How to Know If You Have COVID: Symptoms & Testing

The only way to confirm you have COVID-19 is to take a test, but certain symptoms and timing clues can help you figure out whether testing makes sense right now. COVID symptoms overlap heavily with the flu and common cold, so your body alone won’t give you a definitive answer. Here’s what to look for, how to test accurately, and what to do with the results.

Symptoms Most Likely to Signal COVID

The most common COVID symptoms right now are a runny or stuffy nose, headache, and sore throat. These have shifted over time as the virus has evolved. The full list of possible symptoms includes fever or chills, cough, shortness of breath, congestion, sore throat, fatigue, muscle or body aches, headache, nausea or vomiting, diarrhea, and loss of taste or smell. You might get just two or three of these, or you might get hit with several at once.

Loss of taste or smell used to be a hallmark sign, but it’s become less common with newer variants. If you do notice it, COVID is still a strong possibility. The trickier scenario is when your symptoms look like a garden-variety cold: stuffy nose, scratchy throat, mild fatigue. That combination is now the most typical COVID presentation, which makes testing the only reliable way to sort it out.

How COVID Feels Different From a Cold or Flu

COVID, the flu, and the common cold share so many symptoms that even doctors can’t tell them apart just by looking at you. That said, a few patterns can nudge your suspicion one way or the other.

Flu symptoms tend to hit hard and fast, usually appearing one to four days after exposure. You’ll often feel fine one moment and wiped out the next, with a high fever, intense body aches, and exhaustion that keeps you in bed. COVID can come on more gradually, and fever isn’t always present, especially in vaccinated people. A cold is usually milder overall and resolves within 3 to 10 days without the deep fatigue or body aches that COVID and the flu can cause.

Seasonal allergies are another common source of confusion, but they aren’t caused by a virus. If your symptoms follow a pattern tied to pollen counts, come with itchy eyes, and don’t include fever or body aches, allergies are more likely. But if you’re unsure, a rapid test takes the guesswork out.

When and How to Test

Timing matters more than most people realize. A rapid antigen test (the kind you take at home) is significantly less accurate early in an infection or when you’re not showing symptoms. In a CDC study from late 2022 to mid-2023, rapid tests detected only 47% of infections confirmed by PCR, the more sensitive lab-based test. On days when people had no symptoms at all, sensitivity dropped to just 18%.

The tests perform much better once symptoms kick in. When any COVID symptoms were present, sensitivity rose to 56% compared to PCR. On days when people had a fever specifically, it jumped to 77%. So the practical takeaway: if you feel sick and especially if you have a fever, a rapid test is reasonably reliable. If you feel fine but had a known exposure, a single negative test doesn’t mean much.

The Repeat Testing Rule

A single negative rapid test is not enough to rule out COVID. The FDA now requires all at-home test labels to reflect this. If you have symptoms, test again 48 hours after the first negative result, for a total of at least two tests. If you don’t have symptoms but suspect exposure, you need three tests spaced 48 hours apart over five days.

PCR tests, available at pharmacies, clinics, and some drive-through sites, are more sensitive and can catch infections that rapid tests miss. If your rapid test is negative but your symptoms are getting worse, a PCR test is the logical next step.

Can You Use an Expired Test Kit?

Many at-home test kits have had their expiration dates officially extended by the FDA. The date printed on the box may no longer be accurate. Before you throw out an old kit, check the FDA’s website, which maintains an updated table listing every authorized home test and whether its shelf life has been extended. Search by the manufacturer or test name to see if yours is still valid.

What a Positive Test Means for You

If your test comes back positive, what happens next depends largely on your risk profile. Most healthy people recover at home with rest, fluids, and over-the-counter remedies for fever and congestion. But if you’re 50 or older, or you have certain medical conditions, you’re eligible for antiviral treatment that works best when started within the first few days of symptoms.

The list of conditions that raise your risk of severe illness is longer than many people expect. It includes diabetes (type 1 or type 2), chronic kidney disease at any stage, heart conditions like heart failure or coronary artery disease, moderate to severe asthma, COPD, cancer, obesity, pregnancy, and a weakened immune system from any cause. Mental health conditions including depression and schizophrenia spectrum disorders are also on the list, as are substance use disorders, current or former smoking, and physical inactivity. Even people with certain disabilities or neurological conditions like dementia face higher risk.

If any of these apply to you, getting tested early rather than waiting matters, because the treatment window is narrow.

Warning Signs That Need Immediate Attention

Most COVID infections resolve on their own, but some progress to dangerous territory. Seek emergency care if you experience persistent chest pain or pressure, trouble breathing that’s getting worse rather than staying stable, confusion or inability to stay awake, or bluish lips or face. These signs can indicate that the infection is affecting your lungs or oxygen levels in ways that need hospital-level support.

If You Were Exposed but Feel Fine

COVID’s incubation period, the gap between catching the virus and feeling sick, varies but generally falls between two and five days for current variants, though it can stretch longer. If you know you were exposed, you might feel perfectly healthy for the first few days while the virus replicates. During this window, rapid tests are at their least accurate. The three-test protocol spaced over five days is designed for exactly this situation, catching an infection as your viral load climbs high enough to register.

Even without symptoms, you can still spread the virus to others during this period. If you’ve had a known close exposure, wearing a mask around others while you wait for test results is a practical way to reduce that risk.
