Proper side sleeping comes down to keeping your spine in a straight, neutral line from your head to your hips. That means your head shouldn’t tilt up or down, your shoulders should stay stacked, and your hips shouldn’t twist or sag. Getting this right reduces pressure on your joints, eases back pain, and may even help your brain clear waste more efficiently while you sleep. Here’s how to set up each part of your body for better side sleeping.
How to Align Your Spine
Start by lying on your side with your head on a pillow. Your chin and neck should be centered between your shoulders, and your shoulders should be in line with your hips. Keep your head facing forward so your chin doesn’t tilt down toward your chest or twist to one side. Your arms and hands should be aligned with each other, resting either by your sides or slightly in front of you.
The goal is a straight horizontal line running from the top of your head through your spine to your tailbone. If someone looked at you from behind, your spine should look the same as it does when you’re standing with good posture. Any bend, sag, or twist in that line means something needs adjusting, usually your pillow height, your mattress, or your leg position.
Choosing the Right Pillow Height
Your pillow has one job: fill the gap between your head and the mattress so your neck stays level with the rest of your spine. Side sleepers need a higher, firmer pillow than back or stomach sleepers because the shoulder creates a wider gap. A pillow in the 4 to 6 inch range is a good starting point, but the right height depends on your shoulder width. Broad-shouldered people need a higher loft; narrower frames need less.
If your pillow is too thin, your head drops toward the mattress and your neck bends downward. Too thick, and your head gets pushed up, kinking your neck in the opposite direction. Both create strain you’ll feel by morning. Memory foam and latex hold their shape better through the night than down or cotton, which tend to compress and lose height as you sleep.
What to Do with Your Legs
Placing a pillow between your knees is the single most effective tweak for side sleepers dealing with hip or lower back discomfort. Without it, your top leg tends to fall forward and pull your pelvis into a rotated position, twisting your lower spine. A knee pillow keeps your pelvis neutral and prevents that rotation throughout the night.
This matters especially if you have sciatica, a herniated disc, or general lower back stiffness. Keeping your spine untwisted reduces compression on inflamed nerves and strained muscles. A standard bed pillow works fine, though contoured knee pillows stay in place more easily. The pillow should sit between your knees and extend partway down your shins so your legs stay parallel.
Preventing Shoulder Pain
The bottom shoulder takes the most abuse during side sleeping. It bears a significant portion of your body weight, and if your mattress doesn’t let it sink in enough, pressure builds quickly. Two adjustments help.
First, avoid lying directly on the point of your shoulder. Let your body roll very slightly forward so the weight distributes across more of your upper arm and chest. Second, hug a medium-firm pillow against your chest and rest your top arm on it. This props up the top arm so it doesn’t drop and pull the shoulder joint forward. It keeps the joint in an open, neutral position and lets the muscles around your shoulder blade and rotator cuff relax instead of working all night to stabilize the arm.
If you already have shoulder pain, sleep on the opposite side. Sleeping on a painful shoulder compresses it further and slows recovery.
Left Side vs. Right Side
For most people, either side works fine. The main exception is acid reflux. A study monitoring 57 people with chronic heartburn found that sleeping on the left side didn’t reduce the number of reflux episodes, but acid cleared from the esophagus significantly faster compared to right-side or back sleeping. If nighttime heartburn is a regular problem for you, the left side is worth trying.
During pregnancy, many doctors recommend left-side sleeping, especially in the third trimester. The reasoning is that the growing uterus can compress major blood vessels when you lie on your back, potentially reducing blood flow to the baby. An NIH-funded study found that sleeping position through the 30th week of pregnancy did not appear to increase the risk of stillbirth or other complications. However, the study didn’t evaluate sleep positions beyond 30 weeks, so left-side sleeping remains the standard recommendation for late pregnancy.
Picking a Mattress That Works
Side sleeping concentrates your body weight into two narrow contact points: your shoulders and your hips. A mattress that’s too firm presses back against these areas, creating pressure that wakes you up at night. A mattress that’s too soft lets your hips sink deeper than the rest of your body, bending your spine out of alignment.
The right firmness depends largely on your weight. On a 1 to 10 firmness scale:
- Under 130 pounds: Soft to medium (3 to 5)
- 130 to 230 pounds: Medium to medium-firm (5 to 6)
- Over 230 pounds: Firm (7 to 8)
Memory foam mattresses distribute weight evenly and cradle the shoulders and hips, which is why they tend to perform well for side sleepers. Hybrid mattresses with zoned coil systems offer a good balance, providing extra support under the lower back while allowing the shoulders to sink in. Traditional innerspring mattresses with minimal padding are generally the worst match for side sleeping because metal coils create sharper pressure against the hips and shoulders.
If your current mattress is too firm but you’re not ready to replace it, a 2 to 3 inch memory foam topper can add enough contouring to relieve shoulder and hip pressure.
Why Side Sleeping Benefits Your Brain
Your brain has a waste-removal system that works primarily during sleep. As you sleep, the spaces between brain cells expand, allowing fluid to flow through and flush out metabolic waste, including a protein called amyloid beta that’s associated with Alzheimer’s disease. Research published in the Journal of Neuroscience found that this waste clearance system works most efficiently when subjects are in a lateral (side-lying) position compared to sleeping on the back or stomach. In the stomach-down position, the brain retained more waste and cleared it more slowly. While this research was conducted in rodents, it aligns with the observation that side sleeping is the most common natural sleep position across many species.
Putting It All Together
A quick checklist for your setup: a pillow that fills the gap between your ear and the mattress without tilting your head, a pillow between your knees to keep your hips neutral, and optionally a pillow hugged against your chest to support your top arm. Your chin faces forward, not tucked or craned. Your shoulders and hips form a straight vertical line. Your mattress lets your shoulders and hips sink in slightly without letting your midsection sag.
It takes a few nights to adjust if you’re changing your setup. Start with the knee pillow, since that produces the most noticeable difference in back and hip comfort. Add the chest pillow if you wake with shoulder stiffness. Swap your head pillow last, since pillow height changes feel the most disruptive at first but pay off within a week once your neck muscles adapt.
