Getting pregnant comes down to sperm meeting egg at the right time, but the details of that timing, along with both partners’ health, make a significant difference. For most couples, the chance of conceiving in any single menstrual cycle is roughly 20 to 30 percent, so understanding the biology and optimizing the variables can shorten the path considerably.

How Conception Actually Works

After ejaculation, sperm travel through the cervix and uterus into the fallopian tubes. Along the way, the acidic environment of the vagina triggers a process that ramps up sperm motility and metabolism, essentially supercharging them for the journey. Of the millions released, only a few hundred reach the egg.

When a sperm arrives at the egg, it must break through two protective layers. Enzymes on the sperm’s surface dissolve the outer shell, allowing a single sperm to fuse with the egg. The moment that happens, the egg’s surface chemistry changes instantly, blocking all other sperm from entering. The fertilized egg, now a single cell carrying a complete set of DNA, begins dividing as it travels down the fallopian tube toward the uterus.

By about day four, the cluster of cells has formed a hollow ball. The outer layer will eventually become the placenta, and the inner cell mass becomes the embryo. Implantation into the uterine wall happens roughly six to ten days after fertilization. The hormone progesterone, produced after ovulation, thickens the uterine lining to create a nourishing environment for this implantation.

The Six-Day Fertile Window

Pregnancy is only possible during about six days of each menstrual cycle: the five days before ovulation and the day of ovulation itself. This window exists because sperm can survive inside the uterus and fallopian tubes for three to five days, while an egg is viable for only about 12 to 24 hours after release. Contrary to older estimates based on less precise tracking methods, the fertile window does not appear to extend beyond ovulation day.

For a textbook 28-day cycle, ovulation typically falls around day 14, but many women have shorter or longer cycles, and the day of ovulation can shift from month to month. That variability is exactly why tracking ovulation matters more than counting calendar days.

How to Track Ovulation

Several methods can help pinpoint when ovulation is approaching or has occurred. Using more than one at the same time gives the clearest picture.

  • Ovulation predictor kits (OPKs): These urine test strips detect the surge of luteinizing hormone (LH) that triggers ovulation. Once the surge is detected, ovulation typically follows within 12 to 36 hours, making the next day or two the most fertile time.
  • Cervical mucus: As ovulation approaches, vaginal discharge becomes clear, slippery, and stretchy, often described as resembling raw egg whites. The last day you notice this type of mucus is most likely ovulation day and will almost always fall within four days of actual ovulation.
  • Basal body temperature (BBT): After ovulation, resting body temperature rises by about 0.2 to 0.5°C (roughly 0.4 to 1°F) due to the increase in progesterone. You can detect this shift by taking your temperature first thing each morning before getting out of bed. The limitation is that BBT confirms ovulation after the fact, so it’s most useful for learning your pattern over several cycles rather than predicting the current one.

The most actionable approach is combining OPKs with cervical mucus observation. When you see fertile-quality mucus and get a positive OPK, that’s your clearest signal to have intercourse.

How Often to Have Sex

More frequent intercourse during the fertile window increases the probability of conception. A large prospective study found that couples who had sex more often during fertile days were 70 percent more likely to conceive within six months compared to those who had less frequent intercourse. The median frequency among couples actively trying was about three times per cycle in the study’s Japanese cohort, while American couples trying to conceive averaged about six times per cycle.

A practical target is every one to two days during the fertile window. There’s no strong evidence that daily sex depletes sperm quality enough to reduce your chances, so couples shouldn’t feel the need to “save up.” Having sex in the two to three days leading up to ovulation gives the best odds, since sperm are already in position when the egg is released.

Her Health Before Conception

Body weight has a measurable impact on fertility. The relationship between BMI and infertility follows a U-shaped curve: being significantly underweight or overweight both reduce the chances of conceiving. Research using national health data found that below a BMI of about 19.5, each unit decrease in BMI increased infertility risk by 33 percent. Above that point, each unit increase in BMI raised the risk by about 3 percent. A BMI in the normal range (roughly 19.5 to 25) is associated with the best ovulatory function.

Folic acid supplementation is one of the most important preconception steps. The U.S. Preventive Services Task Force gives its strongest possible recommendation (Grade A) that anyone planning to become pregnant take 400 to 800 micrograms of folic acid daily. Because the neural tube closes just 26 to 28 days after fertilization, often before a woman even knows she’s pregnant, starting supplementation early is critical.

Switching to loose-fitting boxers and avoiding prolonged heat exposure is a simple, evidence-based change.

Smoking reduces sperm count and motility, with effects proportional to how much and how long a person smokes. Chronic heavy drinking lowers testosterone, disrupts hormonal balance, and reduces semen quality. Both are worth eliminating or significantly cutting back when trying to conceive.

Obesity in men, like in women, impairs fertility. Excess body fat raises scrotal temperature, increases oxidative stress, and can trigger epigenetic changes that reduce sperm concentration and motility. Moderate exercise improves fertility markers, but extreme endurance training (marathon-level intensity) can actually lower testosterone and reduce sperm quality. The sweet spot is regular, moderate physical activity.

Prolonged psychological stress raises cortisol, which suppresses testosterone production. While “just relax” is unhelpful advice, managing chronic stress through whatever works for you (exercise, sleep, therapy) does have a biological basis in terms of protecting sperm quality.

How Age Affects Your Chances

Female age is the single strongest predictor of how quickly a couple will conceive. For women under 35, per-cycle conception rates are highest, and IVF live birth rates sit around 48 percent. Between 35 and 37, those rates drop to about 39 percent. At 38 to 40, they fall to roughly 30 percent. After 42, the rate drops below 5 percent. Natural conception rates follow similar age-related declines, though the absolute numbers differ from IVF statistics.

Male age matters too, though the decline is more gradual. Sperm quality, including DNA integrity, decreases with age, but most men remain fertile well into their 40s and beyond.

When Conception Takes Longer Than Expected

Most couples conceive within 6 to 12 months of well-timed, unprotected intercourse. The American College of Obstetricians and Gynecologists recommends a fertility evaluation if pregnancy hasn’t occurred after one year of regular unprotected sex. If the woman is over 35, that timeline shortens to six months. For women over 40, seeking an evaluation right away rather than waiting is appropriate, given the steeper decline in egg quality and quantity at that age.

Infertility affects both partners roughly equally. About one-third of cases trace to a female factor, one-third to a male factor, and one-third to a combination or unexplained causes. A basic evaluation typically includes semen analysis for the male partner and ovulation and reproductive tract assessment for the female partner. Many causes of infertility are treatable, so an earlier evaluation simply means more options and more time to act on them.
