How to Specialize in Nursing: From RN to Certification

Specializing in nursing starts with earning your RN license, then building clinical experience in a specific area before pursuing certification. The process typically takes two to five years beyond your initial nursing degree, depending on whether you’re aiming for a bedside specialty or an advanced practice role. The payoff is real: specialty-certified nurses earn 17% to 23% more on average than their non-certified peers, with some niches like dermatology and neurology commanding salaries nearly 50% higher.

Step 1: Get Your RN Foundation

Every nursing specialty begins with the same starting point: passing the NCLEX-RN and working as a registered nurse. You can enter the profession with an associate degree (ADN) or a bachelor’s degree (BSN), though a BSN opens more doors for specialization later. Many certification bodies, including the American Nurses Credentialing Center, require a bachelor’s or higher degree in nursing as an eligibility requirement for their specialty exams.

Your first year or two as a generalist RN matters more than most new nurses realize. Working on a medical-surgical floor, in an emergency department, or in another general setting builds the clinical judgment you’ll need no matter which specialty you choose. Think of this period as gathering a broad foundation before narrowing your focus.

Step 2: Choose a Specialty Path

Nursing specialties fall into two broad categories: bedside specialties you can enter with your RN and continuing education, and advanced practice roles that require a graduate degree. Your choice shapes how much additional schooling you’ll need and what your day-to-day work looks like.

Bedside and Unit-Based Specialties

These are roles where you specialize through on-the-job experience and certification rather than going back to school for another degree. Examples include critical care, oncology, emergency nursing, pediatrics, labor and delivery, and perioperative (surgical) nursing. You typically transition into these by requesting a transfer to a specialty unit, completing any unit-specific orientation or training, and then accumulating enough clinical hours to sit for a certification exam.

Critical care is one of the most popular specialty paths. To earn the CCRN credential, you need 1,750 hours of direct care with acutely or critically ill patients over two years (with at least 875 in the most recent year), or 2,000 hours over five years. Those hour requirements are fairly representative of what most bedside specialty certifications demand.

Non-Bedside Specialties

Not every specialty involves direct patient care. Nursing informatics, for example, focuses on healthcare data systems and technology. To earn the informatics nursing certification, you need a BSN or higher, two years of RN practice, 30 hours of continuing education in informatics, and at least 2,000 hours of informatics-specific work in the past three years. If you’ve completed graduate-level informatics coursework, the practice hour requirement drops to 1,000. Other non-bedside paths include case management, nurse education, legal nurse consulting, and quality improvement.

Niche Specialties

Some of the most distinctive nursing careers are in smaller, highly focused fields. Forensic nurses work with victims of violence and sexual assault, often earning the Sexual Assault Nurse Examiner credential with an ADN or BSN. Flight nurses provide critical care during air transport and pursue the Certified Flight Registered Nurse designation. Dermatology nurses handle everything from skin cancer screenings to pre- and post-operative care for cosmetic procedures. These niche roles often require you to seek out specialized training programs or clinical rotations on your own, since they’re rarely part of standard hospital orientation tracks.

Step 3: Accumulate the Right Clinical Hours

The single biggest barrier to specialty certification is logging enough hours in your chosen area. Most credentialing bodies want to see between 1,000 and 2,000 hours of specialty-specific practice over two to three years. This isn’t total nursing experience; it’s hours spent doing the specific type of work covered by the certification.

Be strategic about this. If you want to specialize in critical care, working a few shifts per month in the ICU while spending most of your time on a step-down unit won’t accumulate hours fast enough. Request a full-time position on the unit that aligns with your goal. Track your hours from the start, because you’ll need to verify them on your certification application.

Step 4: Pass a Certification Exam

Once you meet the eligibility requirements for your specialty, you’ll apply through the relevant credentialing organization and schedule a certification exam. The ANCC is the largest certifying body and covers dozens of specialties, but it’s not the only one. The American Association of Critical-Care Nurses handles the CCRN, the Oncology Nursing Certification Corporation covers oncology credentials, and the Board of Certification for Emergency Nursing manages emergency nursing credentials, among others.

These exams are substantial. They test clinical knowledge specific to your specialty, and pass rates vary. Preparation typically involves a review course (many are available online), practice exams, and several weeks of focused study. Once you pass, you’ll add the credential initials after your name, signaling to employers and patients that you’ve met a national standard of expertise.

Going Further: Advanced Practice Roles

If you want to practice with greater autonomy, prescribe medications, or diagnose conditions, you’ll need to become an advanced practice registered nurse (APRN). There are four APRN roles: nurse practitioner (NP), clinical nurse specialist (CNS), certified registered nurse anesthetist (CRNA), and certified nurse-midwife (CNM). All require at least a master’s degree in nursing from an accredited program, with each role having different clinical training hours and competency requirements built into the curriculum.

The educational landscape for APRNs is shifting. The Council on Accreditation of Nurse Anesthesia Educational Programs moved to require a doctoral degree (DNP) for entry into nurse anesthesia practice by 2025. The National Organization of Nurse Practitioner Faculties has similarly called for the DNP to become the entry-level degree for nurse practitioners, a position it reaffirmed in April 2023. While many master’s-prepared NPs continue to practice, new students entering these programs are increasingly enrolling in doctoral tracks. If you’re considering an APRN path, check whether your target program offers an MSN, a DNP, or both, and whether your intended specialty is trending toward doctoral-level entry.

Graduate programs typically take two to four years depending on whether you attend full-time or part-time, and whether you’re entering a master’s or doctoral program. Clinical rotations are built into the curriculum, so you’ll graduate with both the degree and the supervised practice hours needed for certification and state licensure in your APRN role.

Keeping Your Specialty Certification Active

Specialty certifications don’t last forever. Most require renewal every two to five years, and renewal involves documenting continuing education hours and, in some cases, ongoing clinical practice. State RN license renewal requirements vary widely. Alabama and Florida require 24 contact hours every two years, Texas and Illinois require 20 every two years, and Iowa requires 36 hours over a three-year license cycle. Specialty certification renewal requirements are separate from and in addition to your state license requirements, so plan on completing continuing education for both.

Many nurses fulfill these requirements through conferences, online courses, professional journal articles with post-tests, and employer-sponsored training. Some credentialing bodies also accept practice hours or professional development activities like publishing, presenting, or precepting students as part of the renewal portfolio.

What Specialization Means for Your Career

Beyond higher pay, specialization changes how you’re perceived in the job market. Certified nurses are often preferred or required for positions in specialty units, and the credential can make you a stronger candidate for leadership roles, educator positions, and consulting work. In a survey of specialty-certified nurses, about 30% of respondents held a certification, and their salaries were 17% higher on average than their non-certified colleagues. For some specialties, the gap is far wider: certified nurses in dermatology, neurology, and ear/nose/throat nursing earned 49% to 52% more than non-certified nurses in similar roles.

Specialization also gives you professional mobility. A CCRN credential earned in one state is recognized nationwide. If you relocate, your specialty certification travels with you even though you’ll need to obtain a new state RN license. For nurses who want to build a career with clear expertise and upward trajectory rather than rotating through general assignments indefinitely, specialization is the most direct route.
