How to Get Rid of Genital Herpes Outbreaks

Genital herpes cannot be cured or fully eliminated from the body. The virus establishes a lifelong infection by hiding inside nerve cells near the base of the spine, where it enters a dormant state that current medications cannot reach. But while you can’t get rid of the virus itself, you can effectively manage it: reducing outbreaks to near zero, shortening flare-ups when they happen, and lowering the chance of passing it to a partner.

Why the Virus Can’t Be Eliminated

After the initial infection, herpes simplex virus travels from the skin into nearby nerve clusters called the dorsal root ganglia. Once there, it essentially goes to sleep. In this dormant phase, the virus actively suppresses its own gene activity so the immune system can’t detect it. It also becomes invisible to antiviral medications, which only work against the virus while it’s actively replicating. This is why even years of treatment won’t clear the infection.

Periodically, the virus “wakes up,” travels back along the nerve to the skin’s surface, and either causes visible sores or sheds silently with no symptoms at all. The frequency of these reactivations varies widely. Some people have multiple outbreaks a year, especially in the first year or two. Others go years without a noticeable episode. The type of virus matters too: genital infections caused by HSV-2 tend to recur more often than those caused by HSV-1, which is more commonly associated with oral cold sores but increasingly causes genital infections as well.

Antiviral Medications

Three prescription antivirals form the backbone of genital herpes treatment: acyclovir, valacyclovir, and famciclovir. All three work by blocking the virus from copying itself during active outbreaks. They don’t destroy the dormant virus, but they shorten flare-ups, reduce their severity, and can prevent many outbreaks from happening at all.

For a first outbreak, treatment typically lasts 7 to 10 days and can be extended if sores haven’t fully healed. The first episode is usually the worst, and starting medication early makes a noticeable difference in how long symptoms last and how painful they are.

For recurrent episodes, you have two strategies. Episodic therapy means keeping medication on hand and taking it at the very first sign of a flare, such as tingling, itching, or redness. The earlier you start, the shorter and milder the outbreak. Suppressive therapy means taking a low dose of antiviral medication every day, whether or not you’re having symptoms. This approach dramatically cuts the number of outbreaks per year and reduces the amount of virus shed from the skin between outbreaks, which lowers the risk of transmitting the infection to a sexual partner. Long-term daily use of all three antivirals has been studied extensively and shown to be safe.

If you’re having more than a few outbreaks per year, or if reducing transmission risk is a priority, daily suppressive therapy is the most effective single step you can take.

Managing Outbreaks at Home

During an active outbreak, keeping sores clean and dry speeds healing. Loose, breathable cotton underwear reduces friction and irritation. A cool compress or a sitz bath (sitting in a few inches of warm water) can relieve pain and itching. Over-the-counter pain relievers like ibuprofen help with discomfort.

Avoid touching sores and wash your hands if you do, since the virus can spread to other areas of skin during an active flare. Urination can sting when sores are present. Pouring warm water over the area while urinating, or urinating in a warm bath, can reduce the burning.

Outbreak Triggers and Prevention

Outbreaks happen when something disrupts the balance between the dormant virus and your immune system’s ability to keep it in check. Common triggers include physical stress on the body (illness, fatigue, overexertion), emotional stress, friction or irritation in the genital area, and a weakened immune system from poor sleep or nutrition.

While you can’t control every trigger, a few consistent habits make a difference. Getting seven to eight hours of sleep, eating regular balanced meals, exercising moderately, and limiting alcohol all support immune function in ways that help keep the virus dormant. Some people notice patterns in their own triggers over time. Tracking outbreaks alongside lifestyle factors like sleep, stress levels, and menstrual cycles can help you identify and avoid your personal triggers.

Supplements and Natural Remedies

Lysine, an amino acid available as a supplement, is one of the most commonly discussed natural remedies for herpes. Some small studies have suggested it may reduce outbreak frequency, but the evidence is inconsistent and not strong enough for mainstream medical guidelines to recommend it. The same is true for zinc, propolis (a bee product), and topical honey, all of which have limited or mixed data.

None of these remedies come close to the effectiveness of prescription antivirals. If you want to try a supplement alongside your medication, that’s a conversation to have with your provider, but supplements should not replace antiviral treatment, especially if you’re trying to reduce transmission to a partner.

Getting Tested and Confirmed

If you suspect you have genital herpes but haven’t been tested, getting the right test matters. The most accurate method is a swab taken directly from an open sore that hasn’t started crusting over. When no sores are present, blood tests can check for antibodies, but these tests have important limitations. False positives are more common with herpes blood tests than with tests for infections like chlamydia or gonorrhea. After a new exposure, it can take up to 16 weeks for antibodies to reach detectable levels, so testing too early can produce a false negative.

A confirmed diagnosis through swab testing also tells you whether you have HSV-1 or HSV-2 genitally. This distinction matters because the two types behave differently over time, and knowing which one you have helps you and your provider plan the right level of treatment.

Reducing Transmission to Partners

Three strategies work together to lower the risk of passing herpes to a sexual partner. Daily suppressive antiviral therapy reduces viral shedding significantly. Consistent condom use provides an additional layer of protection, though it doesn’t cover all potentially affected skin. Avoiding sexual contact during active outbreaks, and ideally during the prodromal phase (the tingling or itching that sometimes precedes sores), eliminates the highest-risk windows.

Combining all three approaches brings the annual transmission risk down substantially. No method is 100% effective on its own because the virus can shed from skin that looks completely normal, but layering these strategies gives both you and your partner meaningful protection.

Gene Therapy Research

The most promising avenue toward actually eliminating the virus from the body involves gene editing. Researchers have developed CRISPR-based tools that target the herpes virus’s DNA directly inside nerve cells, using a delivery vehicle (AAV2) chosen specifically because it’s good at reaching neurons. In lab models, including 3D brain tissue grown from human stem cells, these gene-editing tools drastically suppressed viral replication and significantly reduced the virus’s ability to reactivate from its dormant state. The approach cuts out sections of the viral genome, which also prevents the virus from mutating to escape treatment.

This work is still in preclinical stages. The NIH has identified a “functional cure,” meaning sustained suppression of viral shedding without ongoing medication, as a realistic near-term goal, even if complete eradication of the virus remains further off. Several research groups are working toward clinical trials, but no gene therapy for herpes is available to patients yet.

For now, the practical reality is that genital herpes is a manageable condition. With daily antiviral therapy, most people experience few or no outbreaks, and the infection’s impact on daily life and relationships can be minimal.
