Low sexual desire is one of the most common sexual health concerns among women, affecting an estimated 1 in 10 women at a level that causes real distress. The good news is that libido isn’t fixed. It responds to changes in stress, hormones, medications, relationships, physical health, and even diet. Some solutions are straightforward lifestyle shifts, while others involve working with a healthcare provider to address an underlying cause.
Understand What’s Actually Driving Low Desire
Before trying to fix a low libido, it helps to figure out what’s behind it. Sexual desire in women is shaped by a mix of biological, psychological, and relationship factors, and the most effective approach depends on which ones are at play. A woman who lost interest after starting an antidepressant needs a different strategy than someone whose desire faded gradually during a stressful period at work.
Common causes include chronic stress, hormonal shifts (perimenopause, menopause, postpartum), medications like SSRIs and hormonal birth control, relationship dissatisfaction, depression, poor sleep, and pelvic pain conditions. When low desire persists for six months or longer and causes significant personal distress, it may meet the clinical threshold for hypoactive sexual desire disorder (HSDD), a recognized diagnosis with specific treatment options.
How Stress Suppresses Desire
Chronic stress is one of the most overlooked libido killers. When your body stays in a prolonged stress response, your stress hormone system actively suppresses your reproductive hormone system. This isn’t a metaphor. High levels of stress hormones directly interfere with the signaling that drives estrogen and testosterone production, both of which play roles in sexual desire.
This means that stress-management strategies aren’t just “wellness advice.” They’re biologically relevant to your libido. Regular exercise, adequate sleep, and mindfulness practices all help lower chronic stress activation. Mindfulness-based therapy in particular has strong clinical evidence as a treatment for low sexual desire, both on its own and alongside other approaches.
Therapy That Actually Works for Desire
Psychological interventions have some of the strongest evidence behind them for treating low desire. The 2024 International Consultation on Sexual Medicine rated the evidence for sex therapy, cognitive-behavioral therapy (CBT), and mindfulness-based therapy as “strong” for HSDD. These approaches work by addressing the mental and emotional patterns that suppress desire: anxiety about performance, negative body image, difficulty staying present during intimacy, or unresolved relationship tension.
Mindfulness-based therapy focuses on helping you notice physical sensations without judgment, which can reconnect you with arousal cues your brain has been filtering out. CBT targets unhelpful thought patterns around sex. Sex therapy often includes structured exercises you do at home with a partner, gradually rebuilding comfort and desire over time. These aren’t quick fixes, but they address root causes rather than masking symptoms.
Prescription Medications for Low Libido
Two FDA-approved medications currently exist specifically for low sexual desire in premenopausal women.
The first is a daily pill (flibanserin) that works on brain chemistry by adjusting activity in serotonin and dopamine pathways involved in desire. Clinical trials lasting six months showed consistent improvements in both sexual desire and the number of satisfying sexual experiences, though the effects were modest compared to placebo. It requires daily use, takes several weeks to show results, and cannot be combined with alcohol.
The second option is an injection (bremelanotide) that you self-administer under the skin at least 45 minutes before anticipated sexual activity. It works through a different brain pathway that influences arousal and desire. The main drawback is nausea, which is most common after the first dose and typically lasts about two hours. Unlike the daily pill, this one is used only when needed.
Both medications have moderate to strong evidence behind them for premenopausal women. Evidence for postmenopausal women is more limited, with the daily pill showing moderate support and the injectable lacking sufficient data for that group.
Testosterone Therapy After Menopause
Testosterone isn’t just a male hormone. Women produce it too, and levels decline with age, particularly after menopause. A global consensus statement published in the Journal of Clinical Endocrinology & Metabolism supports testosterone therapy for postmenopausal women with HSDD, with dosing aimed at restoring levels to the normal premenopausal range rather than exceeding it.
Patches and creams (applied to the skin) are the preferred delivery methods. Oral testosterone is not recommended because it negatively affects cholesterol levels. Non-oral forms, at physiological doses, have not shown significant adverse effects on lipid profiles in short-term studies. If you try testosterone therapy, expect a baseline blood test before starting and follow-up testing at three to six weeks, then every six months to monitor levels and screen for signs of excess.
