How to Take Tadalafil: Doses, Food, and What to Expect

Tadalafil comes in two distinct dosing approaches: an as-needed dose taken before sexual activity, or a smaller daily dose taken at the same time every day. Which one you’re prescribed determines everything about how and when you take it, so the first step is knowing which regimen you’re on.

As-Needed Dosing

The standard starting dose for as-needed use is 10 mg, taken at least 30 minutes before you anticipate sexual activity. You can take it with or without food. If 10 mg works well but causes side effects, your prescriber may lower it to 5 mg. If it’s not effective enough, the dose can go up to 20 mg.

The key rule: never take more than one dose in a 24-hour period. Because tadalafil has a half-life of 17.5 hours, it stays active in your body far longer than similar medications. Clinical studies showed that more than 70% of attempts at sexual activity were successful anywhere from 30 minutes to 36 hours after a single dose. That wide window is what sets tadalafil apart and why it doesn’t need to be taken right before sex the way some alternatives do.

Daily Dosing

If you take tadalafil daily, the starting dose is typically 2.5 mg once a day, which your prescriber may increase to 5 mg. You take it at the same time each day regardless of when (or whether) sexual activity happens. After a few days of consistent dosing, the drug reaches a steady level in your bloodstream, so there’s no need to plan around timing.

Daily dosing is also the approach used for treating urinary symptoms from an enlarged prostate, either on its own or alongside erectile dysfunction. The dose for that purpose is 5 mg once daily. If you miss a dose, take it when you remember, but skip it if it’s almost time for your next one. Don’t double up.

Food and Alcohol

Tadalafil can be taken with or without food, and unlike some similar medications, a heavy meal won’t meaningfully delay its effects. That flexibility is one reason many people prefer it.

Alcohol is a different story. While a drink or two is unlikely to cause problems, you should limit yourself to no more than four alcoholic drinks in a short period while taking tadalafil. Both alcohol and tadalafil lower blood pressure, and combining them can cause dizziness, lightheadedness, or fainting. You’ll notice this most when standing up quickly.

Medications That Don’t Mix With Tadalafil

The most dangerous interaction is with nitrate medications. If you take nitroglycerin, isosorbide mononitrate, or any other nitrate for chest pain or heart conditions, tadalafil is completely off the table. Combining the two causes severe, potentially life-threatening drops in blood pressure. This also applies to recreational “poppers” (amyl nitrate or amyl nitrite), which work through the same mechanism.

Alpha-blockers, often prescribed for high blood pressure or urinary symptoms, also interact with tadalafil. Both relax blood vessels, and together they can cause a significant drop in blood pressure. If you’re already on an alpha-blocker like tamsulosin, doxazosin, or terazosin, your prescriber will typically stabilize that medication first before starting tadalafil. For enlarged prostate treatment specifically, the FDA recommends stopping your alpha-blocker at least one day before switching to daily tadalafil.

What to Expect

With as-needed dosing, the medication begins working within about 30 minutes for most people, though some notice effects sooner. Peak concentration in the blood occurs roughly two hours after taking it. The 36-hour window doesn’t mean you’ll have an erection for that entire time. It means that during that period, when you become sexually aroused, the medication will help you achieve and maintain an erection.

Common side effects include headache, indigestion, back pain, and muscle aches. These tend to be mild and often improve after you’ve taken the medication a few times. Back pain and muscle aches are more common with tadalafil than with similar drugs and usually show up 12 to 24 hours after a dose.

Storage

Keep tadalafil at room temperature, between 59 and 77 degrees Fahrenheit, in a cool, dry place away from direct sunlight. Avoid storing it in a bathroom (too humid) or in your car, where temperatures can swing dramatically. A bedroom drawer or kitchen cabinet away from the stove works well.
