Most men with a low sperm count have no obvious symptoms. The primary sign is difficulty conceiving after months of trying, and many don’t discover the issue until they pursue fertility testing. There are, however, some physical clues that can point toward a problem, and understanding what gets tested and when to act can save you significant time.
Why There Usually Aren’t Obvious Symptoms
Low sperm count, called oligospermia, doesn’t change how your body feels day to day. Your semen looks the same, sex feels the same, and nothing in your routine tips you off. That’s because sperm make up a tiny fraction of the fluid you ejaculate. You could have half the normal concentration and never notice a difference.
The one reliable signal is time. If you and your partner have been having regular, unprotected sex for 12 months without a pregnancy, something may be off on one or both sides. If your partner is between 35 and 40, that window shortens to six months. If she’s over 40 or has a known fertility risk factor like prior tubal disease or ectopic pregnancy, evaluation is recommended right away.
Physical Signs Worth Paying Attention To
While low sperm count itself is silent, the underlying conditions that cause it sometimes produce other symptoms. These don’t confirm a low count on their own, but they’re worth noting:
- Low sex drive. A noticeable drop in your interest in sex can reflect a hormonal imbalance, particularly low testosterone, which also affects sperm production.
- Erection or ejaculation problems. Difficulty getting or maintaining an erection, or changes in ejaculation, can stem from the same hormonal or structural issues that reduce sperm count.
- Pain, swelling, or a lump near the testicles. This could indicate a varicocele (enlarged veins in the scrotum), infection, or another condition that directly interferes with sperm production.
- Reduced facial or body hair. Noticeably less hair growth than expected can signal a chromosomal or hormonal condition affecting fertility.
If any of these apply to you, it’s worth getting checked even if you aren’t actively trying to conceive. They point toward conditions that are often treatable, and catching them early gives you more options.
How Varicoceles Fit In
A varicocele is a swelling of the veins that drain the testicle, similar to a varicose vein in the leg. About 15% of all men have one, but among men evaluated for infertility, that number jumps to 40%. In men who previously fathered children but are now struggling (secondary infertility), a varicocele is involved in up to 70% of cases.
The most supported explanation is heat. The enlarged veins impair blood circulation, raising the temperature inside the testicle. Sperm production is highly sensitive to temperature, which is why the testicles sit outside the body in the first place. Even a small, sustained increase can reduce both the number and quality of sperm being made. Varicoceles are often painless, though some men notice a dull ache or heaviness in the scrotum, especially after standing for long periods.
The Only Way to Know for Sure: Semen Analysis
A semen analysis is the standard test. You provide a sample, typically through masturbation at a clinic or at home with a collection kit, and a lab evaluates it under a microscope and by computer. The test measures several things at once:
- Sperm concentration: the number of sperm per milliliter of semen. Normal is 20 million or more per milliliter.
- Total sperm count: the total number across the entire sample. Normal is 40 million or more.
- Motility: the percentage of sperm that are actively swimming. You could have a high count but poor motility, which still affects fertility.
- Morphology: the size and shape of your sperm. Abnormally shaped sperm have a harder time reaching and fertilizing an egg.
- Semen volume: how much fluid you produced. Low volume can indicate a blockage or hormonal issue.
- pH: the acidity level of your semen, which affects whether sperm can survive.
A total motile count below 20 million is considered oligospermia. Below 1 million motile sperm is classified as severe oligospermia. Doctors typically run the test at least twice, spaced a few weeks apart, because sperm counts naturally fluctuate from one sample to the next. A single result doesn’t give a reliable picture.
What About At-Home Sperm Tests?
Home testing kits are widely available and can tell you whether your sperm concentration falls above or below a threshold. They aren’t inaccurate for what they measure, but what they measure is limited. Most kits only check concentration. They can’t assess motility, morphology, volume, or pH.
That’s a significant gap. You could, for example, have 100 million sperm per milliliter but none of them moving. A home test would read as normal, but you’d still have a fertility problem. Even results that appear low on a home test don’t tell you why, and the “why” is what determines treatment. Think of home kits as a rough screening tool. They might prompt you to seek lab testing sooner, but they can’t replace it. A normal result from a home kit does not rule out male-factor infertility.
Common Causes Beyond Varicoceles
Sperm production is a complex process, and many things can disrupt it. Hormonal imbalances, particularly low testosterone or elevated estrogen, directly reduce the signals that tell your body to make sperm. Infections of the reproductive tract, including sexually transmitted infections, can cause inflammation or scarring that blocks sperm from reaching the semen. Prior surgeries in the groin or pelvic area sometimes damage the pathways sperm travel through.
Lifestyle factors also play a measurable role. Chronic heavy alcohol use, tobacco, anabolic steroids, and certain medications (especially testosterone replacement therapy, which paradoxically shuts down the body’s own sperm production) can all lower your count. Obesity raises scrotal temperature and disrupts hormone levels. Frequent exposure to industrial chemicals, heavy metals, or prolonged heat from saunas or hot tubs can temporarily suppress production as well.
How Long Sperm Takes to Regenerate
Your body continuously produces new sperm, but the full cycle from creation to ejaculation takes about 64 days. That means if you make a lifestyle change today, quit smoking, lose weight, stop using a hot tub daily, you won’t see the effect in a semen analysis for roughly two to three months. This is important to keep in mind both for retesting and for evaluating whether interventions are working. Doctors typically recommend waiting at least that long before repeating a semen analysis after any change.
This timeline also explains why a single bad result isn’t necessarily permanent. A high fever, a stressful period, or a temporary medication could suppress your count for one cycle. Retesting after a couple of months often tells a different story.
What Happens During a Clinical Evaluation
If your semen analysis comes back abnormal, the next steps usually include a physical exam of the testicles (checking for varicoceles, lumps, or size changes), blood tests to measure hormone levels, and a detailed medical history covering medications, surgeries, and lifestyle. In some cases, an ultrasound of the scrotum helps identify structural problems that aren’t detectable by touch.
The goal is to find a treatable cause. Varicoceles can be corrected with a minor procedure. Hormonal imbalances can be addressed with medication. Infections are treated with antibiotics. When the cause is lifestyle-related, changes to diet, exercise, substance use, or heat exposure are often enough to improve the numbers over a few sperm cycles. For cases where counts remain very low, assisted reproduction techniques offer additional paths forward.
