How to Strengthen the Muscles Around Your Knee

Strengthening the muscles around your knee is one of the most effective ways to reduce knee pain, protect the joint from injury, and improve everyday movement. The knee itself is primarily stabilized by ligaments and cartilage, but every muscle surrounding it acts as a secondary stabilizer, controlling how the joint moves and absorbing forces before they reach vulnerable structures. A well-rounded routine targets four muscle groups: the quadriceps in front, the hamstrings in back, the calves below, and the hip muscles above.

Why These Muscles Matter for Your Knee

Your quadriceps (the four muscles on the front of your thigh) extend your knee and absorb impact when you walk, climb stairs, or land from a jump. Your hamstrings (three muscles on the back of your thigh) flex the knee and work with the quadriceps to keep the joint from shifting forward or backward. The gastrocnemius, the large calf muscle, crosses behind the knee and adds another layer of support during push-off movements like walking or running.

Less obvious but equally important are the hip muscles, particularly the gluteus medius on the outer hip. When this muscle is weak or fatigues quickly, your knee tends to collapse inward during movement, a pattern called dynamic knee valgus. That inward collapse is present in 70 to 80 percent of ACL injuries. Research suggests that hip abductor endurance matters even more than raw hip strength for preventing this collapse, which means training these muscles with higher repetitions pays off.

Quadriceps Exercises

The quadriceps are the workhorse of knee stability. Three effective exercises, recommended by the American Academy of Orthopaedic Surgeons, form a solid starting point. For all three, aim for 3 sets of 10 repetitions, 4 to 5 days per week. Start with a 5-pound ankle weight and gradually work up to 10 pounds as the exercises become comfortable.

Seated knee extension: Sit in a sturdy chair with your feet flat on the floor. Slowly lift one foot and straighten the knee until your leg is fully extended, keeping the back of your thigh on the seat. Hold for 5 seconds at the top, then lower slowly. The controlled lowering phase is just as important as the lift because it builds the eccentric strength that protects your knee during activities like walking downhill.

Straight-leg raise: Lie on your back with one knee bent and the other leg straight. Tighten the quadriceps of the straight leg and lift it to the height of the opposite knee. Hold briefly at the top and lower slowly. This exercise strengthens the quads without bending the knee joint, making it a good option if bending is painful.

Half squat: Stand with your feet shoulder-width apart, holding light dumbbells at your sides. Lower yourself about halfway down, as if sitting back into a chair, keeping your knees behind your toes. Push through your heels to stand back up. Half squats work the quadriceps, hamstrings, and glutes simultaneously.

The VMO Myth

You may have heard that targeting the inner part of the quadriceps, the vastus medialis oblique or “VMO,” is the key to fixing knee problems. A systematic review of 20 studies covering 387 subjects found strong evidence that the VMO cannot be preferentially activated over the rest of the quadriceps. Changing your foot position, adding a squeeze between your knees, or using specific angles does not meaningfully shift the work to the VMO alone. General quadriceps strengthening is equally effective for knee rehabilitation, so don’t overthink which part of the muscle you’re hitting.

Hamstring and Glute Exercises

The muscles on the back side of your leg and hip deserve equal attention. A common pattern in people with knee pain is overdeveloped quads paired with weak hamstrings and glutes, which creates an imbalance that pulls the knee forward and reduces its ability to decelerate during movement.

Standing hip hinge: Stand with your feet hip-width apart and a soft bend in your knees. Hinge forward from your waist, reaching your hips back as if closing a car door with your rear end. Shift your weight into your heels and lower until you feel a stretch through the hamstrings, then squeeze your glutes and hamstrings to pull yourself upright. Perform 2 to 3 sets of 12 to 15 repetitions. This movement directly trains the ability to bend at the waist and control how force passes through the knee.

Single-leg hip hinge: Once the two-legged version feels easy, progress to a single-leg version. Stand on one foot, hinge forward while extending the opposite leg behind you, and keep your hips level to the floor. Use the glute and hamstring of your standing leg to return to upright. This builds single-leg stability that translates to walking, running, and stair climbing.

Leg press with resistance band: Lie on your back and loop a resistance band around one foot. Press the foot away from you until your leg is nearly straight, then return slowly. This works both the quadriceps and hamstrings with minimal joint stress. Increase band resistance as you get stronger rather than adding ankle weights, which can strain the knee in this position.

Hip Abductor Exercises

Your gluteus medius sits on the outer hip and prevents your knee from caving inward every time you step, squat, or land. Because endurance matters more than peak strength for controlling knee valgus, focus on moderate resistance with higher repetitions rather than heavy loads.

Side-lying leg raise: Lie on your side with your legs stacked. Keeping your top leg straight and your toes pointing forward (not toward the ceiling), lift the leg about 12 inches, pause, and lower it slowly. Aim for 3 sets of 15 to 20 repetitions on each side.

Clamshell: Lie on your side with your knees bent at about 45 degrees and your feet together. Open your top knee like a clamshell while keeping your feet in contact. You should feel the burn deep in the outer hip, not in the front of the thigh. Add a looped resistance band just above your knees once bodyweight feels too easy.

Lateral band walk: Place a resistance band around both legs just above the ankles. Bend your knees slightly and take 10 to 15 steps to the right, then 10 to 15 to the left. Keep tension in the band throughout. This exercise trains the hip abductors in a functional, upright position that mimics real-world movement patterns.

Balance and Proprioception Training

Strength alone isn’t enough. Your knee relies on proprioception, the body’s ability to sense joint position, to make split-second adjustments during movement. Injuries, surgery, and even prolonged inactivity reduce this sense, which means the knee reacts more slowly to sudden changes in direction or uneven terrain.

Single-leg squats engage both the knee and ankle proprioceptors while working the leg and glute muscles. Start by standing on one leg near a wall or counter for support, then slowly lower a few inches and return to standing. As your balance improves, deepen the squat and remove the support. The crossover walk, where you step one foot over the other while moving sideways, is another effective option for improving knee proprioception, particularly during recovery from injury.

Even a simple single-leg stand for 30 seconds at a time, done while brushing your teeth or waiting for coffee, builds the neuromuscular control that keeps your knee stable during unpredictable movements.

Programming and Progression

The AAOS recommends performing knee strengthening exercises 4 to 5 days per week. Start at the lower end of resistance (bodyweight or 5-pound weights) and progress only when the current level feels comfortable for all 3 sets of 10. Rushing to heavier loads before the muscles and tendons adapt is a common mistake that creates new pain rather than resolving existing problems.

If you’re working around knee pain or arthritis, resistance bands are a useful alternative to free weights because they provide smooth, adjustable resistance without the jarring impact of metal weights. Many rehabilitation programs progress through color-coded bands (from lighter resistance like red to heavier resistance like blue) over several weeks. The goal is gradual overload: the exercises should feel challenging in the last few repetitions but not painful in the joint itself. Muscle soreness is expected, especially in the first two weeks. Sharp or swelling knee pain during an exercise is a signal to reduce the range of motion or switch to a lower-impact variation like the straight-leg raise.

A balanced session takes about 20 to 30 minutes and might include two quadriceps exercises, one hamstring or glute exercise, one hip abductor exercise, and a minute or two of single-leg balance work. That combination covers all the muscle groups that share responsibility for keeping your knee stable and pain-free.
