Swollen ankles usually result from fluid pooling in the lower legs due to gravity, and the fastest way to start reducing them is to lie down and elevate your legs above heart level for 15 minutes. That simple position change encourages fluid to drain back toward your core. But lasting relief typically requires a combination of strategies: elevation, movement, compression, and dietary changes working together.
Elevate Your Legs the Right Way
Elevation works because it reverses the gravitational pull that traps fluid in your ankles. The key detail most people miss is height: your legs need to be above the level of your heart, not just propped on an ottoman. Lying on your back with pillows stacked under your calves and feet is the simplest setup. A recliner that tilts back far enough can also work.
Aim for about 15 minutes per session, three to four times a day. If you work at a desk or stand for long stretches, even short elevation breaks during lunch or after work can make a noticeable difference by the end of the day.
Use Your Calf Muscles as a Pump
Your calf muscles act as a natural pump for pushing fluid and blood back up toward your heart. When you sit or stand still for hours, that pump barely activates, and fluid settles into your ankles. Two types of movement help the most: ankle pumps for when you’re stuck sitting, and walking for when you can get up.
Ankle pumps are simple. Sit or lie down with your legs extended in front of you. Point your toes toward your knees as far as they’ll go, then point them away from you. Alternate back and forth for two to three minutes, repeating two to three times per hour. This rhythmic flexing activates the calf muscles enough to push fluid upward, and you can do it at your desk, on a plane, or in bed.
Walking is even more effective because each step engages the full calf muscle. If your ankles tend to swell during the workday, a five-minute walk every hour or two does more than a single long walk at the end of the day. The goal is to avoid prolonged stillness in any position.
How Compression Socks Help
Compression socks apply graduated pressure to your lower legs, squeezing tightest at the ankle and gradually loosening toward the knee. This prevents fluid from accumulating in the first place and supports the upward flow of blood and lymph.
Compression is measured in millimeters of mercury (mmHg), and the right level depends on how much swelling you have. For mild, everyday ankle puffiness, 15 to 20 mmHg provides light support and is available without a prescription. For more persistent or moderate swelling, 30 to 40 mmHg offers firmer compression but is best chosen with guidance from a clinician. Levels above 40 mmHg are reserved for severe cases involving significant tissue changes and always require professional fitting.
Put compression socks on first thing in the morning, before gravity has had a chance to pull fluid down into your ankles. If you wait until your ankles are already swollen, the socks are harder to get on and less effective. Avoid compression entirely if you have an active skin infection, suspected blood clots, significant arterial disease in your legs, or severe numbness in your feet.
Reduce Your Sodium Intake
Sodium causes your body to hold onto water, and excess fluid has to go somewhere. In people who are on their feet a lot or have circulatory issues, that “somewhere” is often the ankles. Cutting back on salt is one of the most effective long-term strategies for reducing chronic swelling.
The American Heart Association recommends less than 1,500 mg of sodium per day for the general population. For context, a single fast-food meal can easily contain 1,200 to 1,800 mg. The biggest sources of hidden sodium aren’t the salt shaker on your table. They’re processed foods: canned soups, deli meats, frozen meals, bread, condiments, and restaurant dishes. Reading nutrition labels and cooking more meals at home gives you the most control.
Drinking more water, counterintuitively, can also help. When you’re well-hydrated, your kidneys are better able to flush excess sodium. Pairing lower sodium intake with adequate water, along with potassium-rich foods like bananas, leafy greens, and sweet potatoes, supports your body’s fluid balance from the inside.
Try Gentle Self-Massage
A specific type of self-massage called manual lymphatic drainage can help move trapped fluid out of swollen ankles. It’s different from a regular deep-tissue massage. Lymph fluid flows just under the skin, so the pressure should be very light, just enough to gently stretch the skin. Firm or deep pressure actually blocks the flow.
Start by placing your hands behind your ankle bones and gently stretching the skin upward toward your calf. Hold for three seconds, then release. Next, place your hands on top of your foot near your toes and stretch the skin toward the ankle. Hold for three seconds and release. Finally, wrap your fingers around each toe individually and gently pull the skin toward the ankle, holding for three seconds each. Repeat the full sequence about 10 times. The rhythm of stretch, hold, and release is what drives the fluid toward larger lymph vessels that can drain it.
This technique works well as a complement to elevation. Lie back with your legs raised above heart level and massage your ankles and feet while the gravity assist is already working in your favor.
Common Causes Worth Understanding
Swollen ankles aren’t a condition on their own. They’re a symptom, and knowing the cause helps you pick the right approach. The most common and least concerning cause is simple gravity and inactivity: sitting for a long flight, standing all day at work, or spending an evening on the couch. Hot weather also dilates blood vessels and worsens fluid retention.
Pregnancy causes ankle swelling in most women, particularly in the third trimester, due to increased blood volume and pressure from the growing uterus on pelvic veins. Hormonal shifts during the menstrual cycle can have a similar but milder effect. Certain medications, including calcium channel blockers for blood pressure, some diabetes drugs, and steroids, list ankle swelling as a common side effect. If the timing of your swelling lines up with starting a new medication, that connection is worth exploring with your prescriber.
More serious causes include heart failure, where the heart can’t pump efficiently enough to return fluid from the legs; kidney disease, where the body can’t filter out excess fluid; and liver disease, which reduces production of proteins that keep fluid inside blood vessels. These conditions cause swelling that tends to be persistent, bilateral, and accompanied by other symptoms like fatigue, shortness of breath, or changes in urination.
Red Flags That Need Prompt Attention
Most ankle swelling is benign, but certain patterns signal something more urgent. Swelling in only one leg, especially when paired with pain or cramping in the calf, redness or purple discoloration, and warmth in that leg, can indicate a deep vein thrombosis (a blood clot in a deep vein). This requires prompt medical evaluation because of the risk that the clot could travel to the lungs.
If you develop sudden shortness of breath, chest pain that worsens when you breathe deeply or cough, dizziness, fainting, a rapid pulse, or you cough up blood, those are signs of a pulmonary embolism, which is a medical emergency. Ankle swelling that develops suddenly alongside rapid weight gain and difficulty breathing can also point to a worsening heart or kidney problem that needs same-day evaluation.
