How to Know If Your Heart Is Healthy: Key Signs

A healthy heart shows itself through a handful of measurable signals: your resting heart rate, blood pressure, cholesterol levels, how quickly you recover from exercise, and how well you handle physical effort. You don’t need a cardiologist to get a preliminary read on most of these. Some you can check at home right now, others require a basic blood test. Here’s what to look for and what the numbers mean.

Resting Heart Rate

A normal resting heart rate for most adults falls between 60 and 100 beats per minute. To measure yours, sit quietly for five minutes, then place two fingers on the inside of your wrist or the side of your neck and count the beats for 30 seconds. Double that number.

Lower within that range is generally better. A resting heart rate in the low 60s or high 50s suggests your heart is pumping blood efficiently without working too hard. Athletes often have resting rates in the 40s or 50s because their hearts have adapted to move more blood per beat. A rate consistently above 80 at rest, while technically “normal,” has been linked to higher cardiovascular risk over time. If yours regularly exceeds 100 without obvious cause (caffeine, stress, dehydration), that’s worth investigating.

Blood Pressure

Blood pressure is the single most important number for predicting heart disease and stroke. The 2025 guidelines from the American Heart Association and American College of Cardiology define the categories clearly:

  • Normal: below 120/80 mm Hg
  • Elevated: 120 to 129 systolic (the top number) with the bottom number still under 80
  • Stage 1 hypertension: 130 to 139 systolic, or 80 to 89 diastolic

Many pharmacies have free blood pressure kiosks, and home monitors are inexpensive and reliable. Take readings at the same time of day, sitting with your feet flat on the floor, and average several readings over a week for the most accurate picture. A single high reading doesn’t mean much. A pattern of high readings does.

The Stair Test

One of the simplest at-home checks for heart health is climbing four flights of stairs, about 60 steps, at a brisk but steady pace. Researchers have found that finishing in under 45 seconds suggests excellent cardiovascular fitness and a lower risk of serious cardiac events. Completing it in under 90 seconds still indicates good fitness. If it takes you longer than 90 seconds, or if you need to stop partway through, your heart may not be pumping blood as efficiently as it should.

This isn’t a diagnostic test, but it’s a useful reality check. If you’re winded and struggling after two flights, that gap between your current fitness and the benchmark tells you something meaningful about where your cardiovascular system stands.

How Quickly Your Heart Recovers

Heart rate recovery, how fast your pulse drops after you stop exercising, is one of the better indicators of cardiovascular health that you can track yourself. After a bout of vigorous activity (a run, a hard bike ride, a fast walk uphill), stop and measure your heart rate immediately, then again after one minute of rest.

A healthy heart should drop by at least 18 beats per minute in that first minute. A smaller drop suggests your autonomic nervous system, the internal wiring that regulates your heart’s speed, isn’t functioning optimally. Poor heart rate recovery has been consistently associated with higher risk of cardiac events regardless of age or fitness level. Many fitness watches track this automatically, making it easy to monitor over time.

Heart Rate Variability

Heart rate variability (HRV) measures the tiny fluctuations in time between each heartbeat. Counterintuitively, more variation is better. High HRV means your heart can adapt quickly to changing demands, whether that’s standing up from a chair, dealing with stress, or shifting from rest to movement. Low HRV suggests your cardiovascular system is less resilient and is more common in people with diabetes, high blood pressure, anxiety, and depression.

HRV naturally decreases with age, and what’s normal varies significantly from person to person, so comparing your numbers to a friend’s isn’t useful. What matters is your own trend over weeks and months. Most modern smartwatches and chest-strap heart monitors track HRV during sleep, which is the most consistent time to measure it. If your HRV is trending upward over time, your cardiovascular fitness is likely improving. A sustained downward trend is worth paying attention to.

