Tooth nerve pain can often be reduced at home with over-the-counter pain relievers, topical numbing gels, clove oil, or saltwater rinses. These methods buy you time, but they don’t fix the underlying problem. If the pain is severe, persistent, or accompanied by swelling, you need a dentist to address the source.
Understanding why your tooth hurts helps you pick the right short-term strategy and know when you’re dealing with something that won’t resolve on its own.
Why Tooth Nerve Pain Happens
Your teeth contain living tissue called pulp, filled with tiny nerve fibers and blood vessels. When bacteria from decay, a crack, or trauma reach that tissue, the body launches an inflammatory response. Inflammatory chemicals like bradykinin and substance P flood the area, dilating blood vessels and increasing pressure inside the rigid walls of the tooth. Unlike swelling in your ankle, which can expand outward, swelling inside a tooth has nowhere to go. That trapped pressure directly compresses nerve endings, producing intense, throbbing pain.
In irreversible pulpitis, the most severe form of pulp inflammation, levels of substance P are roughly eight times higher than in healthy pulp tissue. At that stage, the nerve is damaged beyond self-repair, and no home remedy will permanently fix it.
Temperature sensitivity works through a different mechanism. Cold drinks or hot food create rapid temperature changes at the tooth surface, causing microscopic fluid inside tiny tubes in your tooth (dentinal tubules) to shift. Cold pulls fluid outward, away from the pulp; heat pushes it inward. That fluid movement creates shear forces on nerve endings near the pulp border, triggering sharp, shooting pain. This is why a sip of ice water can send you through the roof even when nothing looks visibly wrong with the tooth.
Over-the-Counter Pain Relievers
The most effective approach for acute dental pain is combining ibuprofen with acetaminophen. They work through different pathways: ibuprofen reduces inflammation at the source, while acetaminophen blunts pain signaling in the brain. Clinical studies on post-dental-surgery pain show that even modest combination doses (200 mg ibuprofen with 500 mg acetaminophen) provide relief comparable to 400 mg of ibuprofen alone, with the benefit of attacking pain from two directions simultaneously.
A practical regimen: take 400 mg of ibuprofen and 500 mg of acetaminophen together, then alternate doses every few hours, staying within the daily limits on each package. Ibuprofen also helps reduce the inflammatory swelling inside the pulp that’s driving the pressure and pain. If you can only take one, ibuprofen is generally more effective for dental pain than acetaminophen alone because of that anti-inflammatory action.
Topical Numbing Gels
Over-the-counter benzocaine gels (sold under names like Orajel and Anbesol) come in 10% and 20% concentrations. Both provide meaningful relief. In clinical testing on patients with active toothaches, the median duration of pain relief exceeded 115 minutes for both strengths. You apply a small amount directly to the gum tissue around the painful tooth with a clean finger or cotton swab.
These gels numb the surface tissue but don’t penetrate deeply enough to fully silence an inflamed pulp. They’re most useful for taking the edge off while you wait for pain relievers to kick in, or for getting through the night until you can call a dentist.
Clove Oil and Saltwater Rinses
Clove oil contains eugenol, a compound that blocks sodium channels in nerve cells. Sodium channels are what allow nerves to fire pain signals, so blocking them produces a localized numbing effect similar to what a dentist achieves with an injection, just much milder. Dab a small amount of clove oil onto a cotton ball and hold it against the painful area for a few minutes. The taste is strong and slightly burning, but the numbing effect is real. You can find clove oil at most pharmacies and health food stores.
A warm saltwater rinse helps in a different way. Mix one teaspoon of salt into eight ounces of warm water, swish gently around the affected area for 30 seconds, and spit. The salt creates an osmotic effect that draws excess fluid out of swollen gum tissue, temporarily reducing pressure and discomfort. It also helps clear bacteria from around the tooth. You can repeat this several times a day. It won’t stop nerve pain inside the tooth, but it reduces irritation in the surrounding tissue and is especially helpful if the gums around the tooth are inflamed or if there’s a visible sore.
Avoiding Triggers
While you’re managing the pain, you can reduce flare-ups by controlling what touches the tooth. Cold and hot foods are the most common triggers because of the fluid-movement mechanism described above. Stick to lukewarm foods and drinks. Chew on the opposite side of your mouth. Sweet and acidic foods can also provoke pain by drawing fluid through exposed dentin through osmotic changes.
Sleeping with your head slightly elevated can help if the pain worsens at night. Lying flat increases blood flow to your head, which raises pressure in the already-swollen pulp tissue. An extra pillow makes a noticeable difference for many people.
When Home Remedies Won’t Be Enough
Home measures are temporary. They manage symptoms while the underlying cause remains. Certain patterns of pain signal that the nerve is too damaged for anything short of professional treatment:
- Spontaneous pain that starts without any trigger (no cold drink, no chewing) and lingers for minutes or longer typically indicates irreversible pulpitis.
- Pain that wakes you up at night suggests significant pulp inflammation.
- A dull, constant ache with pressure sensitivity when biting may mean infection has spread beyond the tooth root into the surrounding bone.
If you develop a fever along with facial swelling, or if swelling in your cheek, jaw, or neck makes it hard to breathe or swallow, go to an emergency room. These are signs that a dental infection has spread beyond the tooth into deeper tissues.
What a Dentist Does to Stop the Pain Permanently
The permanent fix for tooth nerve pain depends on how far the damage has progressed. For mild, reversible inflammation, removing the source of irritation (filling a cavity, for example) allows the pulp to heal and the pain to stop on its own.
For irreversible pulpitis, the two main options are root canal therapy and pulpotomy. A root canal removes all the pulp tissue from the tooth’s interior and root canals, then seals the space. It’s the traditional approach and works well when performed thoroughly, though success rates vary (studies report 24% to 66% in general practice settings), and the tooth becomes more brittle without its living core.
A pulpotomy is a more conservative option that removes only the inflamed portion of the pulp, leaving healthy tissue in the roots alive. Recent clinical trials comparing the two found that pulpotomy patients experienced larger pain reductions on the first day after treatment and had lower rates of postoperative pain overall. The rapid relief comes from releasing the trapped pressure inside the tooth and removing the tissue producing inflammatory chemicals. Root canal patients, by contrast, sometimes experience short-term pain from irritation of tissues beyond the root tip during the cleaning process.
Your dentist will recommend one approach over the other based on how much of the pulp is affected, whether infection has reached the root tips, and the structural condition of the tooth. Either way, the goal is the same: remove or calm the source of inflammation so the nerve stops firing pain signals permanently.
Sensitive Teeth vs. Nerve Damage
Not all tooth nerve pain means something is seriously wrong. Dentin hypersensitivity, the brief zing you feel with cold water or sweet food, happens when the protective enamel layer thins or gum recession exposes root surfaces. Fluid in the exposed dentinal tubules moves more freely, triggering nerve responses to everyday stimuli.
Desensitizing toothpastes containing potassium nitrate work by flooding the area with potassium ions, which block the nerve fibers in your tooth from generating pain signals. They take one to two weeks of consistent use to build up enough effect. If your pain is limited to brief, sharp reactions to temperature or sweets that vanish within a few seconds, a desensitizing toothpaste may be all you need. If the pain lingers for more than 30 seconds after the trigger is removed, or arrives without any trigger at all, the problem is deeper than surface sensitivity.
