A cough that lingers for weeks usually has a treatable underlying cause, and figuring out that cause is the fastest path to relief. Coughs lasting fewer than three weeks are considered acute and typically resolve on their own after a cold or respiratory infection. Between three and eight weeks is subacute, often a post-infectious cough that fades with time. Once a cough crosses the eight-week mark, it’s classified as chronic and almost always needs investigation beyond cough drops and patience.
The Three Most Common Hidden Causes
If you don’t smoke, the vast majority of persistent coughs trace back to one of three problems: postnasal drip (sometimes called upper airway cough syndrome), a form of asthma, or acid reflux. Many people have more than one at the same time, which is why treating just one and hoping for the best often falls short.
Postnasal drip happens when excess mucus from your sinuses drains down the back of your throat, triggering the cough reflex. You might notice throat clearing, a tickle in the back of your throat, or a sensation of something dripping. Allergies, sinus infections, and irritants like dust or dry air are common triggers. Over-the-counter antihistamines and nasal saline rinses can help, and for many people this alone clears the cough within a few weeks.
Cough-variant asthma is trickier because it doesn’t always come with wheezing or shortness of breath. The only symptom may be the cough itself, often worse at night or after exercise. If your cough has a seasonal pattern, gets worse in cold air, or flares around allergens, asthma is worth investigating. A breathing test called spirometry can confirm it, and a trial of an inhaler is sometimes used as both a diagnostic tool and a treatment.
When Reflux Causes a Cough Without Heartburn
Acid reflux is one of the most overlooked causes of a persistent cough because it doesn’t always feel like reflux. A condition called laryngopharyngeal reflux (LPR), sometimes called “silent reflux,” sends stomach contents high enough to irritate the throat and airway without producing the classic burning sensation in the chest. This can trigger a cough in three ways: direct irritation of the throat and voice box by stomach acid, stimulation of the vagus nerve (which runs between the esophagus and the lungs and controls the cough reflex), and changes in how the esophagus moves, which allow acid to linger longer than it should.
Over time, repeated exposure to even small amounts of acid damages the delicate lining of the throat and makes the cough reflex hypersensitive. You may notice a hoarse voice, frequent throat clearing, or a sour taste, but some people have none of these clues. If reflux is suspected, reducing acidic and fatty foods, not eating within three hours of bedtime, and elevating the head of your bed by six inches are the starting points. Acid-reducing medications can take several weeks to fully quiet a reflux-driven cough, so patience matters.

A class of blood pressure medications called ACE inhibitors is a notorious cough trigger. Clinical data show that roughly 11% of patients taking these drugs develop a persistent dry cough, a rate about nine times higher than what drug labels historically reported. The cough can start weeks or even months after beginning the medication, which makes it easy to miss the connection. If you take a blood pressure drug with a name ending in “-pril” (like lisinopril or enalapril) and developed a cough sometime afterward, talk to your prescriber. Switching to a different type of blood pressure medication usually resolves the cough within one to four weeks.
What Actually Helps at Home
While you’re sorting out the underlying cause, several strategies can reduce cough severity day to day.
Honey is one of the best-supported home remedies. In clinical comparisons, honey performed as well as the standard over-the-counter cough suppressant (dextromethorphan) and significantly outperformed no treatment at all, reducing both nighttime coughing and sleep disruption. A spoonful of honey in warm water or tea coats the throat and may calm irritated nerve endings. This applies to adults and children over one year old; honey should never be given to infants.
When choosing an over-the-counter product, know what you’re reaching for. Cough suppressants containing dextromethorphan reduce the urge to cough and are most useful for dry, hacking coughs that disrupt sleep. Expectorants containing guaifenesin work differently: they thin mucus so your cough becomes more productive and can actually clear the irritant faster. Picking the wrong one, like suppressing a wet cough that needs to clear mucus, can slow your recovery.
Staying well-hydrated thins mucus throughout your airways. Warm liquids in particular can soothe an irritated throat and loosen congestion. A humidifier in your bedroom helps if your home air is dry, but clean it regularly since mold in a dirty humidifier can worsen a cough.
Environmental Triggers That Keep a Cough Going
Sometimes the reason a cough won’t resolve is that the irritant never goes away. Indoor and outdoor air quality play a measurable role. Research pooling data across multiple studies found that for every modest increase in fine particulate matter (PM2.5) or nitrogen dioxide exposure, the odds of chronic cough rise by 4 to 10 percent. You can’t always control outdoor air, but you can reduce indoor exposure.
Common indoor culprits include cigarette smoke (even secondhand), cleaning products with strong chemical fumes, scented candles and air fresheners that release volatile compounds, mold in bathrooms or basements, and pet dander. If your cough is consistently worse at home or at work, that’s a strong signal that something in the environment is sustaining it. Running a HEPA air purifier in the rooms where you spend the most time, fixing moisture problems, and switching to fragrance-free cleaning products are practical first steps.
What Happens at the Doctor’s Office
If your cough has lasted eight weeks or more, or if it’s been shorter but came with worrisome symptoms, a structured evaluation is the next step. Your doctor will start with a detailed history: when the cough started, what makes it worse, whether it’s dry or producing mucus, what medications you take, and what you’ve already tried. A chest X-ray is typically ordered early to rule out pneumonia, masses, or structural problems in the lungs.
From there, the approach depends on what seems most likely. If asthma is suspected, you’ll do a breathing test. If the cough produces mucus, a sample may be sent for analysis to check for infection. If reflux seems probable, a trial of acid-reducing treatment may come before any testing, since response to treatment itself can confirm the diagnosis.
When the common causes have been ruled out or treated without improvement, more specialized tests enter the picture. High-resolution CT scans can reveal bronchiectasis (damaged, widened airways), scarring in the lungs, or other structural issues invisible on a standard X-ray. A scope passed through the nose can examine the sinuses and throat directly. A 24-hour pH monitoring test, where a thin probe measures acid levels in the esophagus over a full day, is the most definitive way to diagnose silent reflux.
Symptoms That Need Prompt Attention
Most persistent coughs turn out to have benign, fixable causes, but certain accompanying symptoms change the urgency. Coughing up blood, even small amounts, warrants a call to your doctor that day. If you’re coughing up more than a few teaspoons of blood, that’s an emergency room situation.
Other symptoms that push a persistent cough into more urgent territory include unexplained weight loss, fevers or drenching night sweats, worsening shortness of breath, chest pain, and dizziness or lightheadedness. These don’t necessarily mean something serious is wrong, but they expand the list of possibilities enough that earlier testing is warranted rather than a wait-and-see approach.
