Alka-Seltzer Plus Cold and Flu comes as effervescent tablets that you dissolve in water before drinking. It’s not meant to be swallowed whole like a regular pill, and getting the preparation right matters for both effectiveness and comfort.
How to Prepare and Take It

Alcohol and Alka-Seltzer Plus
Avoid alcohol while using this product. There are two reasons, and both are serious. First, acetaminophen and alcohol are both processed by your liver. People who regularly have three or more drinks a day face a significantly higher risk of liver damage when taking acetaminophen, including rare cases of liver failure. Second, alcohol amplifies the drowsiness caused by the antihistamine, which can impair your judgment, coordination, and reaction time more than either one alone.
Who Should Not Take It
Several groups need to avoid this product entirely:
- Anyone already taking acetaminophen: Many cold medicines, pain relievers, and even prescription drugs contain acetaminophen. Doubling up is one of the most common causes of accidental overdose. If you’re not sure whether another medication you take contains it, check the label or ask a pharmacist.
- People on MAOIs: If you take certain medications for depression or Parkinson’s disease called monoamine oxidase inhibitors, you cannot use this product. The interaction can cause a dangerous spike in blood pressure. The restriction applies for two weeks after stopping an MAOI as well.
- Children under 12: The product is not formulated for pediatric use at any dose.
Conditions That Need Extra Caution
The decongestant in Alka-Seltzer Plus narrows blood vessels to clear your sinuses, but that same effect can raise blood pressure and strain the cardiovascular system. If you have high blood pressure, heart disease, thyroid disease, or diabetes, check with a doctor before using it. The same applies if you have liver disease, since your liver handles the acetaminophen.
The antihistamine can worsen certain conditions too. If you have glaucoma, an enlarged prostate that makes urination difficult, or a chronic breathing condition like emphysema or chronic bronchitis, this product may not be safe for you. The daytime tablets typically skip or reduce the antihistamine to help you stay alert, while the nighttime tablets include it at full strength to help with sleep.
