Most heart palpitations are harmless and stop on their own, but you can often speed things along with simple physical techniques, lifestyle adjustments, or both. That fluttering, pounding, or skipped-beat sensation usually comes from premature heartbeats or a temporarily elevated heart rate, and the triggers are more manageable than you might expect.
Vagal Maneuvers: Stopping Palpitations in the Moment
The fastest way to interrupt a palpitation episode is a vagal maneuver. These are physical actions that stimulate your vagus nerve, a long nerve running from your brainstem to your abdomen that acts as a brake on your heart rate. When activated, it slows the electrical signals controlling your heartbeat. Vagal maneuvers have a 20% to 40% success rate for converting a fast heart rhythm back to a normal one, and they work within seconds when they do.
The most well-studied option is the Valsalva maneuver. Lie on your back, take a deep breath, then try to exhale forcefully with your mouth and nose closed for 10 to 30 seconds. It should feel like blowing hard into a blocked straw. A modified version works even better: after the forced exhale, immediately bring your knees to your chest or raise your legs in the air and hold that position for 30 to 45 seconds.
The diving reflex is another reliable technique. While sitting, take several deep breaths, hold one, and plunge your entire face into a bowl of ice water for as long as you can tolerate. If that sounds unpleasant, pressing a bag of ice or an ice-cold wet towel against your face triggers the same reflex. Your body responds to the cold by rapidly slowing your heart rate.
Other options include forceful coughing, bearing down as if straining during a bowel movement, or even doing a handstand against a wall for 30 seconds. If one maneuver doesn’t work, wait a minute and try a different one.
Breathing Techniques That Lower Heart Rate
The same brain regions that regulate breathing also control heart rate, and the two systems stay closely synchronized. Slow, deliberate breathing activates your body’s calming response and can bring a racing heart back down.
Box breathing is the simplest pattern to remember: inhale for four counts, hold for four counts, exhale for four counts, hold for four counts. Repeat the cycle up to four times. The 4-7-8 technique takes this further by extending the exhale. Inhale for four seconds, hold for seven, then exhale slowly for eight seconds through pursed lips. If holding your breath that long feels uncomfortable, cut the pattern in half (two seconds in, 3.5 seconds hold, four seconds out) and build up gradually. Both techniques lower blood pressure and heart rate within minutes.
Caffeine, Alcohol, and Other Dietary Triggers
Caffeine is the most commonly suspected trigger, though the relationship is more nuanced than most people think. Large studies have found that moderate coffee consumption (two to three cups per day) does not increase the risk of atrial fibrillation and may even slightly lower it. However, about 25% of people with atrial fibrillation identify coffee as a personal trigger. If you notice a clear pattern between your intake and your symptoms, cutting back is reasonable.
What caffeine does at higher doses is flood your system with stress hormones. Around 250 mg, roughly three cups of coffee, your levels of adrenaline spike by over 200%. Energy drinks are a particular concern because they combine high caffeine loads with other stimulants. Even in healthy people, more than one energy drink per day may be harmful.
Alcohol is a more consistent trigger. It directly irritates the heart’s electrical system and contributes to dehydration, both of which promote palpitations. If you’re prone to episodes, reducing or eliminating alcohol is one of the highest-yield changes you can make.
Stay Hydrated to Reduce Episodes
Dehydration is an underappreciated cause of palpitations. When your blood volume drops, your heart has less fluid to pump with each beat. It compensates by beating faster and harder, which you feel as pounding or racing. Dehydration also disrupts the balance of electrolytes like potassium, magnesium, and sodium. These minerals are essential to your heart’s electrical system, and even mild imbalances can provoke irregular rhythms.
There’s no universal daily water target that prevents palpitations, but a practical rule is to drink enough that your urine stays pale yellow. If you exercise heavily, sweat a lot, or drink alcohol, you’ll need more. Adding an electrolyte source (a pinch of salt in water, coconut water, or an electrolyte tablet) can help if you’ve been losing fluids.
Sleep, Stress, and the Nervous System
Sleep deprivation directly increases the frequency of premature heartbeats. Animal research has shown that 72 hours of total sleep deprivation significantly raises the rate of premature ventricular contractions, the type of early beat that often causes that “skipped” or “flopping” sensation. You don’t need to go three days without sleep to notice the effect. Even a few consecutive nights of poor rest can heighten your body’s stress response enough to make palpitations more frequent.
Chronic stress and anxiety work through the same pathway. They keep your sympathetic nervous system, the “fight or flight” system, running on high, which primes the heart for extra beats. Regular physical activity, consistent sleep schedules, and daily breathing exercises all help shift the balance back toward the calming side of your nervous system. The goal isn’t to eliminate stress but to build habits that prevent it from becoming the default state your body operates in.
Tracking Your Palpitations
If palpitations happen often enough to concern you, keeping a simple log helps enormously. Note the time of day, what you were doing, what you ate or drank in the preceding hours, how much sleep you got the night before, and how long the episode lasted. Patterns often emerge quickly: late-afternoon episodes after skipping lunch, morning episodes after a night of poor sleep, or post-meal episodes after large carbohydrate-heavy meals.
Consumer wearables like the Apple Watch and KardiaMobile can record a single-lead ECG during an episode, giving you something concrete to show a doctor. These devices are reasonably accurate. A review of nine studies found single-lead ECG devices achieved pooled sensitivity of about 91% and specificity of about 96% for detecting atrial fibrillation. KardiaMobile specifically reached 94% sensitivity and 97% specificity in pooled data. Apple Watch numbers are similar, though its real-world accuracy drops to around 90% sensitivity and 85% specificity when you include readings the algorithm marks as inconclusive.
These devices are useful screening tools, not replacements for medical-grade monitoring. If your doctor needs more data, a Holter monitor records continuously for one to 14 days, while event monitors and mobile cardiac telemetry devices can track your rhythm for up to 30 days. The longer monitoring window makes it far more likely to catch an episode that only happens a few times a week.
Red Flags That Need Emergency Attention
Most palpitations are benign, but certain combinations of symptoms signal something more serious. Call emergency services immediately if palpitations won’t stop on their own or if they’re accompanied by passing out, chest pain or pressure that spreads to your neck, jaw, arms, or upper back, or significant difficulty breathing. These symptoms together can indicate a dangerous arrhythmia or a cardiac event that requires urgent treatment.
