How to Prevent Hip Dysplasia in Dogs From Puppyhood

Hip dysplasia is one of the most common orthopedic problems in dogs, but a combination of smart breeding choices, proper nutrition, controlled exercise, and the right environment can significantly reduce the risk. The condition develops when the ball and socket of the hip joint don’t fit together properly, leading to looseness, grinding, and eventually arthritis. While genetics play the largest role, environmental factors during the first several months of a puppy’s life can make the difference between a dog that develops symptoms and one that doesn’t.

Start With the Breeder

The single most effective way to prevent hip dysplasia is to choose a puppy from parents with certified hip evaluations. Several grading systems exist worldwide. In the United States, the Orthopedic Foundation for Animals (OFA) evaluates hip radiographs taken under sedation or anesthesia and assigns a rating. PennHIP uses a different approach, measuring how far the femoral head can be pulled from the socket to generate a “distraction index” representing passive hip laxity.

When evaluating a breeder, don’t just look at the parents’ scores. Selection of breeding stock should be based on familial data, meaning the depth and breadth of hip normalcy across the pedigree, not just the individual dog’s results. A puppy from two parents with “Good” or “Excellent” hips but a pedigree full of dysplastic siblings and cousins carries more risk than the parents’ scores alone suggest. On the other end, breeds like Italian Greyhounds, Chinese Cresteds, and American Hairless Terriers report 0% dysplasia in evaluated dogs. If you own a high-risk breed, every other prevention strategy on this list becomes more important.

Get the Nutrition Right Early

Overfeeding a large breed puppy is one of the fastest ways to set the stage for hip problems. Rapid growth doesn’t make a dog bigger as an adult; it just gets them to their adult size faster, and that speed puts stress on developing joints before the bones and cartilage are ready. The goal is slow, steady growth.

Choose a food specifically formulated for large breed puppies. These diets are lower in caloric density and carefully control calcium and phosphorus levels. The ideal calcium-to-phosphorus ratio falls between 1.1:1 and 2:1, which maintains proper hormonal balance for bone development. Recommended diets should have a caloric density between 3,200 and 4,100 kcal per kilogram of food. Avoid supplementing calcium on top of a balanced commercial diet. Excess calcium is more dangerous for large breed puppies than a slight deficiency, because it interferes with normal bone remodeling.

Keep your puppy lean. You should be able to feel ribs easily under a thin layer of fat, and see a visible waist when looking down from above. Extra body weight on immature joints accelerates the damage from any existing laxity, turning mild looseness into a clinical problem.

Provide the Right Surfaces From Day One

Hip joint development is surprisingly vulnerable to something most owners never think about: flooring. A puppy’s hip joint at birth is mostly cartilage, and it converts to bone over the first five months of life. Proper development depends on the head of the femur sitting squarely in the center of the hip socket during this period. When a puppy walks or pushes off on a slippery surface, its legs splay outward and extend fully, pulling the femoral head away from the socket. This strains the ligament that holds the hip together and can create the laxity that leads to dysplasia.

This risk begins in the whelping box. Research from the Institute of Canine Biology found that standard whelping surfaces like vet bed, rubber mats, carpet, and whelping pads did not provide adequate traction for every breed tested. The only surface that consistently prevented full leg extension in all puppies was a rough coconut fiber mat. If you’re a breeder, this is one of the cheapest and most impactful interventions available.

For puppy owners, the same principle applies at home. Hardwood floors, tile, and laminate are all problematic for young puppies. Use area rugs, rubber-backed runners, or yoga mats in areas where your puppy spends the most time. Pay special attention to high-traffic paths and the areas around food bowls where puppies brace and push with their hind legs.

Exercise Smart During Growth

Puppies need physical activity for healthy joint development, but the wrong kind of exercise at the wrong age can cause real harm. The key distinction is between self-directed, varied movement and repetitive, high-impact activity.

Avoid jogging, cycling alongside your puppy, or intense ball-chasing with launchers. These activities are concussive and repetitive, stressing the same joint surfaces over and over before they’ve matured. Interestingly, brisk walking on a short leash can also be problematic for young pups, because it restricts them to walking or trotting in a straight line without developing balance, coordination, and spatial awareness.

Instead, let puppies explore varied terrain at their own pace. Off-leash play in a safe area, gentle play with other dogs of similar size, and short walks on natural surfaces like grass or dirt are all ideal. For exercise duration, start with 15 to 30 minutes per session between two and four months of age, then gradually extend to 45 to 60 minutes between four and six months. Let your puppy set the pace. If they stop and sit down, the session is over.

Most dogs reach skeletal maturity between 12 and 18 months depending on breed, with giant breeds taking longer. High-impact activities like distance running, agility jumping, and repetitive fetch are safer once growth plates have closed, which your vet can confirm with radiographs if you’re unsure.

Maintain a Healthy Weight for Life

Weight management doesn’t stop after puppyhood. Excess weight is the most modifiable risk factor for hip dysplasia symptoms throughout a dog’s entire life. A landmark study of Labrador Retrievers found that dogs kept at a lean body condition developed radiographic signs of hip dysplasia at significantly lower rates, and those that did develop it showed symptoms years later than their heavier counterparts. Even dogs with genetic predisposition can live comfortably if their joints aren’t carrying extra load.

This means measuring food rather than free-feeding, adjusting portions as activity levels change with age, and limiting treats to no more than 10% of daily calories. If your dog is already overweight, slow and steady weight loss under veterinary guidance can meaningfully reduce joint stress.

Early Screening and Surgical Prevention

For puppies in high-risk breeds, early veterinary screening can catch hip laxity before it becomes a structural problem. PennHIP evaluations can be performed as early as 16 weeks, giving owners actionable information while the puppy is still growing.

If significant laxity is detected early, a minor procedure called juvenile pubic symphysiodesis (JPS) can alter the way the pelvis grows to improve hip joint coverage. The procedure works by fusing a small growth plate in the pelvis, which causes the hip sockets to rotate over the femoral heads as the puppy continues to grow. Research has shown significant improvements in hip joint conformation and laxity in dysplastic puppies treated at both 15 and 20 weeks of age, with larger and faster changes when surgery was performed at 15 weeks. The window for this procedure is narrow, typically closing around 20 weeks, so early evaluation is essential for puppies at risk.

JPS is a relatively simple procedure with a quick recovery compared to the major surgeries (like total hip replacement) that may be needed later if dysplasia progresses. For owners of breeds with dysplasia rates above 20 to 30%, scheduling a PennHIP evaluation around 16 weeks is a practical step that can open this option if needed.

Putting It All Together

No single strategy eliminates hip dysplasia risk entirely, because the condition involves multiple genes interacting with the environment. But stacking these interventions together, choosing health-tested parents with strong pedigrees, feeding an appropriate large breed puppy diet, providing high-traction surfaces, exercising thoughtfully, maintaining lean body condition, and screening early, gives your dog the best possible chance. Each factor you control shifts the odds meaningfully, especially for breeds that carry higher genetic risk.
