Liver disease often produces no obvious symptoms in its early stages, which is one reason it goes undetected in so many people. Roughly 23% of adults worldwide now have some form of fatty liver disease alone, up from about 18% in 1990. Knowing what to look for, and what tests can reveal, gives you the best chance of catching liver problems before they cause lasting damage.
Early Symptoms Are Easy to Miss
The liver can keep functioning even when it’s inflamed or partially scarred, so early liver disease frequently feels like nothing at all. When symptoms do appear, they tend to be vague: constant tiredness, loss of appetite, mild nausea, or a general sense of feeling unwell. These overlap with dozens of other conditions, which is why many people dismiss them or attribute them to stress and poor sleep.
One early clue that’s easier to pin on the liver is a dull ache or sense of fullness in the upper right side of your abdomen, just below the ribs. That’s where the liver sits, and inflammation can make it swell enough to press against surrounding tissue. Dark urine and pale or clay-colored stools are another pair of signals worth paying attention to. These changes happen when the liver can’t properly process bilirubin, the pigment that normally gives stool its brown color and gets filtered out of urine.
Visible Signs That Point to Liver Trouble
As liver damage progresses, more distinctive physical signs start to show up. Jaundice, a yellowing of the skin and the whites of the eyes, is one of the most recognizable. It results from bilirubin building up in the blood when the liver can’t clear it efficiently. On darker skin tones, jaundice may be easier to spot in the eyes, the palms of the hands, or the soles of the feet rather than on the face or arms.
Itchy skin without an obvious rash is another hallmark. Bile salts that the liver normally processes can accumulate under the skin and trigger persistent itching that doesn’t respond well to typical moisturizers or antihistamines. You might also notice that you bruise more easily than usual, since the liver produces many of the proteins your blood needs to clot properly.
Two skin changes are particularly associated with chronic liver disease. Palmar erythema is a reddening of the palms, especially around the base of the thumb and the outer edge. Spider nevi are small red lesions with a central dot and tiny blood vessels radiating outward, most commonly found on the face, neck, chest, and arms. If you press the center of one, it blanches white and then refills from the middle outward when you release. Both of these result from hormonal imbalances that develop when the liver can no longer properly metabolize estrogen and other steroid hormones.
Swelling in the Belly and Legs
Fluid buildup is one of the more alarming signs and typically indicates more advanced disease. Ascites, the accumulation of fluid in the abdominal cavity, can make your belly look and feel distended even if you haven’t gained weight elsewhere. It happens because scarring in the liver increases pressure in the blood vessels that feed into it, forcing fluid out into surrounding tissue.
Swelling in the legs and ankles often accompanies ascites. The same pressure changes, combined with the liver’s reduced ability to produce albumin (a protein that helps keep fluid inside blood vessels), allow fluid to pool in the lower extremities. If you press a finger into swollen skin on your shin and it leaves an indent that takes several seconds to fill back in, that’s a sign of significant fluid retention.
Brain Fog and Personality Changes
One set of symptoms people rarely connect to the liver involves the brain. When the liver can’t filter toxins effectively, ammonia and other waste products build up in the bloodstream and cross into the brain. The early effects are subtle: disrupted sleep patterns, forgetfulness, poor concentration, difficulty with fine motor tasks like handwriting, and personality or mood changes that friends and family might notice before you do. Some people develop breath with a musty or sweet odor.
These cognitive symptoms can fluctuate day to day, which makes them easy to write off as fatigue or aging. But if you’re experiencing brain fog alongside any of the physical signs described above, the combination is significant and worth investigating.
How Liver Disease Gets Diagnosed
A standard blood panel called a liver function test measures several enzymes and proteins that indicate how well your liver is working. The key markers and their normal ranges for adult men are:
- ALT: 7 to 55 units per liter
- AST: 8 to 48 units per liter
- ALP: 40 to 129 units per liter
- Bilirubin: 0.1 to 1.2 milligrams per deciliter
Ranges can vary slightly between labs, and normal values for women and children may differ. Elevated levels don’t automatically mean serious disease. They signal that something is irritating or damaging liver cells, which could be anything from a medication side effect to a viral infection to fatty liver disease. Persistently elevated results, or levels that are several times above normal, warrant further investigation.
Imaging and Elastography
If blood work raises concerns, imaging is usually the next step. Ultrasound can detect fatty deposits, tumors, and changes in liver size. A more specialized tool called transient elastography (often known by the brand name FibroScan) measures liver stiffness by sending a vibration through the organ. Stiffer tissue indicates more scarring. The test is painless, takes about 10 minutes, and can be done in a regular clinic visit.
Elastography is highly sensitive, meaning it reliably catches cases of advanced scarring and cirrhosis. In one study, every patient who had advanced fibrosis or cirrhosis on a traditional biopsy was correctly identified by the scan. However, it’s less precise in the other direction: about 35% of patients flagged for advanced scarring by the scan turned out to have minimal or no fibrosis when a biopsy was performed. Higher body weight, active inflammation, and fat deposits in the liver can all make the liver appear stiffer than it actually is. For this reason, liver biopsy remains the most accurate way to assess the exact degree of scarring, though it’s typically reserved for cases where the diagnosis is uncertain or a treatment decision hinges on precise staging.
Stages of Liver Scarring
Liver damage is graded on a scale from F0 to F4. F0 means no scarring at all. F1 is mild scarring with the liver’s overall structure still intact. F2 represents moderate scarring. F3 means advanced scarring that has begun to disrupt blood flow through the liver. F4 is cirrhosis, the most severe stage, where extensive scar tissue has replaced enough healthy liver that the organ’s function is permanently compromised.
The progression from F0 to F4 typically takes years or even decades, depending on the cause. This is important because stages F1 through F3 are often reversible if the underlying cause is addressed. Removing the source of damage, whether that’s alcohol, excess weight, or an untreated infection, allows the liver to slowly replace scar tissue with healthy cells. Once cirrhosis develops, the damage is largely permanent, though further progression can still be slowed or stopped.
Who Should Be Paying Attention
Certain factors significantly raise your risk. Heavy alcohol use is the most well-known, but metabolic factors now account for a larger share of liver disease globally. Carrying excess weight (particularly around the midsection), having type 2 diabetes or insulin resistance, and high triglyceride levels all increase the likelihood of fat accumulating in the liver. Viral hepatitis B and C cause direct liver inflammation and can lead to cirrhosis if untreated. Autoimmune conditions, certain herbal supplements, and long-term use of some over-the-counter pain medications also contribute.
