How to Heal a Fractured Wrist Faster and Stronger

Most fractured wrists heal within 6 to 8 weeks with proper immobilization, though certain bones in the wrist can take 12 weeks or longer. How your wrist heals depends on which bone you broke, whether the pieces shifted out of alignment, and what you do during recovery. The good news is that the vast majority of wrist fractures heal completely, and there’s a lot you can do at home to support that process.

What Happens Inside Your Wrist as It Heals

Bone repair follows a predictable sequence. In the first 24 hours, inflammation surges around the break. This sounds like a problem, but it’s actually the repair process launching: your body sends blood and immune cells to the site to clear debris and lay the groundwork for new bone. This acute inflammatory phase wraps up within about 7 days.

Next, your body builds a soft bridge of cartilage across the fracture gap, called a callus. This rubbery tissue stabilizes the break and acts as scaffolding. Over the following weeks, the soft callus gradually hardens as minerals are deposited into it, turning it into woven bone. By about 6 to 8 weeks, this hard callus is typically strong enough for you to start using your wrist again.

The final phase, remodeling, begins around 3 to 4 weeks but continues quietly for months or even years. During this stage, your body reshapes the rough woven bone into smooth, organized bone that matches the original structure. You won’t feel this happening, but it’s why full strength and function can continue improving long after the cast comes off.

Cast or Surgery: How Treatment Is Decided

The most common wrist fracture is a break at the end of the radius, the larger forearm bone. If the bone cracked but the pieces stayed in place (a stable, nondisplaced fracture), a cast or splint is usually all you need. You’ll wear it for roughly 6 weeks while the bone knits back together.

Surgery becomes necessary when the broken pieces have shifted, the fracture extends into the joint surface, or the break is unstable enough that it’s likely to slip out of alignment inside a cast. In these cases, a surgeon realigns the bone and holds it in place with a metal plate and screws. Fractures that involve the joint surface or result in multiple bone fragments almost always require surgical fixation because they heal poorly without it. Your surgeon will also weigh your activity level and overall health when making this decision.

Scaphoid Fractures: A Special Case

The scaphoid is a small, cashew-shaped bone on the thumb side of your wrist, and it’s notorious for slow healing. About 80% of its blood supply enters from one direction and has to travel backward through the bone, which means the far end (the proximal pole) gets very little direct blood flow. Without adequate blood, bone can’t repair itself efficiently.

A nondisplaced scaphoid fracture typically requires 10 to 12 weeks of immobilization, sometimes extending to 16 weeks if healing is uncertain. Percutaneous screw fixation, a minimally invasive surgery where a small screw is placed through a tiny incision, can shorten that timeline to about 9 weeks compared to nearly 14 weeks in a cast alone. Because of the blood supply issue, scaphoid fractures carry a higher risk of nonunion, where the bone simply fails to heal.

Home Care in the First Few Weeks

What you do at home matters more than most people realize. In the early days, swelling is your biggest source of pain and stiffness, and controlling it speeds up everything that follows.

Keep your wrist elevated above your heart as much as possible, especially in the first 48 to 72 hours. That means propping your arm on pillows while sitting or lying down so your hand is higher than your chest. Apply ice with a cloth or towel between the ice and your skin for 10 to 20 minutes at a time, every hour or two. Don’t ice directly on bare skin or leave it on longer, as this can damage tissue.

Even while your wrist is immobilized, keep your fingers, elbow, and shoulder moving. Gentle finger wiggling and making loose fists helps prevent stiffness from creeping into joints that aren’t injured. It also promotes circulation, which brings the nutrients your bone needs to rebuild.

Exercises After the Cast Comes Off

Once your doctor confirms enough healing has occurred, rehabilitation exercises become essential. Your wrist will be stiff and weak, and your grip strength will have dropped significantly. This is normal. The goal is to gradually restore range of motion first, then build strength.

