Once a cataract has started forming, you can’t reverse it, but you can take meaningful steps to slow its progression and protect the vision you have. Cataracts develop when proteins in your eye’s lens clump together and cloud your sight, a process driven by oxidative damage, UV exposure, and metabolic factors you have some control over. The strategies below won’t eliminate a cataract, but they can buy you years before surgery becomes necessary.
Protect Your Eyes From UV Light
Ultraviolet radiation is one of the most well-established accelerators of cataract growth. UV rays damage the proteins in your lens over time, causing them to break down and clump together faster. This is cumulative damage, meaning every hour of unprotected sun exposure adds to the total load your lenses have absorbed over your lifetime.
The single most effective daily habit you can adopt is wearing sunglasses that block 100% of UV rays. Look for lenses labeled “UV400,” which filter both UVA and UVB wavelengths. Wrap-around styles offer extra protection by blocking light that enters from the sides. A wide-brimmed hat paired with good sunglasses cuts UV exposure to your eyes even further. This matters year-round, not just in summer. UV rays reflect off water, snow, and pavement even on overcast days.
Quit Smoking
Smoking floods your body with free radicals that directly damage lens proteins. It also depletes the antioxidants your eyes rely on for protection. A large meta-analysis published in Investigative Ophthalmology & Visual Science found that quitting does reduce your risk over time, though the recovery is gradual. Data from the Swedish Mammography Cohort showed that people who had quit more than 20 years earlier no longer carried any excess cataract risk compared to people who never smoked.
Other research from the Nurses’ Health Study and the Health Professionals’ Follow-up Study found that risk dropped steadily with each year after quitting, though it may never fully return to the level of a lifelong nonsmoker. The takeaway: some of the damage smoking does to your lens is reversible, but it takes time. The sooner you quit, the more you stand to gain.
Keep Blood Sugar Under Control
High blood sugar accelerates cataract formation through a process called glycation, where excess glucose binds to lens proteins and causes them to stiffen and clump. This is why people with diabetes develop cataracts earlier and more frequently than the general population. If you have diabetes or prediabetes, keeping your blood sugar well managed is one of the most important things you can do for your eyes. Consistently elevated levels speed up lens clouding in a way that compounds over months and years.
Watch Your Alcohol Intake
Alcohol’s relationship to cataracts follows a pattern researchers see with many health outcomes: moderate intake appears neutral or slightly protective, while heavy drinking raises risk. A large study reviewed by the American Academy of Ophthalmology found that people who drank one to four times per week had a 6% to 7% lower risk of needing cataract surgery compared to nondrinkers. Heavy or daily drinkers, on the other hand, had a 5% to 6% higher risk than moderate drinkers.
You don’t need to eliminate alcohol entirely, but if you’re drinking daily or heavily, cutting back is a reasonable step for your eye health alongside the many other benefits.
Talk to Your Doctor About Steroid Medications
Corticosteroids, whether taken as pills, inhaled for asthma, or used as eye drops, are a known cause of a specific type of cataract that forms on the back surface of the lens. The risk is dose-dependent. A study in Ophthalmology found that topical corticosteroid eye drops more than doubled the risk of cataract formation, but this risk dropped significantly at lower doses. At two or fewer drops daily, new cataract formation essentially dropped to zero in the study population.
If you’re using corticosteroids regularly for any condition, this doesn’t mean you should stop them on your own. But it’s worth having a conversation with your prescribing doctor about the lowest effective dose and whether alternative medications might work for your situation. Short-term use carries far less risk than ongoing daily treatment.
What About Supplements?
You’ll find plenty of claims that vitamins and antioxidant supplements can slow cataracts. The evidence, unfortunately, is disappointing. The largest and most rigorous trial on this topic, AREDS2 (Age-Related Eye Disease Study 2), tested daily supplementation with lutein (10 mg) and zeaxanthin (2 mg) in thousands of participants. After five years, there was no statistically significant difference in cataract surgery rates or vision loss between the supplement group and the placebo group.
That said, eating a diet rich in fruits and vegetables still makes sense. Leafy greens, citrus fruits, and colorful vegetables supply a broad range of antioxidants in forms your body absorbs well. The benefit of whole foods may come from the combination of nutrients working together rather than isolated supplements. You just shouldn’t expect a pill to meaningfully slow cataract progression based on current evidence.
Other Habits That Help
Beyond the major factors, a few additional strategies are worth incorporating. Staying well hydrated supports the fluid balance in your eye. Wearing protective eyewear during activities that could injure your eye matters too, since trauma to the lens can trigger or accelerate cataract formation. If you spend significant time in front of screens or under fluorescent lighting, giving your eyes regular breaks reduces overall strain, though this is more about comfort than cataract progression specifically.
Regular eye exams are essential for tracking how your cataracts are changing. Your eye doctor can measure the density and location of the clouding over time and help you decide when, if ever, surgery makes sense. Many people live with early cataracts for years or even decades before they affect daily life enough to warrant intervention.
When Slowing Isn’t Enough
Cataract surgery is one of the most common and successful procedures in medicine, with a success rate above 95%. It involves removing the clouded lens and replacing it with a clear artificial one. The procedure typically takes under 30 minutes, and most people notice dramatically improved vision within days. Knowing this can take some of the pressure off. The goal of slowing cataract progression isn’t to avoid surgery forever. It’s to preserve your quality of vision for as long as possible and let you choose the timing on your terms rather than being forced into it.
