How to Squat With Bad Knees Safely and Pain-Free

You can squat with bad knees, and in most cases you should. Strengthening the muscles around the knee is one of the most effective ways to reduce pain and improve function over time. The key is modifying your technique so the load shifts away from the knee joint and toward the hips and glutes. A few changes to your stance, depth, and body position can dramatically lower the stress on your kneecaps while still giving you the benefits of the movement.

Why Squatting Helps Rather Than Hurts

It sounds counterintuitive, but avoiding squats often makes knee problems worse. The quadriceps, hamstrings, and glutes act as shock absorbers for the knee joint. When those muscles weaken from disuse, the joint itself takes more of the load during everyday activities like climbing stairs or getting out of a chair. The American College of Sports Medicine recommends at least two strength training sessions per week for people with osteoarthritis, noting that consistency matters more than intensity for managing symptoms. Their guidance is clear: high-intensity and low-intensity training produce similar benefits, so you should work at whatever level feels manageable.

One important finding from the ACSM: pain reduction requires a minimum of about 12 sessions before it kicks in and becomes sustainable. That means the first few weeks may feel discouraging. Stick with it. The payoff comes from accumulated sessions over time, not from any single workout.

Shift the Load to Your Hips

The single most impactful change you can make is turning your squat into a hip-dominant movement. In biomechanical terms, leaning your trunk slightly forward shifts the work from your knees to your hips and glutes. When your torso leans forward more than your shins lean forward, the hip muscles do most of the heavy lifting and the demand on the knee joint drops. A difference of just 10 degrees between trunk angle and shin angle is enough to create a meaningful hip-dominant pattern.

Think of it as sitting back into a chair rather than dropping straight down. Push your hips behind you first, let your torso come forward naturally, and keep your weight in your heels. This is the same basic pattern as a box squat or a goblet squat with a sit-back emphasis. If you’re new to this, actually squatting to a box or bench gives you a target and a safety net.

Keep Your Shins Vertical

The more your knees travel forward over your toes, the more compressive force builds behind your kneecap. Research on patellofemoral pain consistently identifies forward knee translation (the knee drifting past the toes) as one of the two primary aggravating factors for kneecap pain. The other is muscle imbalance, particularly weakness in the inner quad and the side glute muscles.

Keeping your shins close to vertical naturally limits how far forward your knees travel. You don’t need to obsess over whether your knee crosses your toe by a centimeter, but as a general cue, think “shins stay stacked” and “hips go back.” This pairs perfectly with the hip-dominant approach above.

Control Your Depth

The highest stress on the kneecap joint occurs when your knee is bent between about 60 and 90 degrees, which roughly corresponds to a parallel or below-parallel squat. If deep squats bother your knees, there’s no rule that says you have to go that low. A half squat or a squat to a high bench keeps you above that peak-stress zone while still loading the muscles effectively.

Start with whatever depth is pain-free and gradually work deeper over weeks as your strength improves. Depth is something you earn back, not something you force on day one.

Stance Width and Foot Position

Stance width is defined relative to your shoulders: narrow is roughly shoulder width, medium is up to 1.5 times shoulder width, and wide is 1.5 to 2 times shoulder width. A wider stance increases the involvement of the hip muscles, which is generally what you want. However, it also increases sideways stress on the knee (valgus moments increase by about 23% compared to a narrow stance), which can be problematic if your knees tend to cave inward.

For most people with knee pain, a medium stance (just outside shoulder width) with toes turned out 15 to 30 degrees is the sweet spot. It gives your hips room to work without creating excessive lateral knee stress. The most important thing is that your knees track in the same direction as your toes throughout the movement. If your knees collapse inward at any point, narrow your stance or reduce the load until you build enough hip strength to maintain alignment. Inward knee collapse during single-leg movements is strongly linked to knee pain, particularly in women.

Warm Up Before You Load

The cartilage in your knee relies on movement to stay lubricated. Joint fluid is produced and distributed when you move the joint through its range of motion, which is why stiff knees loosen up after a few minutes of walking. The American Academy of Orthopaedic Surgeons recommends 5 to 10 minutes of low-impact activity before any knee exercises. Walking, riding a stationary bike, or doing bodyweight leg swings all work. The goal is to get blood flowing to the muscles and fluid moving through the joint before you add resistance.

Don’t skip this step. Loading a cold, stiff knee is one of the fastest ways to flare up pain that would have been avoidable.

Heel Elevation and Footwear

If you have limited ankle mobility (common with knee issues, since people tend to stiffen up the whole chain), your body compensates by letting the knees drift forward or rounding the lower back. Both are problematic. Elevating your heels solves this by giving your ankle the range it’s missing.

Weightlifting shoes with a raised heel reduce the amount of ankle bend needed and allow a more upright trunk position. Research comparing heeled shoes to flat shoes found that the raised heel significantly reduced ankle flexion demands. You don’t need specialty shoes to try this: placing small weight plates or a thin board under your heels during bodyweight squats achieves the same effect. If your squat immediately feels smoother and more controlled with elevated heels, limited ankle mobility was likely part of the problem.

Knee Sleeves for Pain and Stability

Neoprene knee sleeves provide warmth and compression that can meaningfully reduce pain during exercise. In a study of 60 patients with knee osteoarthritis, both basic elastic sleeves and reinforced sleeves produced immediate, statistically significant reductions in pain scores after being worn. The sleeves also improved balance, likely because the compression enhances your brain’s ability to sense the joint’s position in space (proprioception).

Programming: Start Light, Stay Consistent

Begin at roughly 50 to 60 percent of what you think you could handle for a maximum effort. Two to three sets of 8 to 12 repetitions is the standard recommendation. The weight should feel moderate, not challenging, for the first several sessions. Progression means adding small amounts of resistance every week or two, not chasing heavy loads.

The ACSM’s recent analysis found that the total number of sessions and overall time spent in a program matter more for pain reduction than sets, reps, or intensity. In practical terms, squatting twice a week with modest weight for three months will do more for your knees than going heavy twice and quitting because it hurt. Two sessions per week is the minimum effective dose. Three is fine if you recover well between sessions.

Squat Variations That Reduce Knee Stress

  • Box squat: Sit back to a box or bench, pause briefly, then stand. This enforces the hip-dominant pattern and limits depth to a consistent, pain-free range.
  • Goblet squat: Hold a weight at your chest. The front-loaded position naturally encourages an upright torso and controlled descent.
  • Wall sit: A static hold with your back against a wall. Lets you strengthen the quads without moving through a painful range of motion.
  • Split squat (short stance): Keeps the front shin relatively vertical while building single-leg strength. Use a shorter stride to limit forward knee travel.
  • Leg press: A machine alternative that lets you control foot placement precisely. Place your feet higher on the platform to shift stress toward the hips.

Signs You Should Stop

Some knee discomfort during the first few weeks of a new program is normal, especially if you’ve been inactive. Pain that increases as you continue the set, sharp or catching sensations inside the joint, swelling that lasts more than a few hours after training, or any feeling that the knee is giving way or locking are all signals to stop and get evaluated. Pain behind or around the kneecap that worsens specifically between 60 and 90 degrees of knee bend is a hallmark of patellofemoral stress and means you need to reduce depth, reduce load, or both before continuing.
