There is no single test or checklist that can definitively tell you whether you have “leaky gut.” The term itself is not a recognized medical diagnosis. Intestinal permeability, the clinical concept behind it, is real and measurable in research settings, but the tools available to consumers and even most doctors remain unreliable. What you can do is learn which symptoms are commonly associated with gut barrier problems, understand what actually causes them, and know which tests are worth your time.
What “Leaky Gut” Actually Means

If you have digestive symptoms alongside unexplained fatigue, skin flare-ups, or joint pain, it’s worth investigating, but those symptoms alone don’t confirm a leaky gut.
Why Testing Is Still Unreliable
One of the most frustrating aspects of this topic is that there is no established gold standard for measuring intestinal permeability in living humans. The tests that exist each come with significant limitations.
The Lactulose-Mannitol Test
This is the closest thing to a direct permeability measurement. You drink a solution containing two sugars: lactulose (a larger molecule that shouldn’t cross a healthy gut lining easily) and mannitol (a smaller one that passes through normally). After a few hours, your urine is collected and the ratio of the two sugars is calculated. A higher-than-expected ratio of lactulose to mannitol suggests the gut barrier is letting through molecules it shouldn’t.
The test has real scientific grounding, but it’s tricky in practice. Every lab needs to establish its own reference ranges because results vary based on gastric emptying speed, intestinal transit time, kidney function, and how completely the urine was collected. One research center reported that even among healthy volunteers, the ratio ranged from 0.003 to 0.25, a nearly hundredfold spread. This test is primarily used in research, not in routine clinical care, and most gastroenterologists won’t order it.
Zonulin Blood Tests
Zonulin is a protein your body produces that helps regulate tight junctions. In theory, high blood levels would indicate the gut barrier is opening up too much. Several direct-to-consumer and functional medicine labs offer zonulin testing. The problem: a major 2021 review published in the journal Gut found that commercially available tests don’t actually measure zonulin. The antibodies used in these test kits react with unknown proteins instead. Results from these kits correlate poorly with actual permeability as measured by sugar absorption tests. In plain terms, a “high zonulin” result from a commercial lab tells you very little about your gut barrier.
IgG Food Sensitivity Panels
These tests, often marketed alongside leaky gut protocols, measure antibodies your immune system makes in response to specific foods. The idea is that if your gut is leaky, food particles entering the bloodstream will trigger an immune response detectable through IgG levels. The American Academy of Allergy, Asthma & Immunology has recommended against using these tests. The presence of IgG antibodies to food is a normal immune response to eating, not evidence of a problem. Higher levels of certain food antibodies may actually reflect tolerance to those foods, not sensitivity. No scientific evidence supports using IgG panels to diagnose food intolerances or leaky gut.
At-Home Gut Health Kits
Several companies sell stool-based or blood-based kits claiming to assess your gut barrier. A 2021 review in the journal Gastroenterology noted that claims about nutrients, supplements, or medications altering intestinal barrier function remain unsubstantiated. Many of these kits rely on markers like lipopolysaccharide (a bacterial toxin) or zonulin, both of which have questionable accuracy as permeability indicators. Spending $150 to $300 on one of these kits is unlikely to give you a clear answer.
What’s Actually Causing Your Symptoms
If you’re experiencing the cluster of symptoms associated with leaky gut, something is damaging your intestinal lining or disrupting your gut bacteria. Common culprits include chronic alcohol use, long-term use of anti-inflammatory painkillers, untreated celiac disease, inflammatory bowel disease, infections, and radiation therapy. Chronic stress, poor diet, and bacterial imbalance in the gut can also contribute to barrier dysfunction over time.
This is an important distinction. Leaky gut is not a root cause; it’s a downstream effect. The more productive question isn’t “do I have leaky gut?” but rather “what is irritating my gut, and can I address it?” A gastroenterologist can test for conditions like celiac disease, Crohn’s disease, or small intestinal bacterial overgrowth, all of which have validated diagnostics and known treatments. Identifying and treating one of these conditions often resolves the permeability issue along with it.
Practical Steps if You Suspect Gut Barrier Issues
Since no reliable consumer test exists, your best approach is a combination of symptom tracking and medical evaluation. Keep a food and symptom diary for two to three weeks, noting what you eat and when symptoms like bloating, pain, diarrhea, or fatigue appear. Patterns often emerge that point toward specific food triggers or timing clues.
Dietary changes that reduce gut irritation tend to help regardless of whether permeability is formally confirmed. Reducing processed foods, alcohol, and anti-inflammatory painkillers removes some of the most common sources of intestinal damage. Increasing fiber intake supports beneficial gut bacteria, which in turn help maintain the tight junctions between intestinal cells. Fermented foods like yogurt, kimchi, and sauerkraut introduce bacterial strains associated with a healthier gut lining.
If your symptoms are persistent, worsening, or accompanied by weight loss, blood in your stool, or severe fatigue, those are signs of something that needs medical investigation beyond a leaky gut framework. A gastroenterologist can run validated tests for specific conditions and help you find an actual diagnosis rather than chasing a label that, for now, medicine can’t reliably confirm or rule out.
