A blocked ear during a cold is caused by swollen, inflamed tissue around the small tubes that connect your middle ears to the back of your throat. These tubes, called Eustachian tubes, normally open and close to equalize air pressure and drain fluid. When a cold virus inflames the surrounding tissue, the tubes can’t open properly, trapping fluid and creating that familiar feeling of fullness, muffled hearing, or pressure. The good news: several home techniques can coax them open, and the blockage typically clears within a few days as your cold improves.
Why a Cold Blocks Your Ears
Your Eustachian tubes are only about 3 to 4 centimeters long, and they’re lined with the same mucous membrane that lines your nose and throat. When a cold virus triggers swelling in that membrane, the tubes narrow or seal shut entirely. Air can’t flow in or out of the middle ear, so pressure builds on one side of the eardrum. At the same time, fluid that would normally drain down the back of your throat gets trapped behind the eardrum. That combination of negative pressure and fluid is what makes your ear feel plugged, achy, or like you’re hearing underwater.
Physical Techniques to Pop Your Ears
The simplest approach is swallowing or yawning repeatedly. Both actions pull the Eustachian tubes open briefly, which can equalize pressure. Chewing gum works on the same principle. If those don’t do enough, try one of these more targeted maneuvers:
- Pinch-and-blow (Valsalva maneuver): Pinch your nostrils shut, close your mouth, and gently blow through your nose. You should feel a soft pop as air pushes into the middle ear. Blow gently and hold for no more than five seconds. Blowing too hard can damage the delicate membranes of the inner ear.
- Pinch-and-swallow (Toynbee maneuver): Pinch your nostrils shut and swallow. The swallowing motion pulls the Eustachian tubes open while the closed nose compresses a small puff of air against them.
- Jaw-and-yawn technique: Push your jaw forward and down as if starting a big yawn while tensing the muscles at the back of your throat. This physically tugs the Eustachian tubes open without any pressure from blowing.
A few important cautions with the pinch-and-blow method. If the tubes are already swollen shut, forcing air against them can lock them tighter rather than opening them. Blowing too hard can also rupture the round and oval windows of the inner ear. Keep it gentle. If you feel pain or dizziness, stop immediately.
Steam and Warm Compresses
Breathing in steam can help shrink swollen tissue in your nasal passages and around the Eustachian tube openings. Take a hot shower, or drape a towel over your head and lean over a bowl of hot water for 10 to 15 minutes. The moist heat loosens mucus and encourages drainage, which indirectly takes pressure off your ears.
For the ear pain itself, a warm washcloth or a heating pad set on low provides relief. Place a thin cloth between the heating pad and your skin to prevent burns. Heat won’t fix the underlying blockage, but it can ease the aching while you wait for the swelling to go down.
Over-the-Counter Decongestants
If physical techniques and steam aren’t enough, a decongestant can shrink the swollen tissue more aggressively. The two most common active ingredients are pseudoephedrine (sold as Sudafed) in pill form and oxymetazoline (sold as Afrin) as a nasal spray. Both reduce swelling in the nasal passages and around the Eustachian tube openings, making it easier for air and fluid to move through.
Nasal sprays work faster because they deliver the medication directly to the swollen tissue. However, there’s a strict time limit: do not use decongestant sprays for more than three days. After that, they can trigger rebound congestion, a condition called rhinitis medicamentosa, where the spray itself causes the swelling to worsen. You end up more blocked than when you started. Oral decongestants like pseudoephedrine don’t carry this rebound risk, but follow the dosing instructions on the package and be aware they can raise blood pressure or cause jitteriness in some people.
Sleep Position for Drainage
Gravity matters when you’re trying to drain a blocked ear. If only one ear is affected, sleep on the opposite side so the blocked ear stays elevated. This lets fluid drain downward rather than pooling against your eardrum. If you sleep on your back, add an extra pillow to elevate your head and upper body. Even a modest incline helps fluid move toward the throat where it can drain naturally. This won’t produce instant results, but over several hours of sleep it can noticeably reduce that morning-ear-stuffed feeling.
What a Normal Timeline Looks Like
Cold-related ear blockage usually follows the arc of the cold itself. Expect the worst stuffiness around days two through four of your cold, with gradual improvement after that. Most people notice their ears clearing within a week as the viral inflammation subsides. The blocked feeling sometimes lingers a day or two after your other cold symptoms have resolved, which is normal since the Eustachian tubes are among the last areas to fully open back up.
If your symptoms haven’t improved within 48 to 72 hours, or if they’re actively getting worse, that can signal a secondary bacterial infection in the middle ear. Watch for sharp or worsening ear pain, fever, fluid or pus draining from the ear, or noticeable hearing loss. A high fever, severe pain behind the ear, or any facial weakness or paralysis warrants immediate medical attention. In children under six months, any ear infection symptoms should be evaluated promptly. For older children, decongestant nasal sprays should not be used for more than five days.
Putting It All Together
The most effective approach combines several of these strategies rather than relying on just one. Start with steam inhalation to loosen things up, then try the pinch-and-swallow or jaw-thrust technique to pop the tube open. Use a decongestant if the swelling is too stubborn for physical maneuvers alone, but keep spray use under three days. Sleep elevated on the unaffected side. Repeat the physical techniques several times a day, especially after a hot shower when the tissue is most relaxed. For most colds, this routine clears the blockage within a few days without any medical intervention.