For premenopausal women, the picture is different. Current evidence is limited, and expert guidelines do not recommend testosterone therapy for younger women with low desire.
What About Estrogen and Other Hormones?
Standard hormone replacement therapy (estrogen and progesterone) is widely used for menopausal symptoms like hot flashes, but the data do not support using it specifically to treat low desire. The same is true for DHEA supplements in postmenopausal women with normal adrenal function and for tibolone, a synthetic hormone sometimes used in Europe. None of these have shown meaningful improvements in sexual desire in women diagnosed with HSDD.
That said, if vaginal dryness or pain during sex is contributing to your low desire, local estrogen therapy can address those symptoms effectively. Removing pain from the equation often helps desire recover on its own.
If Your Medication Is the Problem
SSRIs and other antidepressants are a well-known cause of reduced libido. If you suspect your medication is affecting your desire, several strategies can help without abandoning treatment for your mental health.
- Wait it out. Sexual side effects sometimes diminish on their own after the first few weeks or months on a medication.
- Adjust the dose. A lower dose may still treat your depression effectively while reducing sexual side effects.
- Time it differently. Side effects can peak a few hours after taking a dose. Shifting when you take your medication, or planning intimacy around that window, can help.
- Switch medications. Some antidepressants are much less likely to cause sexual side effects. Bupropion and mirtazapine are two alternatives that tend to spare libido.
- Add a second medication. Adding bupropion to an existing SSRI regimen has been shown to counter sexual side effects, boost drive and arousal, and increase orgasm intensity in some people.
Any changes to your medication should be a conversation with whoever prescribes it. Stopping an antidepressant abruptly can cause withdrawal symptoms or relapse.
Pelvic Floor Health and Desire
Your pelvic floor muscles play a bigger role in sexual desire than most people realize. A well-functioning pelvic floor increases blood flow to the genital area, which improves sensitivity and arousal. Stronger pelvic muscles are also linked to more intense orgasms, and positive sexual experiences reinforce desire over time.
On the flip side, pelvic floor problems can actively suppress libido. Women with pelvic floor disorders like urinary incontinence or pelvic organ prolapse report significantly higher rates of lost desire. Pain during sex caused by excessive tension in the pelvic floor muscles creates a negative feedback loop where the anticipation of discomfort shuts down interest before it starts. Pelvic floor physical therapy can address both weakness and excessive tension, improving symptoms that feed into low desire.
Diet, Exercise, and Sleep
A Mediterranean-style diet rich in vegetables, olive oil, fish, and whole grains has shown measurable benefits for sexual function. In a randomized trial following over 200 adults with type 2 diabetes for eight years, women assigned to a Mediterranean diet showed significantly less decline in sexual function scores compared to those on a standard low-fat diet. The effect likely comes from improved cardiovascular health, better blood flow, and reduced inflammation, all of which support sexual response.
Exercise matters for similar reasons. Regular physical activity improves circulation, reduces stress hormones, boosts mood, and increases body confidence. Even moderate activity like brisk walking for 30 minutes most days can make a noticeable difference over several weeks. Intense or excessive exercise, however, can backfire by triggering the same stress response that suppresses reproductive hormones.
Sleep is the third pillar. Poor sleep disrupts hormone production, increases stress reactivity, and drains the mental energy that desire depends on. Prioritizing consistent, adequate sleep (seven to nine hours for most adults) supports every other strategy on this list.
Supplements: Limited but Emerging Evidence
Fenugreek seed extract has some clinical data behind it. In a randomized, placebo-controlled trial, postmenopausal women taking 500 mg daily (split into two doses) for six weeks showed significant increases in free testosterone and estradiol levels within the normal range, along with improvements in vaginal dryness and irritability. The evidence is promising but still limited to small studies in specific populations.
Maca root is one of the most popular supplements marketed for libido, typically studied at doses of 3,000 mg per day. However, the clinical trial data in women is thin. One of the better-designed studies, which aimed to test maca in women taking mood-stabilizing medications, was never completed due to insufficient enrollment. The supplement appears safe, but the evidence for its effectiveness in women remains weak.
No supplement has the strength of evidence behind it that therapy, stress management, or prescription treatments do. If you try one, give it at least six to eight weeks before judging whether it’s working, and be skeptical of products making dramatic claims.
