How to Prevent Your Bunions from Getting Worse

Bunions are progressive, meaning they almost always get worse over time if nothing changes. But the speed at which they worsen depends heavily on what you do about them now. The right shoes, targeted exercises, and a few daily habits can significantly slow progression and, in some cases, even slightly reverse the angle of the big toe joint.

Understand Where Your Bunion Stands

Bunions are measured by the angle your big toe deviates toward your other toes. A mild bunion has an angle under 30 degrees, moderate falls between 30 and 40 degrees, and severe is anything above 40 degrees. Knowing your severity matters because the strategies that work best differ at each stage, and catching progression early gives you the most room to intervene. If you haven’t had an X-ray, your doctor can measure this angle in minutes.

Choose Shoes That Actually Fit

Footwear is the single biggest controllable factor. Tight, narrow shoes don’t just hurt an existing bunion; they actively push the joint further out of alignment with every step. The fix isn’t necessarily buying a “wide” shoe. Many people, especially women, need a wide toe box specifically, not a wider shoe overall.

Here’s what to look for:

  • Wide, tall toe box. Your toes need room to spread both sideways and upward. If any toe touches the top or side of the shoe, it’s too small. Avoid tapered or pointy toe boxes entirely.
  • Soft, stretchy upper material. A stiff upper presses directly against the bunion. Knit or pliable materials conform to your foot instead of forcing your foot to conform to the shoe.
  • Low to moderate heel. Higher heels shift your body weight forward onto the ball of the foot, increasing pressure on the bunion joint. Flat or low-drop shoes (under 8mm of heel-to-toe drop) distribute weight more evenly.
  • Moderate flexibility. The sole should bend enough for comfortable walking but not be so floppy that it offers no support. Too much flexibility can allow excess motion at the big toe joint.

If you spend long hours on your feet at work, these shoe features matter even more. Prolonged standing in poorly fitting shoes compounds the mechanical stress on the joint throughout the day.

Strengthen the Muscles Around the Joint

Your big toe has its own set of small muscles that help hold it in alignment. In most people with bunions, these muscles have weakened over time, often from years of wearing shoes that don’t let the toes move freely. Rebuilding that strength can stabilize the joint and slow further drift.

A few exercises with solid clinical support:

  • Toe spread-outs. Sit with your foot flat on the floor, heel down. Lift and spread all your toes as wide as you can, hold briefly, then relax. Repeat 10 to 20 times per foot. This targets the muscle that pulls the big toe away from the other toes.
  • Resistance band abduction. Loop a small exercise band around both big toes. Pull your feet apart so the band creates resistance against each big toe. Hold for a few seconds, release, and repeat. This directly strengthens the muscle responsible for keeping the big toe straight.
  • Towel scrunches. Place a towel on the floor and use your toes to scrunch it toward you. This builds overall intrinsic foot strength, which supports the arch and reduces abnormal loading on the bunion joint.

Consistency matters more than intensity. A few minutes daily will do more than a long session once a week.

Orthotics, Spacers, and Splints

Toe spacers, bunion splints, and custom orthotics are among the most common non-surgical tools people reach for. They’re not all equally effective, and it helps to know what the evidence actually shows.

A 12-month clinical study compared three types of orthotics. Rigid biomechanical orthotics, the kind that physically hold and adjust the big toe’s position, produced the most consistent results. In patients with moderate bunions, this type reduced the bunion angle by about 5 degrees over a year. That’s a meaningful change, enough to move someone from one severity category closer to another. Wrap-style splints with a metal bar reduced the angle by roughly 2.4 degrees in mild cases, though results were less consistent. Simple silicone gel toe spacers, the inexpensive kind you slip between your toes, did reduce the angle somewhat but showed volatile results with no statistically significant improvement.

The takeaway: silicone spacers can provide comfort and temporary separation, but if you want to actually change the alignment over time, a structured orthotic with real corrective force is more likely to deliver. A podiatrist can fit you for one that matches your foot’s specific biomechanics.

Taping for Daily Support

Kinesiology tape offers a lightweight option for day-to-day support, especially during activities that stress the joint. The tape gently guides the big toe toward a straighter position, redistributes pressure around the bunion, and can reduce pain during walking or exercise.

A basic method: cut a strip long enough to wrap around your big toe and extend past the bunion. Start from the top of the big toe, wrap it around and over the bunion area, and apply additional strips in an X pattern for extra support. The tape should pull the toe gently inward without cutting off circulation. It works best as a complement to proper footwear and exercises, not as a standalone fix.

Manage Inflammation Before It Compounds

When a bunion flares up, the fluid-filled sac over the joint (the bursa) becomes inflamed and swollen. Repeated inflammation episodes can thicken the tissue around the joint, making the bunion appear larger and feel stiffer. Controlling flare-ups quickly helps prevent this buildup.

Ice is the simplest tool. Place an ice cube in a plastic bag and rub it directly over the bunion for about 5 minutes. Repeat two or three times a day during a flare. Over-the-counter anti-inflammatory medications can also help bring down swelling, but ice applied consistently works well for localized joint inflammation like this.

Pay attention to what triggers your flare-ups. A long day in dress shoes, a hike in stiff boots, or an extra-intense workout can all set one off. Identifying your triggers lets you plan around them or at least prepare with better footwear and post-activity icing.

Watch Your Weight

Every pound of body weight translates to several pounds of force on your feet during walking. Carrying extra weight increases the mechanical load on the big toe joint specifically, accelerating misalignment and fueling inflammation. Maintaining a healthy weight reduces that load and can noticeably slow progression. The research linking high BMI directly to bunion worsening is still developing, but the basic biomechanics are straightforward: less weight means less force pushing the joint out of place with every step.

When Conservative Measures Aren’t Enough

Everything above works best for mild to moderate bunions, and the earlier you start, the more effective these strategies are. But bunions can reach a point where no amount of spacers, exercises, or shoe changes will reverse the damage. Surgery becomes worth considering when the bunion causes disabling pain, prevents you from doing activities you care about, creates a skin ulcer over the joint, or when the big toe has pushed so far that the second toe is at risk of dislocating.

If your bunion doesn’t significantly bother you, surgery isn’t necessary regardless of how it looks. It carries its own recovery period and risks, so it’s best reserved for cases where conservative approaches have genuinely failed to keep you functional and comfortable.
