The fastest way to bring a fever down is to take an over-the-counter fever reducer like acetaminophen or ibuprofen, which typically starts working within 30 to 45 minutes and reaches peak effect in about an hour. While you wait for medication to kick in, a few simple strategies can help your body shed heat more efficiently.
Why Your Body Raises Its Temperature
When your immune system detects an infection, it triggers the production of a signaling molecule called prostaglandin E2 in the hypothalamus, the brain’s thermostat. This chemical essentially resets your body’s target temperature higher, the same way you’d turn up a thermostat dial. Your body then works to reach that new set point by constricting blood vessels, generating heat through shivering, and conserving warmth. Understanding this matters because it explains why some cooling strategies backfire: if the thermostat is still set high, aggressive external cooling just makes your body fight harder to stay warm.
Fever-Reducing Medication: Your Fastest Option
Acetaminophen and ibuprofen both work by blocking prostaglandin production, which lowers the hypothalamic set point back toward normal. Oral acetaminophen begins working in 30 to 45 minutes, with peak fever reduction hitting around 30 to 60 minutes after you take it. Ibuprofen follows a similar timeline and also reduces inflammation, which can help if body aches are part of the picture.
You can alternate between the two if one alone isn’t enough. For example, take acetaminophen, then three hours later take ibuprofen, continuing to alternate while following the dosing intervals on each package. Don’t exceed the maximum daily dose of either. Avoid giving ibuprofen to infants under 6 months unless directed by a pediatrician.
Physical Cooling That Actually Works
Once you’ve taken medication and given it about 30 minutes to start lowering your internal set point, physical cooling becomes much more effective. Before that point, your body is still actively trying to reach the higher temperature, so external cooling triggers shivering and stress responses that can actually increase your metabolic rate by 35% to 40%, raise blood pressure, and spike stress hormones. Water is fine for most adults. If you’re struggling to eat or the fever has lasted more than a day, drinks with electrolytes (sports drinks diluted with water, oral rehydration solutions, or broth) help replace sodium and potassium lost through sweat. For children, pediatric electrolyte solutions are a better choice than sports drinks. Small, frequent sips work better than trying to drink a large amount at once, especially if nausea is present.
What Not to Do
Several common instincts actually slow fever reduction or create new problems:

For infants and young children, the rules are stricter. Any baby under 3 months with a rectal temperature of 100.4°F (38°C) or higher needs immediate medical evaluation, regardless of how the baby appears. For babies 3 to 6 months, a temperature above 101°F (38.3°C) or unusual irritability and lethargy warrants a call. Children 6 to 24 months with a fever above 101°F that lasts more than one day should also be seen, even without other symptoms.
