You can’t fully heal a sprained ankle overnight. A sprain means the ligaments in your ankle have been stretched or torn, and even the mildest version takes one to three weeks to repair. More severe sprains with partial tears need three to six weeks, and a complete rupture can take several months. But what you do in the first 12 to 24 hours has a major impact on how quickly you recover, how much pain you feel, and whether you avoid complications that drag healing out even longer. Here’s how to make the most of that first night and the days that follow.
What to Do in the First Few Hours
The current best practice for soft tissue injuries is called the PEACE protocol, which replaces the older RICE method. The key steps for that first night are protect, elevate, and compress.
Protect the ankle. Stop putting weight on it and restrict movement for the first one to three days. This minimizes bleeding inside the joint, prevents the injured ligament fibers from stretching further, and reduces the risk of making things worse. That said, total rest beyond a few days can actually weaken the healing tissue, so protection is a short-term strategy.
Elevate your leg above your heart. Prop your ankle on pillows while lying down so it sits higher than your chest. This uses gravity to drain fluid away from the injured area and limit swelling. If you’re sitting upright, even resting your foot on an ottoman or coffee table helps, though lying down with pillows is more effective. Keep it elevated as much as possible through the night.
Compress the ankle. Wrap it with an elastic bandage or use compression taping. External pressure helps limit swelling inside and around the joint and tends to improve comfort. Wrap firmly but not so tight that your toes tingle, turn white, or go numb.
Rethink Ice and Anti-Inflammatories
This is where the advice has shifted from what most people expect. Ice and ibuprofen have been the go-to moves for decades, but the evidence behind both is weaker than you’d think.
There is no high-quality evidence that ice actually speeds healing of soft tissue injuries. While it does numb pain temporarily, it may also interfere with the inflammation process your body needs to repair the damage. Inflammation sends specialized cells to the injury site to clean up debris and lay down new tissue. Disrupting that process with ice could delay repair and lead to lower-quality scar tissue. If you use ice for pain relief, keep sessions short (10 to 15 minutes) and don’t treat it as a healing tool.
Anti-inflammatory medications like ibuprofen and naproxen carry a similar tradeoff. They reduce pain and swelling, but the inflammatory response they suppress is part of how your body rebuilds the ligament. Research published in the British Journal of Sports Medicine suggests that anti-inflammatory drugs should not be part of standard soft tissue injury care, particularly at higher doses, because they can impair long-term tissue healing. Acetaminophen (Tylenol) works about as well for ankle sprain pain without blocking inflammation, making it a reasonable alternative for getting through the night.
What Happens While You Sleep
Your body does its most active tissue repair during sleep. Growth hormone release peaks during deep sleep, and blood flow to injured areas increases when you’re at rest. You can support this process by keeping the ankle elevated on pillows and maintaining gentle compression with a wrap. Sleep on your back if possible to keep the leg stable and elevated. If you’re a side sleeper, place a pillow between your legs to keep the injured ankle from rolling.
You’ll likely wake up with less pain than you had the evening before, but the ankle will probably be stiff and still swollen. That’s normal. The swelling peaks around 24 to 48 hours after the injury, so your ankle may actually look worse by morning even though healing has begun.
Starting Movement Safely
Complete immobilization helps with pain and swelling in the first week or so, particularly during the first 10 days. But staying off the ankle entirely for too long weakens the healing ligament and the surrounding muscles. The goal is to begin gentle, pain-free movement as soon as you can tolerate it.
For a mild sprain, this might mean simple ankle circles and up-and-down foot pumps the morning after. Sit on the edge of a bed and slowly trace the alphabet in the air with your toes. This keeps blood flowing, prevents stiffness, and signals the healing tissue to organize along functional lines. The principle is called “optimal loading”: mechanical stress, added gradually and kept below your pain threshold, actually stimulates repair and builds stronger tissue.
If any movement causes sharp pain or a feeling of instability, back off and give it another day. Pushing through significant pain is not the same as gentle mobilization.
After 48 Hours: Contrast Baths
Once you’re past the initial 48-hour window, contrast baths can help reduce lingering swelling. Fill one container with warm water (around 100 degrees Fahrenheit) and another with cold water (55 to 65 degrees). Soak your ankle in the warm water for three minutes, then switch to the cold water for one minute. Repeat five times for a total of about 20 minutes. The alternating temperatures create a pumping effect that helps move excess fluid out of the area.
Signs You May Need an X-Ray
Most ankle sprains don’t involve a fracture, but some do. Doctors use a set of screening criteria called the Ottawa Ankle Rules to decide whether imaging is needed. You should get an X-ray if you have any of the following:
- Tenderness when pressing on the bony bumps on either side of your ankle (the back edge or tip of the bone), not just general soreness in the soft tissue
- Tenderness at the base of the small bone on the outside of your midfoot or the bone on the inner side of your midfoot arch
- Inability to take four steps, even with a limp, both right after the injury and when you try again later
If you can hobble four steps and the bony landmarks don’t hurt when pressed, a fracture is unlikely. But significant bruising that spreads quickly, numbness in the foot, or an ankle that looks visibly deformed all warrant prompt evaluation.
A Realistic Recovery Timeline
A Grade 1 sprain, where the ligament is stretched but not torn, typically resolves in one to three weeks. You’ll probably be walking with mild discomfort within a few days and back to normal activity within two to three weeks. A Grade 2 sprain involves a partial tear and takes three to six weeks. A Grade 3 sprain, a complete ligament rupture, can take several months and sometimes requires a brace or surgical consultation.
The biggest mistake people make is returning to full activity as soon as the pain fades. Pain resolves faster than the ligament heals, and going back too early is a common cause of repeat sprains. Without proper rehabilitation, a badly sprained ankle can lose range of motion and stability permanently, setting you up for chronic problems. Even a mild sprain benefits from a few weeks of balance and strengthening exercises before you return to sports or high-demand activities.
What you do tonight matters. Elevate, compress, protect it, manage pain sensibly, and start gentle movement when it feels safe. You won’t wake up healed, but you’ll wake up having given your body the best possible start.