Cholesterol and Triglycerides

A standard lipid panel, drawn from a basic blood test, gives you four key numbers. For most adults age 20 and older, the desirable targets are:

  • Total cholesterol: below 200 mg/dL
  • LDL (“bad” cholesterol): below 100 mg/dL
  • HDL (“good” cholesterol): 60 mg/dL or higher
  • Triglycerides: below 150 mg/dL

HDL acts as a cleanup crew, pulling excess cholesterol out of your arteries and carrying it back to your liver. LDL does the opposite, depositing cholesterol in artery walls where it can build up into plaques over decades. That’s why the ratio matters: you want HDL high and LDL low. Triglycerides, a type of fat in your blood, spike with sugary diets and excess alcohol and contribute independently to heart risk.

These numbers don’t cause symptoms until damage is advanced, which is why a blood test is the only way to know where you stand. Most guidelines recommend your first lipid panel by age 20, with repeat testing every four to six years if results are normal.

Blood Sugar

Chronically elevated blood sugar damages blood vessels from the inside, which is why diabetes is one of the strongest risk factors for heart disease. The most useful measure is hemoglobin A1c, a blood test that reflects your average blood sugar over the previous two to three months.

An A1c below 5.7% is considered normal and serves as the reference point for heart risk. Between 5.7% and 6.4% (the prediabetic range), the risk of cardiovascular events rises by roughly 25%. At 6.5% or above (the diabetic threshold), risk increases by about 43%. Even within the prediabetic range, the elevated sugar is already putting extra strain on your arteries, so catching it early gives you the most room to change course through diet, exercise, and weight management.

Inflammation

A blood marker called high-sensitivity C-reactive protein (hs-CRP) measures low-grade inflammation throughout your body. Chronic inflammation contributes to the buildup of arterial plaques, so this test adds information beyond what cholesterol alone can tell you. Johns Hopkins Medicine categorizes the results simply:

  • Low risk: below 1 mg/L
  • Intermediate risk: 1 to 3 mg/L
  • High risk: above 3 mg/L

This test isn’t part of routine screening for everyone, but it’s particularly useful if you have borderline cholesterol or other risk factors and want a fuller picture. Infections and injuries can temporarily spike CRP, so a single high reading should be confirmed with a repeat test a few weeks later.

Waist Size and Body Fat Distribution

Where you carry fat matters more than how much you weigh. Visceral fat, the deep belly fat surrounding your organs, is metabolically active and drives inflammation, insulin resistance, and arterial damage. Research from the American Heart Association has found that waist measurements predict heart attacks more accurately than BMI, especially in women.

The National Heart, Lung, and Blood Institute sets the risk thresholds at a waist circumference greater than 35 inches for women and greater than 40 inches for men. To measure, wrap a tape measure around your bare abdomen at the level of your navel, standing upright after a normal exhale. If you’re above those thresholds, reducing waist circumference even modestly, by two or three inches, meaningfully lowers your cardiovascular risk.

Exercise Capacity

How much physical work your body can handle is one of the strongest predictors of how long you’ll live, and it directly reflects heart function. Exercise capacity is measured in METs (metabolic equivalents), where 1 MET equals the energy you burn sitting still. Walking at 3 miles per hour is roughly 3 to 4 METs. Jogging is 7 to 8. Climbing stairs briskly is around 8 to 9.

Research published in Circulation identified age-specific MET thresholds that separate higher-risk individuals from lower-risk ones. If you’re under 50, you should be able to handle 8 to 9 METs (sustained jogging or vigorous cycling). Between 50 and 59, the threshold drops to 7 to 8 METs. At 60 to 69, it’s 6 to 7 METs. And at 70 and older, 5 to 6 METs. In practical terms, if you can jog for 10 minutes without stopping (around 8 METs) and you’re in your 40s, your heart is performing well. If you can’t sustain a brisk walk (about 4 METs) at any age, your exercise capacity is well below the safety threshold.

The good news is that exercise capacity is one of the most modifiable risk factors. Even small improvements, going from 4 METs to 6 METs over several months of consistent activity, produce significant reductions in cardiovascular risk.