A straightforward starting exercise is wrist flexion and extension. Rest your forearm on a table with your hand hanging over the edge, palm down. Bend your wrist upward, gently close your hand into a fist, then lower it back down and relax your fingers. Hold each position for about 6 seconds and repeat 8 to 12 times.

Hand flips are another early exercise: sit with your forearm resting on your thigh, palm down. Flip your hand over so your palm faces up, then flip it back down. Repeat 8 to 12 times. This motion restores the forearm rotation you need for everyday tasks like turning a doorknob or using a screwdriver. Radial and ulnar deviation, where you hold your hand out palm-down and tilt it side to side (toward your thumb, then toward your pinky), helps restore the last degrees of wrist mobility.

Start slowly. Some discomfort is expected, but sharp pain means you’re pushing too hard. Most people regain functional use within 3 to 4 months, though full strength can take 6 months or more.

Nutrition That Supports Bone Repair

Your body needs raw materials to build new bone. Calcium, vitamin D, and protein are the three most important nutrients during fracture recovery. A reasonable daily target is at least 1,000 mg of calcium, 800 IU of vitamin D, and about 1 gram of protein per kilogram of body weight (for a 150-pound person, that’s roughly 68 grams of protein per day).

Dairy products, leafy greens, and fortified foods cover calcium for most people. Vitamin D is harder to get from food alone, so a supplement is often practical, particularly if you’re spending more time indoors during recovery. Protein supports the collagen framework that calcium crystals attach to, so skimping on it can genuinely slow healing. Eggs, fish, poultry, beans, and Greek yogurt are all solid sources.

What Slows Down Healing

Smoking is the single biggest controllable risk factor for poor bone healing. Smokers have 2.2 times the risk of delayed union or nonunion compared to nonsmokers, and on average, their fractures take nearly 28 extra days to heal. Nicotine constricts blood vessels and reduces oxygen delivery to the fracture site, directly undermining the repair process. If there’s ever a reason to quit or at least pause, a fracture is it.

Over-the-counter pain relievers also deserve attention. Common anti-inflammatory drugs like ibuprofen and naproxen have long been suspected of interfering with bone healing. A meta-analysis of clinical trials found that short-term use (under two weeks) does not significantly increase the risk of nonunion. However, using these medications for longer than two weeks was associated with a more than fivefold increase in nonunion risk. One specific anti-inflammatory, indomethacin, showed a significantly higher nonunion rate regardless of duration. For pain management during fracture recovery, acetaminophen is a safer default, and if you do use ibuprofen or naproxen, keep it brief.

Bone Stimulators: Do They Work?

Low-intensity pulsed ultrasound (LIPUS) devices are small, portable units that deliver painless sound waves through your skin to the fracture site, typically for 20 minutes a day. They’re sometimes prescribed for fractures that are healing slowly or have stalled entirely.

For fresh fractures, studies have shown LIPUS can accelerate healing by 34 to 39%. For fractures that have failed to heal after 9 months (nonunions), the device has achieved success rates between 70 and 86% in clinical studies. Research on scaphoid fractures specifically found a 31% acceleration in bone formation, with 70% of cases going on to heal. Your doctor may recommend one if your fracture isn’t progressing on schedule or if you have risk factors for delayed healing.

Warning Signs That Need Immediate Attention

Most wrist fractures heal without complications, but a small number develop problems that require urgent care. Compartment syndrome occurs when swelling inside the cast or deep tissue builds pressure to the point where it cuts off blood flow. The hallmark symptom is pain that is far worse than you’d expect from the injury itself, pain that does not improve with elevation or pain medication. Other red flags include numbness, tingling, weakness, pale skin, or an inability to move your fingers.

If pain under your cast is increasing rather than gradually improving, especially in the first few days, don’t wait it out. This can progress quickly, and the treatment is straightforward: the cast is cut open to relieve pressure. Catching it early prevents permanent damage to muscles and nerves.
