The fastest way to stop an attack in progress is high-flow oxygen or a fast-acting triptan injection, while breaking the broader cycle requires a preventive medication started early in a cluster period. Alcohol is the most reliable and well-known cluster headache trigger, but it works in a counterintuitive way. It’s not alcohol itself that causes attacks so much as the amount. Small quantities can potently and reliably provoke an attack during an active cluster period, while oddly, larger amounts sometimes don’t. Most people learn quickly to avoid alcohol entirely during a cluster period and find they can drink normally between periods with no consequences.

One puzzling finding: medications that effectively abort a cluster attack once it starts don’t prevent attacks when they’re experimentally triggered by alcohol or nitroglycerin. A handheld vagus nerve stimulator (gammaCore) offers a non-drug option for treating acute attacks. You hold the device against your neck, and it delivers mild electrical pulses to the vagus nerve through the skin.
