How to Read Your Blood Pressure: What the Numbers Mean

A blood pressure reading has two numbers separated by a slash, like 120/80. The top number (systolic) measures the force of blood pushing against your artery walls when your heart beats. The bottom number (diastolic) measures that same force between beats, when your heart is resting. Both numbers are measured in millimeters of mercury (mmHg), and together they tell you whether your cardiovascular system is under too much strain.

What the Two Numbers Mean

Think of your heart as a pump that squeezes and relaxes about 100,000 times a day. Each squeeze sends a wave of blood through your arteries, and that peak pressure is your systolic number. Between squeezes, the pressure drops but doesn’t reach zero because your arteries maintain some baseline tension. That lower pressure is your diastolic number.

Both numbers matter, but they carry different weight depending on your age. For people over 50, the systolic (top) number is generally a stronger predictor of heart disease risk. That’s because arteries stiffen with age and plaque accumulates over time, pushing that top number higher even when the bottom number stays the same. In younger adults, an elevated diastolic number can be an early signal worth paying attention to.

Blood Pressure Categories

The 2025 guidelines from the American Heart Association and American College of Cardiology define four categories based on readings taken in a healthcare setting:

  • Normal: below 120/80 mmHg
  • Elevated: 120 to 129 systolic and below 80 diastolic
  • Stage 1 Hypertension: 130 to 139 systolic or 80 to 89 diastolic
  • Stage 2 Hypertension: 140 or higher systolic or 90 or higher diastolic

If your systolic and diastolic numbers fall into two different categories, you go by whichever category is higher. So a reading of 135/72 counts as Stage 1 Hypertension because the top number lands in that range, even though the bottom number is normal.

A reading at or above 180/120 is a hypertensive crisis. If that number comes with chest pain, shortness of breath, blurred vision, confusion, or stroke symptoms like sudden numbness or tingling on one side of the body, call 911 immediately. Even without symptoms, a reading that high needs prompt medical attention.

How to Take an Accurate Reading at Home

A home reading is only useful if it’s taken correctly. Small mistakes can inflate or deflate your numbers enough to change which category you fall into. Here’s the preparation that matters:

Avoid caffeine, alcohol, smoking, and exercise for at least 30 minutes before measuring. Then sit quietly for five minutes before you start. That resting period lets your cardiovascular system settle to its true baseline. During those five minutes, sit in a chair with your back supported and both feet flat on the floor. Don’t cross your legs.

Place the cuff on your bare upper arm, about one inch above the bend of your elbow. Rest your arm on a table so it’s level with your heart. If your arm is dangling at your side or raised above your chest, the reading won’t be accurate. Keep your palm facing up and stay still. Don’t talk during the measurement.

Cuff Size Matters

A cuff that’s too small will give you a falsely high reading, and one that’s too large will read low. The bladder inside the cuff (the inflatable part) should wrap at least 80 percent of the way around your upper arm in length, and its width should be at least 37 percent of your arm circumference. Most home monitors come with a standard cuff sized for arms between roughly 9 and 13 inches around. If your arm falls outside that range, you’ll need a different size.

How Many Readings You Actually Need

A single reading is a snapshot, not a diagnosis. Blood pressure fluctuates throughout the day based on stress, hydration, meals, and dozens of other factors. The American Heart Association recommends taking two readings at least one minute apart, both in the morning and in the evening. That’s four readings per day. Do this for at least three days, ideally seven, to get a reliable average. That gives you somewhere between 12 and 28 total readings to work with.

Once your blood pressure is stable and well-controlled for several months, you can scale back to monitoring one to three days per week. Discard the readings from the first day, since they tend to run higher from the novelty of measuring. Average the remaining readings to get your true baseline number.

Why Home and Office Readings Can Differ

If your blood pressure reads high at the doctor’s office but normal at home, you may have what’s called white coat hypertension. The stress of a clinical visit pushes your numbers up. Under current U.S. guidelines, this is defined as an office reading of 130/80 or higher paired with home readings that stay below 130/80.

The reverse also happens. Masked hypertension means your readings look normal in the office but run high in everyday life. This is harder to catch and potentially more dangerous because it goes undetected without home monitoring. If your office readings hover around 120 to 129 systolic with a diastolic under 80 (the “elevated” range), home monitoring is especially useful to rule out masked hypertension.

Both of these patterns are common enough that home monitoring has become a standard part of blood pressure management, not just a convenience. Your home readings give a more complete picture of what’s happening across a typical day, outside the artificial environment of a clinic.

Common Mistakes That Skew Your Numbers

Research on automated blood pressure measurement has identified a consistent set of errors, even in clinical settings. At home, these same mistakes are easy to make without realizing it:

  • Talking during the reading raises both numbers.
  • Crossing your legs restricts blood flow and inflates the reading.
  • Unsupported back means your core muscles are working to hold you upright, which can increase pressure.
  • Placing the cuff over clothing dampens the sensor’s accuracy.
  • Skipping the five-minute rest captures residual stress or activity rather than your resting pressure.
  • Taking only one reading misses the natural variation between measurements.

Any one of these can push your numbers several points in either direction. Stack two or three together and the reading becomes unreliable. The fix is simple: build a short routine. Sit down, set a five-minute timer, position your arm, take two readings a minute apart, and record the average. The whole process takes under ten minutes.

Tracking Your Numbers Over Time

Write down every reading with the date, time, and which arm you used, or use the memory function on your monitor if it has one. Patterns matter more than any single reading. A steady creep upward over weeks is more meaningful than one high reading after a stressful day. When you visit your doctor, bring your log. Home readings combined with office readings give the most complete picture of your cardiovascular health, and they’re often the basis for treatment decisions.
