Most skin pigmentation fades with the right combination of sun protection, topical treatments, and patience. The catch: your skin replaces itself roughly every 28 to 40 days, so even the most effective treatments need several weeks to show visible results, and deeper pigmentation can take 3 to 24 months to fully resolve. The approach that works best depends on what type of pigmentation you’re dealing with and how deep the excess melanin sits in your skin.
Identify Your Type of Pigmentation
Not all dark patches respond to the same treatments. The three most common types have different causes, which matters when choosing how to treat them.
Sunspots (solar lentigines): Brown, tan, or black spots that show up on sun-exposed areas, especially the face and hands. They develop because your skin produces extra melanin after years of UV exposure. These tend to be the most straightforward to treat because the pigment usually sits close to the skin’s surface.
Post-inflammatory hyperpigmentation (PIH): Dark spots or patches left behind after acne, eczema, burns, cuts, or any skin injury that triggered inflammation. PIH can appear anywhere on the body and ranges from pink to dark brown depending on your skin tone. The pigment can be superficial or deep, which affects how long it takes to clear.
Melasma: Large, symmetrical patches of darkened skin, most often on the forehead, cheeks, and upper lip. Hormonal changes during pregnancy, from birth control pills, or hormone therapy commonly trigger it. Melasma is the most stubborn form of pigmentation because the pigment often extends into deeper skin layers and tends to recur.
Sunscreen Is the Foundation
No pigmentation treatment will work if you skip sun protection. UV exposure triggers melanin production, which means unprotected skin will keep darkening even while you’re actively treating it. A broad-spectrum SPF 50 is the minimum for anyone working on pigmentation.
Here’s something most people miss: visible light (the kind from sunlight that you can actually see) also worsens hyperpigmentation, particularly in medium to dark skin tones. Standard sunscreens only block UV rays. A 12-week study of women with Fitzpatrick skin types III through VI found that adding an iron oxide-containing product to a daily SPF 50 routine led to measurable improvements in skin tone and texture. Among melasma patients, 36% of those using iron oxide saw superior gains in skin radiance, compared to zero in the SPF-only group. Look for tinted sunscreens or mineral foundations that list iron oxides in the ingredients.
Topical Treatments That Work
Topical products are the first line of treatment for most pigmentation. They work by slowing melanin production, speeding up cell turnover, or both. Expect to use them consistently for at least 8 to 12 weeks before judging results.
Retinoids
Prescription-strength retinoids accelerate the rate at which your skin sheds old, pigmented cells and replaces them with new ones. Significant improvements in skin tone typically become apparent between months 3 and 6 of consistent use. Over-the-counter retinol works on the same principle but at lower potency, so results take longer. Start slowly (two to three nights per week) to avoid irritation, which can actually trigger more pigmentation, especially in darker skin tones.
Azelaic Acid
Available over the counter in concentrations up to 12% and by prescription at 15%, azelaic acid decreases inflammation and speeds up cell turnover simultaneously. A systematic review comparing 20% azelaic acid to 4% hydroquinone for melasma found comparable results on objective response scales, making it a solid option for people who want to avoid hydroquinone or need a gentler long-term treatment. It’s also effective for PIH left by acne, since it treats both the breakouts and the dark marks they leave behind.
Tranexamic Acid
Tranexamic acid has become one of the more promising options for stubborn melasma. It’s available as a topical serum (look for concentrations around 5 to 10%) or, in more resistant cases, as an oral medication prescribed at doses between 500 and 1500 mg daily. A split-face study found that combining microneedling with 10% tranexamic acid produced statistically significant improvement compared to microneedling alone. Topical versions are widely available in over-the-counter serums and are generally well tolerated.
