Facial volume loss is driven by changes in three layers: bone, deep fat pads, and skin. It typically becomes noticeable in your mid-30s to early 40s and accelerates from there. The good news is that several factors are within your control, and understanding what’s actually shrinking (and why) helps you target prevention where it matters most.
What’s Actually Shrinking in Your Face
Most people assume facial aging is just about skin. In reality, the scaffolding underneath is collapsing. Your facial skeleton literally shrinks with age. The eye sockets widen, the cheekbone area (maxilla) recedes, and the jawbone loses height and length. The maxilla is especially vulnerable, with the angle between the upper jaw and the rest of the face decreasing by about 10 degrees between your 20s and 60s. The nasal opening enlarges as surrounding bone pulls back. These changes remove the structural platform that everything else sits on.

Rapid or significant weight loss is one of the fastest ways to lose facial volume. The cheek fat pad and the fat pad near the temples are hit hardest. Fat Grafting for Lasting Restoration
For people who have already experienced significant volume loss, fat grafting transfers fat from one part of your body to your face. The appeal is that it uses your own tissue rather than a synthetic filler, and surviving fat cells can provide permanent volume. The challenge is unpredictability: clinical studies report retention rates between 30% and 70%, with graft loss rates of 40% to 60%. Most people need more than one session to achieve their goal.
Several factors influence how much fat survives. Smaller graft volumes do better than larger ones, since oversized grafts are more prone to tissue death. The method used to harvest the fat matters significantly: suction- and ultrasound-assisted approaches preserve cell viability far better than laser-assisted liposuction. After the procedure, avoiding pressure on the treated area and not smoking are critical for retention.
Newer techniques are improving these numbers. Enriching fat grafts with concentrated stem cells from the same tissue (a technique called cell-assisted lipotransfer) has shown a 35% higher survival rate compared to standard grafting. Daily Habits That Add Up
Sun protection is non-negotiable. UV radiation breaks down collagen and elastin directly, and the cumulative effect over decades is substantial. Daily broad-spectrum sunscreen, even on cloudy days, is the single most impactful preventive habit for maintaining skin structure.
Smoking accelerates every layer of facial volume loss. It constricts blood vessels that feed the skin and fat pads, degrades collagen, and reduces the skin’s ability to repair itself. The repetitive facial movements of smoking also deepen lines around the mouth. If you smoke, quitting is one of the most effective anti-aging interventions available.
Sleep position can contribute to asymmetric volume changes over time. Side sleepers consistently compress the fat pads on one side of the face, which may accelerate their breakdown. Sleeping on your back or using a contoured pillow that reduces facial contact helps distribute pressure more evenly. Staying well-hydrated supports skin plumpness in the short term and helps maintain the extracellular matrix that cushions deeper structures.
Strength training and adequate protein intake support the musculoskeletal health of your entire body, including the facial skeleton. While you can’t fully prevent bone resorption in the face, maintaining overall bone density through weight-bearing exercise, calcium, and vitamin D gives your facial structure the best chance of holding up over time.
