Burn scars can be significantly improved with the right combination of treatments, though the approach depends on how severe the burn was, what type of scar formed, and how long ago the injury happened. Most burn scars peak in severity around 6 months after injury and continue maturing for 12 to 18 months. That maturation window is when scars respond best to treatment, so starting early gives you the best results.
Why Healing Time Determines Your Scar
Not every burn leaves a permanent scar. Superficial burns that heal within 10 days generally produce no lasting scarring at all. Burns that take 14 to 21 days to heal carry a moderate risk. Burns that need more than 21 days to close, or that require skin grafting, are at high risk for noticeable, raised scarring.
The type of scar that forms also matters for choosing a treatment. Hypertrophic scars are thick and raised but stay within the boundaries of the original wound. They appear within one to two months, look pink to red, and often become less noticeable over time on their own. Keloid scars grow beyond the original wound edges, appear red to purple, can develop months to years after the injury, and never resolve without treatment. Keloids are also harder to treat and have a high recurrence rate. Contracture scars form when large areas of skin tighten during healing, sometimes restricting movement in joints or limbs.
Silicone Gel Sheets
Silicone sheeting is one of the most widely used first-line treatments for raised burn scars. These are thin, flexible medical-grade sheets placed directly over the scar. They work by hydrating the scar tissue and creating a controlled environment that helps flatten and soften raised areas. About 50 to 75 percent of patients see a beneficial effect, according to surveys of plastic surgeons.
For results, you need to wear them at least 12 hours a day for two to three months. They can be worn alone or underneath pressure garments. Silicone sheets also help with the intense itching that many burn survivors experience. If sheets feel impractical for your scar location, silicone gel that you apply like a cream is an alternative, though the evidence is stronger for sheets.
Pressure Garments
Custom pressure garments are elastic, tight-fitting garments designed to apply steady pressure to healing scar tissue. Research indicates that at least 15 mmHg of pressure is necessary for effectiveness. The standard recommendation is to wear them 23 out of 24 hours per day, removing them only briefly for bathing and skin care.
Pressure therapy works best when started during the active scarring phase, typically within the first few months. Even in cases where pressure garments don’t dramatically improve the scar’s appearance, they reliably decrease itching and protect fragile new skin from further injury. Your care team may also recommend custom inserts made of foam or rubber worn under the garment to increase pressure on specific areas.
Massage and Moisturizing
Regular scar massage helps soften and desensitize the tissue. When combined with stretching, it can make the scar looser and more comfortable, which is especially important for scars near joints. You can do this at home as part of a daily routine.
Moisturizing matters more than most people realize. Burn scars lose the skin’s normal ability to retain moisture, leading to dryness and intense itching. Thicker moisturizers from a tube or jar last longer on the skin and need less frequent reapplication than thinner, water-based lotions from a bottle. Apply in thin layers and massage gently, especially while scars are still fragile. You can moisturize healed areas as often as needed throughout the day.
Laser Treatment
Fractional CO2 laser therapy is one of the most effective professional treatments for burn scars. The laser creates tiny, controlled wounds in the scar tissue. Your body heals these micro-injuries by replacing dense scar collagen with more normal tissue, improving texture, flexibility, and appearance over time.
This is not a one-session fix. You’ll typically need 6 to 10 treatments to see the best possible result, with sessions spaced weeks apart to allow healing between each one. Laser treatment can improve scar thickness, color, and tightness. It’s particularly useful for scars that haven’t responded well enough to silicone or pressure therapy alone.
Steroid Injections
For raised scars that remain thick and firm, injections of corticosteroids directly into the scar tissue can help flatten them. The medication reduces inflammation and slows the overproduction of collagen that makes scars raised. Injections are typically repeated at monthly intervals until the scar improves.
This approach works better on hypertrophic scars than keloids. Keloids have a high rate of returning even after successful treatment, so injections for keloids are often combined with other therapies to reduce recurrence.
Onion Extract Products
Over-the-counter scar gels containing onion extract (the active ingredient in products like Mederma) show modest benefits. Clinical studies have found improvements in scar pigmentation, surface relief, and pliability compared to untreated scars. The results are more noticeable in people who already have some scar maturity rather than completely fresh wounds. These products won’t eliminate a scar, but they can improve its appearance as part of a broader routine that includes silicone and sun protection.
Sun Protection Is Non-Negotiable
Healing burn scars are extremely vulnerable to sun damage. UV exposure can cause permanent darkening or discoloration that becomes much harder to treat later. Sun protection is recommended for at least one full year after a burn injury, and many specialists advise longer.
Use sunscreen with SPF 30 or higher on all exposed scars. Reapply every couple of hours when outdoors. Moisturizers with built-in SPF can simplify this by combining hydration and protection in one step. When possible, plan outdoor activities for early morning or late evening, and cover scars with clothing when sun exposure is unavoidable. Lip balm with SPF 30 or higher is important if the burn affected your face.
When Surgery Becomes an Option
Surgery is reserved for severe cases where scars restrict movement or significantly affect function. Contracture scars that limit your range of motion, interfere with tendon movement, or cause joint stiffness are the clearest candidates. The aesthetic impact matters too, but functional limitation is the primary reason surgeons intervene.
It’s generally best to wait for scars to mature before operating, which takes 6 to 12 months. The exception is severe contractures that are actively causing joint problems, where waiting could lead to permanent stiffness. The specific surgical technique depends on the scar’s size and the condition of surrounding skin. Small scars can often be improved with techniques that rearrange existing skin. Larger areas may need skin grafts or tissue transferred from elsewhere on the body.
Putting a Treatment Plan Together
No single treatment works perfectly for every burn scar. The most effective approach typically layers multiple therapies. During the first year, that often means silicone sheets, pressure garments, daily moisturizing, massage, and rigorous sun protection all working together. Once the scar has matured, laser treatments or injections can address whatever thickness, tightness, or discoloration remains.
Timing matters. The 12 to 18 months after a burn is when scar tissue is most actively remodeling, which means it’s most responsive to intervention. Starting treatment during this window, rather than waiting years, consistently produces better outcomes. Older scars can still be improved with laser therapy and other professional treatments, but the gains tend to be more incremental.
