How to Get a Fever to Go Down: Meds, Rest, and Fluids

The fastest way to bring a fever down is to take an over-the-counter fever reducer like acetaminophen (Tylenol) or ibuprofen (Advil), stay hydrated, and keep your environment cool. Most fevers respond to these steps within 30 to 60 minutes. But before you rush to lower a fever, it helps to understand what’s actually happening in your body, because not every fever needs to be treated aggressively.

Why Your Body Runs a Fever

A fever isn’t a malfunction. It’s a deliberate immune response. When your body detects an infection, immune cells release signaling molecules that travel to the brain’s temperature control center. These signals trigger the production of a chemical messenger called prostaglandin E2, which raises the body’s internal thermostat to a new, higher set point. Your brain then treats your normal temperature as “too cold” and activates shivering, blood vessel constriction, and heat conservation to reach that new target.

This elevated temperature actually helps you fight infection. Research shows that fever-range temperatures enhance the activity of immune cells and make it harder for many pathogens to replicate. Suppressing a fever through physical cooling alone can blunt parts of the immune response that rely on that heat. So a mild fever, while uncomfortable, is your body doing its job. The goal of treatment is usually comfort, not eliminating the fever entirely.

Over-the-Counter Fever Reducers

Acetaminophen and ibuprofen are both effective at lowering fever. They work through different mechanisms and are processed by different organs, which is why they can be used together safely when one alone isn’t enough.

For adults, acetaminophen can be taken every four to six hours, with a maximum of 4,000 milligrams per day. Ibuprofen can be taken every six to eight hours, with a daily cap of 1,200 milligrams for over-the-counter use. Staying within these limits is important: exceeding the acetaminophen cap can damage the liver, and too much ibuprofen can irritate the stomach or affect the kidneys.

For children, dosing is based on weight rather than age. Follow the instructions on the packaging carefully, and note that ibuprofen is not recommended for babies under six months. Children’s formulations come in different concentrations (infant drops, children’s syrup, chewable tablets), so always check the label rather than estimating.

Alternating Between the Two

If a fever isn’t responding well to one medication alone, you can alternate between acetaminophen and ibuprofen. The key is to stagger them rather than taking both at the same time. For example, you might take acetaminophen, then three hours later take ibuprofen, then three hours later return to acetaminophen. This approach provides more consistent fever control because the medications peak and wear off at different times. If you’re alternating for more than three days, that’s a signal to check in with a healthcare provider, not because the combination is dangerous, but because the underlying illness may need attention.

Stay Hydrated

Fever increases your metabolic rate and causes you to lose fluids faster than normal. For every degree Celsius above 38°C (100.4°F), fluid losses through the skin rise by roughly 10%. That adds up quickly, especially if the fever lasts more than a day or is accompanied by sweating, vomiting, or diarrhea.

Water is fine for most adults. For children or anyone who has been vomiting, an oral rehydration solution or diluted electrolyte drink helps replace lost salts. Sip frequently rather than trying to drink large amounts at once. If you notice dark urine, dry lips, or dizziness when standing, you’re already behind on fluids.

Adjust Your Environment

Your instinct when feverish is often to pile on blankets, especially during the chills that come as your body temperature is climbing. But once the fever has peaked, extra layers trap heat and make it harder for your body to cool down. Wear light, breathable clothing and keep your room at a comfortable temperature. You don’t need to make the room cold, just avoid overheating it.

A lukewarm sponge bath can help if the fever is high and you want additional relief beyond medication. Use water between 90°F and 95°F (32°C to 35°C), and sponge for 20 to 30 minutes. Do not use cold water, ice, or rubbing alcohol. Cold water causes blood vessels near the skin to constrict, which actually traps heat in the body’s core and can trigger shivering, raising the temperature further. Rubbing alcohol can be absorbed through the skin and is toxic.

Rest and Give It Time

Most fevers from common viral infections peak within one to three days and resolve on their own. Your body is burning extra energy to mount an immune response, which is why you feel exhausted. Resting isn’t just about comfort. It frees up metabolic resources for your immune system. Trying to push through a fever with normal activity levels can prolong recovery.

Sleep may be difficult when you’re feverish. Taking a fever reducer before bed can help you get several hours of uninterrupted rest, which is often the most useful time to use medication even if you’re tolerating the fever during the day.

What Counts as a Fever

Normal body temperature sits in a range of 97°F to 99°F (36.1°C to 37.2°C), varying by time of day, activity level, and where you measure. A fever is generally defined as 100.4°F (38°C) or above. Temperatures between 99°F and 100.3°F fall into a gray zone sometimes called a low-grade fever, and they often don’t require treatment unless you feel miserable.

Warning Signs That Need Medical Attention

Most fevers are harmless, but certain situations call for prompt medical care.

For infants under three months, any rectal temperature of 100.4°F (38°C) or higher warrants an immediate call to the pediatrician, regardless of how the baby seems. Babies between 3 and 6 months need evaluation for temperatures above 102°F (38.9°C), or lower temperatures combined with unusual irritability or lethargy. For children between 7 and 24 months, a fever above 102°F that persists beyond one day should be evaluated.

For adults, a temperature of 103°F (39.4°C) or higher is the threshold to contact a provider. Regardless of the number on the thermometer, seek immediate care if a fever comes with any of the following: stiff neck or pain when bending the head forward, rash, confusion or altered speech, persistent vomiting, difficulty breathing, chest pain, sensitivity to bright light, seizures, or pain with urination. These can indicate serious infections like meningitis or sepsis that need treatment beyond fever management at home.

If a seizure occurs during a fever (most common in young children), call 911 if it lasts more than five minutes or if the child doesn’t recover quickly afterward.
