How to Make an Emergency Arm Sling Out of a Shirt

You can turn a button-up or pullover shirt into a functional arm sling in under two minutes. The goal is to hold your forearm across your chest with the elbow bent at roughly 90 degrees and the hand slightly higher than the elbow to limit swelling. There are two reliable methods depending on what type of shirt you have available.

Button-Up Shirt Method

A button-up shirt is the easiest option because the fabric naturally forms a pouch. Lay the shirt flat and fold it diagonally corner to corner so it forms a triangle. This is the same shape as a medical triangular bandage, and it works the same way.

Slide one long end of the triangle under the injured arm and up over the opposite shoulder. Let the fabric cradle the forearm so the elbow sits in the corner of the triangle. Bring the other long end up and over the shoulder on the injured side, then tie both ends together behind the neck. Adjust the knot so your forearm rests level or with the hand slightly above the elbow. Tuck any loose fabric around the elbow and pin or tie it so the arm doesn’t slip out.

Pullover T-Shirt Method

If you only have a t-shirt or pullover, you have two options. The first is to take the shirt off, fold it into a triangle the same way described above, and use it as a triangular sling. A large t-shirt gives you enough fabric to work with.

The second option works if you’re wearing the shirt and don’t want to remove it. Pin or tuck the bottom hem of your shirt up and over the injured forearm, securing the hem to the chest of the shirt with a safety pin, tape, or even a simple knot in the fabric. This creates a pouch that holds the arm against your body. It’s less secure than a proper triangular sling, but it keeps the arm still and elevated enough for a trip to get medical care.

Getting the Arm Position Right

The sling should hold your elbow at about a 90-degree angle, with your forearm roughly parallel to the ground. Your hand should sit slightly higher than your elbow. This position reduces blood pooling in the fingers and helps control swelling. Make sure the sling supports your wrist and hand, not just the forearm. If the hand hangs off the edge of the fabric, it can flop downward and increase pain, especially with a wrist or forearm injury.

For injuries to the hand, wrist, or lower forearm, you want the hand elevated even higher above the elbow. Pull the fabric so it cradles the hand closer to the opposite shoulder rather than letting it rest at waist level.

Protecting Your Neck

A shirt sling puts all the weight of your arm on one strap behind your neck, which gets uncomfortable fast. If you have any soft material available (a sock, a folded bandana, even a wad of paper towels), tuck it under the strap where it crosses the back of your neck. This spreads the pressure and prevents the fabric from digging into your skin. If you’re going to wear the sling for more than a few minutes, this padding makes a real difference.

Checking Circulation After You Tie It

Once the sling is in place, check your fingers. They should be their normal color and warm to the touch. Press a fingernail on the injured hand until it turns white, then release. The color should return within two seconds. If your fingers turn pale, blue, or cold, or if you feel tingling or numbness, the sling is too tight. Loosen the knot and readjust the fabric until circulation returns to normal. Recheck every 15 to 20 minutes, since swelling can increase over time and make a previously comfortable sling too restrictive.

When a Shirt Sling Isn’t Enough

A shirt sling is a temporary measure to stabilize an arm until you can get proper medical care. It’s not a substitute for treatment. Certain signs mean the injury needs emergency attention right away: bone visible through the skin, an arm or wrist bent at an unusual angle, severe swelling that develops rapidly, or inability to move the arm or rotate the wrist at all. If you heard a snap or crack at the time of injury, that also warrants urgent evaluation.

The sling’s job is to keep the arm still and supported during transport. Avoid adjusting the arm’s position if there’s a possible fracture. Immobilize it in whatever position is most comfortable and get to a medical facility.
