Fresh breast milk has a mild, slightly sweet smell. When it spoils, the smell shifts to something distinctly sour or rancid, similar to spoiled cow’s milk. But telling the difference between truly spoiled milk and milk that simply changed during storage isn’t always straightforward, because breast milk naturally separates, shifts in color, and can develop unusual odors even when it’s perfectly safe. Here’s how to sort out what’s normal from what needs to be tossed.
The Smell Test Is Your Best Tool
The most reliable way to check breast milk is to smell it. Spoiled breast milk has a strong, sour odor that’s hard to miss. It smells unmistakably “off,” the way any dairy product does when it’s gone bad. If you open a container and immediately pull back from the smell, that milk should be discarded.
However, stored breast milk often develops a soapy or metallic smell that can be alarming but is not the same thing as spoilage. One long-held theory attributes this to lipase, a naturally occurring enzyme in breast milk that continues breaking down fats even during freezing. As those fats break down, they release fatty acids, some of which (particularly one with soapy flavor characteristics) can make the milk smell and taste different. Exposure to air during pumping and storage also oxidizes fats in the milk, further altering its scent.
Interestingly, recent research has complicated the lipase explanation. A 2019 study collected frozen milk that babies had refused from 16 mothers at five different time points and found that none of the samples actually contained high levels of lipase. The exact cause of the smell change during storage is still being worked out, but the key point for parents is this: a soapy or metallic smell is not the same as a sour, rotten smell. Soapy or metallic milk is generally safe. Sour, foul-smelling milk is not.
How Spoiled Milk Looks Different From Normal Separation
Breast milk naturally separates when it sits. The fat rises to the top and forms a cream layer, while the thinner, watery portion sinks to the bottom. This is completely normal and can make the milk look lumpy or even slightly curdled, especially after refrigeration or freezing. It works the same way cream rises to the top of whole cow’s milk.
To check whether separation is normal or a sign of spoilage, gently swirl the container. Normal breast milk will recombine into a smooth, uniform liquid within a few seconds of gentle swirling. Spoiled breast milk won’t. If you swirl it and chunks remain floating or the texture stays lumpy and clumpy rather than blending back together, that’s a sign the milk has turned. A consistently chunky texture that refuses to mix is a red flag, even if the smell isn’t overwhelmingly sour yet.
Taste It If You’re Unsure
If the smell is borderline and you can’t tell whether it’s soapy-different or genuinely sour, a small taste will give you a clearer answer. Spoiled breast milk tastes distinctly sour. Milk that has undergone fat breakdown during storage may taste soapy, metallic, or slightly “off” compared to fresh milk, but it won’t taste rancid the way spoiled milk does. The difference is similar to the difference between butter that’s been in the fridge a while and butter that’s gone bad. You’ll know.
Your Baby’s Reaction Matters
Babies are surprisingly good at identifying milk they don’t want to drink. If your baby refuses a bottle of stored milk, pulls away, or becomes unusually fussy during a feeding, that’s worth paying attention to. Research has shown that the reason babies refuse stored milk isn’t necessarily bacterial contamination. One study found that bacteria levels were not higher in milk that babies rejected compared to milk they accepted. But the flavor changes from fat breakdown can be enough to make some babies turn it down.
If your baby consistently refuses thawed or stored milk but happily nurses or drinks freshly expressed milk, the issue is likely flavor change rather than true spoilage. That said, even flavor-altered milk that a baby refuses is effectively unusable, whether or not it’s technically safe.
Storage Time Limits to Track
Knowing how long milk has been stored is one of the simplest ways to gauge whether it might be spoiled. The CDC’s current guidelines for freshly expressed milk are:
- Room temperature (77°F or cooler): up to 4 hours
- Refrigerator: up to 4 days
- Freezer: about 6 months is ideal, up to 12 months is acceptable
These are outer limits, not guarantees. Milk stored at the back of a consistently cold refrigerator will last longer than milk stored in a door compartment where the temperature fluctuates every time you open it. Similarly, a deep freezer holds milk better than the freezer compartment of a standard fridge.
Labeling containers with the date and time of expression is the easiest habit to protect against accidentally using milk that’s been stored too long. If you’re unsure when milk was pumped, treat it with extra caution and rely heavily on the smell and swirl tests.
Thawed and Leftover Milk Spoils Faster
Once frozen milk has been thawed, the clock speeds up considerably. Thawed breast milk should be used promptly and never refrozen.
Leftover milk from a feeding, where the baby’s mouth has been on the bottle, introduces bacteria from saliva into the milk. The CDC recommends using any leftover milk within 2 hours of the baby finishing the feeding. After that window, discard it regardless of how it smells or looks. This is one case where the timer matters more than sensory checks, because bacterial growth from saliva can reach problematic levels before the milk smells noticeably off.
What Happens If a Baby Drinks Spoiled Milk
If you realize your baby drank milk that may have been spoiled, watch for signs of digestive upset over the next several hours. The most common reactions are similar to mild food poisoning: diarrhea, vomiting, stomach cramps, and increased irritability or fussiness. Most episodes are self-limiting, meaning they resolve on their own.
In rare cases, heavily contaminated milk can cause more serious bacterial infections with persistent vomiting, watery or bloody diarrhea, or fever. These symptoms warrant prompt medical attention. A single feeding of mildly off milk, though, is unlikely to cause anything beyond a brief bout of fussiness or a loose stool.
Preventing Spoilage Before It Happens
Clean hands and sanitized pump parts are the foundation. Bacteria introduced during pumping are the primary driver of true spoilage, so washing hands before expressing and cleaning all pump components that touch milk after every session makes a meaningful difference.
Cool milk quickly. Freshly pumped milk can sit at room temperature for up to 4 hours, but getting it into the refrigerator sooner slows bacterial growth. If you’re pumping at work or away from home, an insulated cooler bag with ice packs buys you time until you can refrigerate or freeze it.
Store milk in small portions (2 to 4 ounces) so you only thaw what you need. This reduces waste from leftovers and avoids the situation where a large batch gets thawed, partially used, and then sits at room temperature too long. Storing in smaller volumes also means the milk freezes and thaws faster, which limits the window for fat breakdown and flavor changes.
