Most mild swelling from sprains, strains, or bumps responds well to these simple measures and begins improving within 48 to 72 hours. For swelling caused by chronic conditions or surgery, the same principles apply, but the timeline stretches longer and may require additional strategies like anti-inflammatory medication or dietary changes.
Why Your Body Swells in the First Place
Swelling happens when fluid leaks out of your blood vessels and pools in the surrounding tissue. After an injury, your immune system releases chemical signals called cytokines that make blood vessel walls more permeable. This is deliberate: the extra fluid carries immune cells to the damaged area so healing can begin. The problem is that this fluid buildup also causes pain, stiffness, and pressure that can slow your recovery if it’s not managed.
Understanding this helps explain why the standard treatments work. Ice narrows blood vessels to slow the flood of fluid. Compression physically limits how much space the fluid can occupy. Elevation uses gravity to drain fluid back toward your heart. Each approach targets a different part of the same process.
The RICE Method, Step by Step
RICE stands for Rest, Ice, Compression, and Elevation. It’s the standard first-aid approach recommended by emergency medicine providers for acute soft tissue injuries like sprains, strains, and bruises.
Rest means avoiding stress or strain on the injured area for at least the first few days. You don’t need to be completely immobile, but you should stop the activity that caused the injury and avoid putting weight or pressure on the area.
Ice is most effective in the first eight hours after injury. Apply it for 10 to 20 minutes at a time, every hour or two. Always place a thin cloth or towel between the ice and your skin to prevent frostbite. After the first day, you can continue icing if it provides relief, but its impact on swelling diminishes as the initial inflammatory response settles.
Compression means wrapping the swollen area with an elastic bandage. The wrap should feel snug but not tight enough to cause numbness, tingling, or increased pain below the bandage. If your fingers or toes turn blue or feel cold, loosen the wrap immediately.
Elevation works best when you raise the injured limb just above the level of your heart. If you’ve twisted an ankle, that means lying down and propping your foot on a pillow or two, not just resting it on an ottoman while you sit upright. The key is getting the swollen area higher than your chest so gravity can help pull fluid back into circulation.
Over-the-Counter Anti-Inflammatory Medication
Nonsteroidal anti-inflammatory drugs (NSAIDs) like ibuprofen and naproxen reduce swelling by blocking the chemical signals that trigger inflammation. They’re most useful when swelling is accompanied by significant pain.
For ibuprofen, the typical adult starting dose is 400 mg, followed by 200 to 400 mg every four hours as needed, up to four doses in 24 hours. For naproxen, the starting dose is 440 mg, followed by 220 mg every 8 to 12 hours, with a maximum of 660 mg per day. Take either with a full glass of water and food to reduce stomach irritation. These medications generally start relieving pain within 30 to 60 minutes, though their full anti-inflammatory effect builds over several days of consistent use.
Acetaminophen (Tylenol) helps with pain but does not reduce swelling. If your main goal is getting the puffiness down, an NSAID is the better choice, assuming you can take one safely.
What to Do After the First 48 Hours
Once the initial acute phase passes, gentle movement actually helps reduce lingering swelling. Contracting the muscles around a swollen joint acts like a pump, pushing trapped fluid back into your lymphatic system and veins. This is why physical therapists often recommend ankle pumps (pointing and flexing your foot) after an ankle injury, or gentle fist-making after a hand or wrist injury.
Some people find that switching from cold to warm compresses after the first two days helps. Warmth increases blood flow, which can speed the removal of waste products from damaged tissue. If warmth makes the swelling worse, go back to ice.
Keeping the area elevated remains useful well beyond the first day. Any time you’re sitting or lying down during recovery, prop the limb up above heart level. Consistent elevation over several days makes a noticeable difference in how quickly swelling resolves.
Compression Garments for Ongoing Swelling
If swelling persists beyond a typical injury recovery, compression sleeves or stockings provide sustained pressure that prevents fluid from pooling. Medical compression garments come in four classes based on how much pressure they apply, measured at the ankle:
- Class I (18 to 21 mmHg): light compression for mild swelling, heavy or tired legs
- Class II (23 to 32 mmHg): moderate compression for more significant swelling or varicose veins
- Class III (34 to 46 mmHg): firm compression for severe or chronic swelling
- Class IV (above 49 mmHg): very firm compression for advanced lymphatic conditions
The right class depends on your specific situation. Over-the-counter compression sleeves and socks typically fall in the Class I range and work well for mild post-injury or post-exercise swelling. Higher classes usually require a prescription and proper fitting.
Dietary Approaches That May Help
Certain foods and supplements have mild anti-inflammatory properties. Turmeric (specifically its active compound curcumin) and bromelain, an enzyme found in pineapple, have both shown measurable effects on swelling in clinical studies. One study using a combination of 500 mg bromelain and 200 mg curcumin daily found a statistically significant reduction in tissue swelling over time.
These supplements won’t replace ice and elevation for an acute injury, but they can be a useful addition for people dealing with ongoing or recurring swelling. Omega-3 fatty acids from fish, flaxseed, and walnuts also have well-documented anti-inflammatory effects when consumed regularly.
On the flip side, high sodium intake is one of the most common dietary causes of swelling, especially in the hands, feet, and face. Cutting back on processed foods, canned soups, and restaurant meals can reduce fluid retention noticeably within a few days.
When Swelling Is a Warning Sign
Most swelling from injuries or overuse is harmless and resolves with basic care. But certain patterns of swelling signal something more serious.
Swelling in one leg only, especially if accompanied by calf pain, warmth, redness, or a calf that measures more than 3 cm larger than the other side, can indicate a deep vein thrombosis (a blood clot). This risk increases if you’ve been bedridden for more than three days, had recent surgery, have active cancer, or have been immobilized in a cast. A blood clot in the leg can become life-threatening if it travels to the lungs, so unexplained one-sided leg swelling warrants urgent medical evaluation.
Swelling that affects both legs equally and worsens over weeks, or swelling in the face and around the eyes that appears in the morning, can point to problems with the heart, kidneys, or liver. Swelling at an injury site that keeps getting worse after 48 hours of RICE treatment, or swelling accompanied by fever, could mean an infection that needs medical attention.
Swelling that leaves a visible dent when you press on it (called pitting edema) and doesn’t resolve within a few days is also worth getting checked, particularly if you can’t connect it to a specific injury or activity.
