How to Get Rid of Scalp Pimples: Causes & Treatments

Scalp pimples are usually inflamed hair follicles, and they respond well to a combination of the right shampoo ingredients, better hair care habits, and patience. Most cases clear up within a few weeks of home treatment, though persistent or painful bumps may need stronger intervention.

What’s Actually Causing Your Scalp Pimples

What looks like acne on your scalp is most often folliculitis, a condition where hair follicles become inflamed and infected. The culprit is usually Staphylococcus aureus bacteria, but fungi, product buildup, and excess oil can all trigger it. The bumps tend to be itchy and sometimes pus-filled, clustering along the hairline, at the crown, or wherever sweat and oil accumulate.

There’s an important distinction worth knowing. Bacterial folliculitis responds to antibacterial treatments, while fungal folliculitis (caused by an overgrowth of Malassezia yeast, which lives on the skin of 75 to 98 percent of healthy people) does not respond to standard acne treatments at all. If you’ve been using regular acne products without improvement, a fungal cause is worth considering. The bumps from fungal folliculitis tend to be uniform in size and intensely itchy, while bacterial bumps vary in size and are more likely to be tender or painful.

Medicated Shampoos That Work

The fastest route to clearing scalp pimples is switching to a shampoo with an active ingredient that targets the underlying cause. You have several options depending on what type of bumps you’re dealing with.

Salicylic acid (0.5% to 2%) breaks down the oil and dead skin cells plugging your follicles. It works best for pimples driven by excess sebum and clogged pores. Start by using it once daily and increase to two or three times daily as your scalp tolerates it. Lather the shampoo and let it sit on your scalp for a minute or two before rinsing so the active ingredient has time to penetrate.

Benzoyl peroxide (2.5% to 10%) kills bacteria directly. It’s effective for red, inflamed, or pus-filled bumps. Be aware that it can bleach towels, pillowcases, and colored hair, so rinse thoroughly and start with a lower concentration to see how your scalp reacts.

Ketoconazole shampoo targets fungal folliculitis specifically. It works by disrupting an enzyme that fungi need to maintain their cell structure, slowing or stopping their growth entirely. If your bumps are uniform, very itchy, and haven’t responded to antibacterial treatments, this is the ingredient to try. It’s available over the counter at 1% strength and by prescription at 2%.

Selenium sulfide and ciclopirox are two other antifungal options found in anti-dandruff shampoos. They’re worth trying if ketoconazole irritates your scalp or isn’t available.

A Simple Daily Routine

Wash your hair daily or whenever it gets greasy, especially after exercise or sweating. Sweat trapped under hats, helmets, or thick hair creates a warm, moist environment where bacteria and yeast thrive. When you shampoo, focus on your scalp rather than the lengths of your hair. Use your fingertips (not your nails) to gently work the lather across your entire scalp, giving the active ingredients at least 60 seconds of contact time.

Between washes, avoid touching or picking at the bumps. Squeezing scalp pimples pushes bacteria deeper into the follicle and can turn a minor bump into something that scars. If a pimple is draining on its own, keep the area clean and let it heal.

Check Your Hair Products

Heavy styling products are one of the most common and overlooked causes of scalp pimples. Pomades, waxes, thick serums, and oil-based leave-in conditioners can coat the scalp and block follicles. Ingredients with high viscosity and high melting points relative to skin temperature are the worst offenders.

Some specific ingredients to watch for on labels: coconut oil, argan oil, avocado oil, beeswax, grapeseed oil, corn oil, cottonseed oil, and almond oil. These are all rated as comedogenic, meaning they’re prone to clogging pores. Switch to products labeled oil-free, noncomedogenic, or non-acnegenic. If you use dry shampoo, don’t let it build up for days without washing it out.

Also wash anything that regularly touches your scalp. Hats, headbands, pillowcases, and helmet liners collect oil, sweat, and product residue that gets reintroduced to your skin every time you wear or use them. Pillowcases should be changed at least weekly, and hats should be washed regularly if you wear them often.

Tea Tree Oil as a Natural Option

Tea tree oil has genuine antifungal and antibacterial properties. A clinical study found that a shampoo containing 5% tea tree oil reduced dandruff by 41% after four weeks of daily use, and the same anti-inflammatory effects can help with folliculitis. Never apply undiluted tea tree oil directly to your scalp. Mix 5 milliliters of tea tree oil per 100 milliliters of carrier (a gentle shampoo or a carrier oil like jojoba), or buy a shampoo that already contains it at around 5% concentration.

Tea tree oil works best as a complement to other treatments rather than a standalone fix for moderate or severe cases. It’s a reasonable first step for mild, occasional bumps.

When Home Treatment Isn’t Enough

Most scalp pimples respond to the strategies above within two to four weeks. If yours don’t improve, get worse, or keep coming back, a dermatologist can determine whether you need prescription-strength treatment. For extensive bacterial folliculitis, oral antibiotics taken for about 10 days can clear the infection when topical products can’t reach deep enough.

There are also a few warning signs that your scalp bumps are something more serious than typical folliculitis. If you notice hair growing in tufts (several strands emerging from a single follicle, resembling toothbrush bristles), bald patches forming where bumps have healed, or scarring on your scalp, you may have a condition called folliculitis decalvans. This is a rare, chronic form of scalp inflammation that causes permanent hair loss with scarring and requires professional treatment. A dermatologist can examine your scalp with magnification and take cultures to identify the exact cause and guide treatment.
