The safest way to get earwax out at home is to soften it with drops and let your ear push it out naturally. Your ear canal has a built-in conveyor belt: skin cells on the eardrum slowly migrate outward, carrying wax and debris toward the opening at a rate of roughly 0.1 millimeters per day. For most people, this system works without any help. When it doesn’t, a few simple methods can clear things up without risking damage.
How to Tell If Earwax Is Actually a Problem
Earwax only needs to come out when it’s blocking your ear canal enough to cause symptoms. A visible bit of wax near the opening of your ear is normal and healthy. It traps dust, protects the delicate skin of the canal, and has mild antibacterial properties.
Impacted earwax, the kind that warrants removal, typically causes a feeling of fullness or pressure in the ear, muffled hearing, ringing (tinnitus), ear pain, itching, or occasionally a persistent cough triggered by a nerve branch deep in the ear canal. If you’re not experiencing any of these, you likely don’t need to do anything at all.
Softening Drops: The Best First Step
Softening the wax before trying to remove it makes every other method safer and more effective. You have several options, and none is dramatically superior to the others.
- Olive or almond oil. The NHS recommends lying on your side with the affected ear up, placing 2 to 3 drops in the canal, and staying in that position for 5 to 10 minutes. Repeat 3 to 4 times a day for 3 to 5 days. This is gentle and inexpensive.
- Mineral oil or baby oil. Works the same way. A few drops at body temperature, repeated over several days, gradually breaks down hard wax.
- Over-the-counter earwax drops. Most contain 6.5% carbamide peroxide, which releases small oxygen bubbles inside the ear canal to help loosen and lift compacted wax. Follow the package directions, which typically call for several drops twice daily for up to four days.
After a few days of softening, much of the wax will migrate out on its own. You can gently wipe away anything that reaches the outer ear with a damp cloth. If the blockage persists, you can try irrigation.
How to Irrigate Safely at Home
Irrigation means flushing the ear canal with water to wash out loosened wax. Bulb syringes designed for ear use are sold at most pharmacies. The key detail most people miss is water temperature: use water between 37°C and 40°C (about 98°F to 104°F), close to body temperature. Water that’s too cool or too warm stimulates the balance organs in your inner ear and can cause sudden dizziness or nausea.
Tilt your head so the affected ear faces slightly down over a sink or bowl. Gently squeeze the bulb to direct a slow stream of warm water toward the upper wall of the ear canal, not straight at the eardrum. The water flows in, circles behind the wax, and carries it out. You may need to repeat this several times. If it doesn’t work after a few attempts, go back to softening drops for another day or two and try again.
Never use a high-pressure stream, a showerhead, or a water pick. The ear canal is only about 2.5 centimeters long, and excessive pressure can perforate the eardrum.
What Not to Put in Your Ear
Cotton swabs are the single most common cause of earwax problems. They push wax deeper into the canal, compacting it against the eardrum instead of removing it. A Johns Hopkins review of pediatric emergency room data found at least 35 ER visits per day in children alone from cotton-swab injuries, including bleeding ear canals, perforated eardrums, and pieces of cotton lodged inside the ear. The same risks apply to adults.
Foreign objects like bobby pins, pen caps, keys, and rolled-up tissue carry the same dangers. Ear candles, which involve placing a hollow cone in the ear and lighting the other end, have no evidence of creating meaningful suction and carry a real risk of burns and dripping wax entering the canal. The ear canal’s skin is thin and easily scratched, and any break in the surface invites infection.
Hearing aids, earbuds, and earplugs don’t injure the ear the way swabs do, but they do block the canal’s natural outward migration. If you wear any of these regularly, you’re more prone to wax buildup and may need to use softening drops periodically as maintenance.
When Home Methods Won’t Work
Some situations call for a professional. Do not irrigate or use drops at home if you have, or suspect you have, a hole in your eardrum. Signs of a perforation include sudden sharp pain followed by drainage, a pop during a pressure change, or hearing loss after an ear infection. Irrigation is also not safe if you’ve had ear surgery, have an active ear infection (pain, swelling, discharge), or experience hearing in only one ear, since any complication could affect your only functional ear.
People who are immunocompromised should also be cautious with home irrigation because even a minor abrasion in the canal can lead to a serious infection. And if you’ve tried softening and irrigating for a week with no improvement, it’s time to have it done professionally.
What Professional Removal Looks Like
Clinicians typically use one of three methods, depending on the situation and what equipment is available.
Manual removal with a curette involves a small, curved instrument that the clinician slides past the wax and uses to scoop it out. It’s quick and doesn’t require water, which makes it a good option when irrigation isn’t safe. You’ll feel pressure but usually not pain.
Irrigation in a clinical setting uses a controlled, low-pressure stream of warm water, similar to the home method but with better equipment and direct visualization. The clinician can see exactly what’s happening and adjust in real time.
Microsuction uses a thin vacuum nozzle inserted into the ear canal under magnification. The clinician can see the wax the entire time and suction it out piece by piece, sometimes finishing with small forceps. Microsuction is generally considered the gentlest option and can be used even with a ruptured eardrum, a history of ear surgery, or a mild outer ear infection, situations where irrigation would be off-limits.
Most professional removals take under 15 minutes. Hearing improvement, if wax was the cause, is usually immediate.
Preventing Buildup in the First Place
The most effective prevention strategy is simply leaving your ears alone. The canal’s self-cleaning migration handles wax removal continuously. If you’re prone to buildup because of narrow ear canals, heavy wax production, or regular use of hearing aids or earbuds, a few drops of olive oil once or twice a week can keep things moving. Wipe the outer ear with a damp cloth after showering, and resist the urge to go deeper. The old advice holds: nothing smaller than your elbow should go in your ear.
