility test starts with your OB-GYN or primary care provider, who can order initial bloodwork and exams or refer you to a reproductive endocrinologist for more specialized evaluation. Most people don’t need to jump straight to a specialist. Your regular doctor can run the first round of tests and help you decide on next steps based on the results.

When to Get Tested

The general guideline is straightforward: if you’re under 35 and haven’t conceived after one year of regular, unprotected sex, it’s time to get evaluated. If you’re over 35, that window shortens to six months. And if you’re over 40, the American College of Obstetricians and Gynecologists recommends talking to your doctor right away rather than waiting.

You don’t have to meet these timelines to ask for testing, though. If you have irregular cycles (more than 35 days apart), a known condition like endometriosis or polycystic ovary syndrome, or a partner with a history of reproductive issues, your doctor can start testing sooner. The timelines exist as general benchmarks, not gatekeeping rules.

Where to Start

Your OB-GYN is typically the first stop. They can order blood tests, arrange imaging, and refer your partner for a semen analysis. If initial results reveal something that needs further investigation, or if you need treatment like IVF, your OB-GYN will refer you to a reproductive endocrinologist, a specialist with advanced training in infertility.

Some people go directly to a fertility clinic without a referral, which is an option in most areas. The trade-off is cost: fertility clinics tend to be more expensive for initial consultations, and your insurance may require a referral from your primary provider before covering specialist visits. Calling your insurance company first can save you a billing surprise.

Blood Tests for Ovarian Reserve

For women and people with ovaries, blood tests measure key hormones that reflect how many eggs remain and how well the ovaries are responding to reproductive signals. Three hormones matter most.

FSH (follicle-stimulating hormone) is drawn on day 2 or 3 of your menstrual cycle. Your pituitary gland produces FSH to stimulate a follicle to grow and an egg to mature. When ovarian reserve is declining, FSH levels rise because the brain has to work harder to get a response from the ovaries. Higher day-3 FSH levels are associated with lower chances of success with both natural conception and fertility treatments.

Estradiol is often tested alongside FSH on the same day. If estradiol is already elevated early in your cycle, it can mean a follicle started developing too quickly, which tends to interfere with proper egg maturation. An early estradiol spike can also mask a high FSH reading, which is why the two are interpreted together.

AMH (anti-Müllerian hormone) is produced by the follicles that house your eggs and is considered an especially sensitive marker of ovarian reserve. Unlike FSH, AMH can be drawn at any point in your cycle, which makes it convenient to test. A lower AMH level suggests fewer remaining eggs, while a higher level generally indicates a larger reserve.

Ultrasound and Antral Follicle Count

A transvaginal ultrasound performed in the first few days of your cycle (typically day 2, 3, or 4) lets your doctor count the small follicles visible on each ovary. These antral follicles, measuring 2 to 10 millimeters, represent the pool of eggs available for that cycle. The total count across both ovaries gives a snapshot of ovarian reserve that correlates strongly with age and overall reproductive potential.

The ultrasound itself takes only a few minutes. A small probe is inserted vaginally, and your doctor measures follicle number and ovarian volume on a screen. It’s not painful for most people, though mild discomfort is common. Combined with your blood work, this count gives your doctor a much clearer picture than either test alone.

Checking the Fallopian Tubes

A hysterosalpingogram, usually called an HSG, is an X-ray procedure that checks whether your fallopian tubes are open and whether the uterine cavity has a normal shape. Your doctor inserts a thin catheter through the cervix and pushes contrast dye into the uterus. A series of X-ray images tracks the dye as it flows. If the dye spills freely out the ends of both tubes, they’re open. If it stops or pools, there’s a blockage.

HSGs are typically scheduled 7 to 10 days after the first day of your period. This timing falls before ovulation, which minimizes any chance of being pregnant during the test. Most women describe the procedure as causing brief, moderate cramping that subsides quickly. The whole thing takes about 15 to 30 minutes, and you can usually go back to normal activities the same day.

Semen Analysis for Male Partners

Male factor issues contribute to roughly half of all infertility cases, so a semen analysis is a standard part of any fertility workup. Your partner provides a sample at a clinic or andrology lab, where it’s evaluated both under a microscope and by computer.

The World Health Organization sets minimum reference values for a normal sample: at least 15 million sperm per milliliter, at least 32% moving forward progressively, and at least 4% with normal shape. These thresholds represent the fifth percentile among men whose partners conceived within 12 months, so falling below them doesn’t mean conception is impossible, but it does signal that further evaluation is warranted.

Lab testing also measures ejaculate volume, semen acidity, and what percentage of sperm are alive (vitality). These details matter because a man could have a high sperm count but poor motility, or normal motility with abnormal morphology. Only a full lab analysis captures all of those dimensions.

What About At-Home Fertility Kits?

At-home hormone kits and sperm tests have become widely available, and they can be a reasonable first look, particularly if you want some data before committing to a clinical appointment. But they have real limitations.

Home sperm tests, including smartphone-based versions, typically measure only sperm concentration. They can’t assess motility, morphology, vitality, or semen pH. You could have 100 million sperm per sample, but if none of them are moving, a home test might still read as “normal.” Lab analysis evaluates all of these factors together, which is why fertility specialists view home kits as a useful addition, not a replacement for clinical testing.

Home hormone kits for women face similar constraints. They may test AMH or LH (the hormone that triggers ovulation) from a finger-prick blood sample, and the results can be directionally helpful. But a single hormone in isolation doesn’t tell the full story. Clinical testing interprets multiple markers together, timed to specific days of your cycle, and pairs them with imaging. If your at-home results come back normal, you could still have fertility issues that the kit simply wasn’t designed to detect.

What to Expect From the Process

A complete fertility evaluation for both partners typically takes one to two menstrual cycles to finish, because some tests need to happen on specific cycle days. Blood work on days 2 or 3, an ultrasound around the same time, and an HSG about a week later means your doctor is working within a narrow scheduling window each month.

Cost varies widely depending on your insurance. Some plans cover diagnostic testing fully once you meet the standard criteria for infertility. Others exclude fertility-related care entirely. Before scheduling, ask your provider’s office to run a benefits check so you know what’s covered. Many fertility clinics also offer self-pay pricing for individual tests, which can range from under $100 for basic blood work to several hundred dollars for an HSG.

Once results are in, your doctor will walk you through what’s normal, what’s borderline, and what needs attention. For some couples, the workup reveals a clear, treatable cause. For others, results come back normal across the board, which is called unexplained infertility and accounts for a significant share of cases. Either way, having the data lets you and your doctor make informed decisions about next steps, whether that’s timed intercourse, medication to support ovulation, or a referral to a fertility specialist for more advanced options.
