How to Help Middle Back Pain: Stretches and Exercises

Most middle back pain comes from muscle tension or poor posture and responds well to a combination of movement, stretching, and simple adjustments to how you sit and sleep. The thoracic spine, the 12 vertebrae between your neck and lower back, is more rigid than the rest of your spine because it’s anchored to your rib cage. That stability means disc herniations here are rare compared to the neck or lower back, but it also means the surrounding muscles tighten easily when you’re hunched over a desk or phone for hours at a time.

Why Your Middle Back Hurts

The most common culprit is muscle irritation from prolonged sitting and poor posture. When you round your shoulders forward, the muscles between your shoulder blades (the rhomboids and mid-trapezius) get stretched and strained while the chest muscles shorten. Over time, this imbalance creates a dull, persistent ache across the middle back that worsens throughout the day.

Other causes include costochondritis (inflammation where the ribs meet the breastbone, which can radiate to the back), joint stiffness in the small facet joints along the thoracic spine, and occasionally a thoracic disc herniation. Stress also plays a role: people tend to carry tension in the upper and middle back, which compounds the effects of sitting.

Stretches That Loosen the Thoracic Spine

The thoracic spine’s primary job beyond support is rotation and extension. When it stiffens, you lose both, and the muscles around it compensate painfully. These three stretches target that stiffness directly.

Cat-cow: Start on all fours with hands under your shoulders and knees under your hips. As you exhale, press your hands into the floor and round your mid-back toward the ceiling, letting your head hang. On the inhale, reverse the curve by dropping your belly toward the floor and lifting your chest. Move slowly back and forth 10 to 15 times, focusing on getting as much motion as possible through the middle of your spine rather than just your lower back.

Child’s pose: From all fours, sit your hips back onto your heels and walk your hands forward until your forehead rests on the ground. Breathe slowly and deeply, feeling the stretch run from your lower back up through your mid-back and into your fingertips. Hold for 30 to 60 seconds and repeat three times. This gently opens the space between the thoracic vertebrae and releases tension in the muscles along both sides of the spine.

Seated spinal twist: Sit on the floor with your legs extended. Cross one foot over the opposite knee and place it flat on the floor. Twist your torso toward the bent knee, using your opposite elbow against the outside of that knee for leverage. Hold for 20 to 30 seconds on each side. This targets the thoracic spine’s rotational range, which is often the first thing to go when the area stiffens up.

Strengthening Exercises for Lasting Relief

Stretching provides short-term relief, but building strength in the muscles that hold your shoulder blades in place is what keeps the pain from coming back. Weak rhomboids and mid-trapezius muscles can’t counteract the forward pull of gravity and desk work, so the pain cycle repeats.

Resisted rows: Loop a resistance band around a sturdy object at about waist height. Stand facing it and hold one end in each hand with your arms extended in front of you. Pull the bands back, squeezing your shoulder blades together, until your elbows are bent at 90 degrees and tucked at your sides. Slowly return to the starting position. Do 8 to 12 repetitions for 2 to 3 sets. This is one of the most effective exercises for the exact muscles that weaken from prolonged sitting.

Prone Y-raises: Lie face down on the floor with your arms extended overhead in a Y shape, thumbs pointing toward the ceiling. Lift your arms a few inches off the ground by squeezing between your shoulder blades. Hold for 2 to 3 seconds and lower. Repeat 10 to 15 times. This builds endurance in the mid-back muscles without any equipment.

How Posture Correction Reduces Pain

Exercise programs targeting thoracic posture consistently produce measurable improvements. In studies on people with excessive upper-back rounding, structured exercise reduced the curvature by roughly 5 to 9 degrees over several weeks, with some programs achieving even larger corrections. That translates to noticeably less strain on the muscles and joints of the middle back during daily activities.

The key finding across these studies is that corrective exercises combining both strengthening and stretching outperform stretching alone. Programs that paired chest-opening stretches with back-strengthening exercises produced about twice the improvement in spinal curvature compared to programs using only one approach.

Fix Your Desk Setup

If you work at a desk, your setup directly determines how much strain your middle back absorbs each day. A few specific adjustments make a significant difference.

Your monitor should sit at eye level so you’re not looking down. Your elbows should rest at roughly 90 degrees, meaning your desk or keyboard tray shouldn’t force you to reach forward or hunch your shoulders up. Your chair needs to support your lower back’s natural curve. When the lower back collapses, the middle back compensates by rounding forward, and that’s where the pain starts. Your feet should rest flat on the floor.

Standing desks help some people, but they don’t automatically fix the problem. The same principles apply: arms at 90 degrees, screen at eye level. The real benefit of a standing desk is that it encourages you to shift positions throughout the day. If you’re sitting, set a reminder to stand and move for a few minutes every 30 to 45 minutes.

Heat, Ice, and Other Home Remedies

For a sudden flare-up or new onset of middle back pain, cold therapy is the better first choice. Applying an ice pack for no more than 20 minutes at a time, four to eight times a day, reduces inflammation and muscle spasms. Always wrap the ice pack in a cloth to protect your skin.

Once the initial sharpness fades, or for chronic middle back stiffness, switch to heat. A warm towel, heating pad, or hot shower raises your pain threshold and relaxes tight muscles. Keep the temperature comfortable but not hot. Anything above about 113°F can be painful, and temperatures over 122°F risk burning the skin. Heat wraps worn throughout the day have been shown to reduce back pain and improve function.

Some people benefit from alternating between ice and heat. If you know a particular activity tends to trigger your mid-back pain, applying cold before and after that activity can help prevent a flare-up.

How You Sleep Matters

Sleep positions affect middle back pain more than most people realize, because you’re in one position for hours. Side sleeping with a pillow between your knees keeps the spine, pelvis, and hips aligned and takes pressure off the back. A full-length body pillow works well if you tend to shift positions.

If you sleep on your back, place a pillow under your knees to help the back muscles relax and maintain a natural spinal curve. A small rolled towel under the waist adds extra support if the mattress doesn’t provide enough. Stomach sleeping is the toughest position for back pain, but if you can’t sleep any other way, placing a pillow under your hips and lower abdomen reduces strain.

When Middle Back Pain Needs Medical Attention

Middle back pain is taken more seriously by clinicians than lower back pain because the thoracic spine is closer to vital organs, and pain here is more likely to signal something beyond a muscle strain. You should get it evaluated promptly if your pain came on after a fall, accident, or impact. Pain that wakes you up at night or is present even at complete rest warrants attention, as does middle back pain paired with unexplained weight loss, fever, or numbness spreading into your legs.

For pain that doesn’t improve after several weeks of consistent stretching, strengthening, and ergonomic changes, physical therapy is the standard next step. It relieves pain for most people and helps prevent further problems. In cases involving disc issues or joint inflammation, steroid injections can provide relief lasting anywhere from a week to over a year, and they’re most effective when timed just before starting a physical therapy program so you can do the strengthening work without pain getting in the way.