Cysteamine
Cysteamine cream is a newer option that works differently from traditional brightening agents. In a head-to-head study against hydroquinone, 5% cysteamine showed gradual but accumulating improvement over 16 weeks. Hydroquinone produced faster initial results (noticeable advantages at weeks 4 and 8), but the gap between the two narrowed significantly by week 16. The practical takeaway: hydroquinone works faster for short-term treatment bursts, while cysteamine may be better suited for longer-term maintenance since it doesn’t carry the same safety concerns about prolonged use.
Alpha Hydroxy Acids
Glycolic acid and other AHAs speed up your skin’s natural exfoliation process, helping pigmented cells shed faster. At lower concentrations (5 to 15%), they work well as daily-use products that gradually improve tone. At professional-strength concentrations (around 70%), glycolic acid functions as a medium-depth chemical peel that causes dispersion of melanin while also stimulating collagen production. Over-the-counter products are a good complement to other treatments on this list.
Professional Treatments
When topical products aren’t enough, in-office procedures can target pigment more aggressively. These come with more risk but also faster results for some people.
Chemical Peels
Professional chemical peels use higher concentrations of acids to remove pigmented skin layers in a controlled way. Medium-depth peels penetrate through the outer skin and into the upper dermis, which allows them to reach pigment that surface-level products can’t touch. Glycolic acid at 70% and trichloroacetic acid (TCA) at 30% are both used as medium-depth peels. TCA peels are sometimes combined with other solutions for better outcomes with fewer side effects. Multiple sessions are typically needed, spaced several weeks apart.
For darker skin tones, chemical peels require extra caution. They are generally considered the most effective professional treatment for hyperpigmentation, but in very dark skin, they carry a meaningful risk of causing the opposite problem: lighter patches (hypopigmentation) that can be permanent. Superficial peels at lower concentrations are safer for darker complexions.
Laser Treatments
Picosecond lasers achieve fair improvement (25 to 49% reduction) in about 3 to 4 sessions on average, compared to older Q-switched lasers, which require more sessions to reach similar results. Neither laser type works as well on pigment sitting deep in the dermis.
The biggest risk with lasers is rebound hyperpigmentation, where the treatment itself triggers new dark patches. This is especially common in Asian skin and darker skin tones. Recurrence rates for pigmentation after laser treatment are high across the board, and mottled light spots can also develop. Laser therapy is generally considered less effective than chemical peels for pigmentation and should be approached cautiously in darker skin, where the risk of making things worse is real.
Special Considerations for Darker Skin
If you have medium to dark skin, the stakes are higher with any pigmentation treatment. The same melanin-producing cells you’re trying to calm down can overreact to irritation, inflammation, or trauma from aggressive treatments, creating new dark patches on top of the ones you’re trying to fix. This is why gentle, consistent topical treatment with strong sun protection is often a better strategy than jumping to lasers or deep peels.
Azelaic acid, tranexamic acid, and retinoids are all effective in darker skin tones with a lower risk of backfiring. One clinical trial of a retinoid-based treatment found that Black and Hispanic participants experienced less hyperpigmentation compared to a control group over 12 weeks. Whatever you use, introduce it gradually. If a product causes redness, stinging, or peeling beyond what’s expected, scale back. The inflammation itself can leave marks that outlast the original pigmentation.
Realistic Timelines
Superficial pigmentation from a recent breakout or mild sun damage may start fading within a few weeks of consistent treatment. Deeper or more established pigmentation follows a slower path. Post-inflammatory hyperpigmentation can take 3 to 24 months to disappear entirely, and some cases take even longer. Melasma is a chronic condition for many people, meaning treatments can improve it dramatically but it often returns with hormonal changes or sun exposure.
The most common mistake is stopping treatment too early because results aren’t visible yet. Your skin needs to cycle through multiple rounds of cell turnover before deeper pigment reaches the surface and sheds. Combining approaches (a retinoid plus azelaic acid plus diligent sunscreen, for example) tends to produce better results than relying on a single product. Layer in treatments one at a time over several weeks so you can identify what’s working and catch any irritation before it causes new pigmentation.
