Most methods claiming to increase penile girth have limited evidence, and the ones that do work come with real trade-offs. The average erect circumference is about 11.9 cm (4.7 inches), based on a meta-analysis of over 5,000 men. If you’re close to that number and still feel unsatisfied, it’s worth understanding what actually changes girth, what doesn’t, and what the risks look like before pursuing anything.
What the Options Actually Are
Girth enhancement falls into three broad categories: injectable fillers, surgical implants, and fat transfer. Each works differently, lasts a different amount of time, and carries different risks. Outside of these, there is no proven non-surgical method that meaningfully increases circumference. Manual exercises, supplements, and pumps do not produce lasting girth changes supported by clinical evidence.
Injectable Fillers
Hyaluronic acid filler is the most studied non-surgical option for girth enhancement. In one clinical study, men received about 20 cc of filler injected beneath the skin of the shaft. Their midshaft circumference increased from roughly 7.5 cm to 11.4 cm, a gain of nearly 4 cm (about 1.5 inches). That increase held up at 18 months with no statistically significant drop-off from the one-month measurement.
The procedure is done in-office and doesn’t require general anesthesia. Pricing typically starts around $2,500, and sexual activity can resume within about 72 hours. The results aren’t permanent, since hyaluronic acid is gradually absorbed by the body, but the timeline before a touch-up is needed appears to be well over a year based on available data.
Complications are relatively uncommon but real. About 2.2% of patients develop small nodules under the skin, 1.3% experience minor bleeding, and 1% get infections. Filler migration, where the material shifts from its original position, is another known issue that can create an uneven appearance.
Fat Transfer
Autologous fat transfer involves liposuctioning fat from another part of your body (typically the abdomen or thighs) and injecting it into the penile shaft. The appeal is that you’re using your own tissue, which reduces the chance of an allergic reaction. The downside is unpredictability: fat survival rates range from 10% to 80%, meaning much of what’s injected may be reabsorbed within months. That wide range makes outcomes hard to predict, and uneven absorption can lead to a lumpy or asymmetric result.
Across various techniques studied for girth enhancement, reported increases range from 0 to 4.9 cm. Fat transfer falls somewhere in that range, but the inconsistency is why many urologists consider it less reliable than filler-based approaches.
The Penuma Implant
The Penuma is a silicone sleeve surgically placed under the skin of the penis. It’s the only FDA-cleared device for cosmetic penile enhancement, having received clearance in 2004. The implant adds girth along the shaft and is intended to be permanent.
Complication rates vary depending on who’s reporting them. The device’s developer published an overall complication rate of 3.3% in a retrospective study, including seroma (fluid buildup), scar formation, and infection. A separate study of 400 patients found slightly higher individual rates: 4.8% for seroma, 4.5% for scarring, and 3.3% for infection. More concerning, a single-center review of 13 patients found that 62% experienced the implant protruding at the tip, causing discomfort for both the patient and their partner. That’s a small sample, but the rate is striking enough to take seriously.
The surgery requires general anesthesia and a longer recovery period than injectable procedures. Cost is significantly higher than fillers, typically running into several thousand dollars or more depending on the surgeon and facility.
Why Manual Exercises Don’t Work
Jelqing, a technique involving repeatedly squeezing and stroking the semi-erect penis, is probably the most commonly searched “natural” method. There is no clinical evidence that it increases girth. What it can do is cause harm. Documented side effects include pain, bruising, and skin irritation. Over time, the repeated trauma can create scar tissue or hard deposits inside the penis, a condition called Peyronie’s disease that causes painful curvature. Some men who practice jelqing develop erectile dysfunction.
Penile traction devices, which are worn for hours each day, have some limited evidence for increasing length in men with specific conditions like Peyronie’s disease. But for girth, the data is not encouraging. One study had men wear a traction device for up to 9 hours a day over three months and found no significant change in shaft circumference. There was a small increase in the circumference of the glans (the tip), but nothing meaningful along the shaft itself.
PRP Injections (the P-Shot)
Platelet-rich plasma injections, marketed as the “P-Shot,” involve drawing your blood, concentrating the platelets, and injecting them into the penis. Providers sometimes claim this increases girth. Cleveland Clinic states directly that this claim is not supported by any scientific evidence. PRP may have applications for erectile function in some contexts, but as a girth enhancement method, it remains unproven.
When Size Concerns Are the Real Issue
A significant number of men who seek girth enhancement have penises that fall within the normal range. Some of these men meet the criteria for what’s called penile dysmorphic disorder, a form of body dysmorphic disorder where the size or shape of the penis becomes a consuming preoccupation that causes significant shame or difficulty functioning. There are currently no standardized screening tools to distinguish between men who have a realistic concern and those whose distress stems from a distorted self-perception.
This matters because no procedure will resolve dissatisfaction rooted in dysmorphia. Men in this category often remain unhappy with their results even after successful enhancement, or they pursue increasingly aggressive procedures. If your concern about size is causing persistent anxiety, interfering with relationships, or dominating your thoughts, talking with a mental health professional who specializes in body image may be more effective than any procedure. That’s not a dismissal of the concern. It’s a recognition that the solution might not be surgical.

- Hyaluronic acid filler: Most consistent non-surgical results. Gains of up to 4 cm reported. Not permanent but long-lasting (18+ months). Costs start around $2,500. Low complication rate.
- Fat transfer: Uses your own tissue, but fat survival is unpredictable (10% to 80%). Risk of lumpy or uneven results. May require repeat procedures.
- Penuma implant: Only FDA-cleared device. Permanent but carries surgical risks including implant protrusion, infection, and scarring. Higher cost and longer recovery.
- Traction devices, jelqing, PRP: No reliable evidence for girth increase. Manual exercises carry risk of tissue damage and erectile dysfunction.
If you’re seriously considering enhancement, the most important step is finding a urologist or andrologist with specific experience in these procedures, not a general cosmetic clinic. Outcomes depend heavily on technique, and complication management requires specialized knowledge. Ask about their complication rates, how many procedures they’ve performed, and what revision looks like if results aren’t what you expected.
