How to Tell If You Have a Cavity Before It Hurts

Cavities don’t always hurt, especially in the early stages. Many people have one without knowing it. But your teeth do give off signals, starting with subtle visual changes and progressing to sensitivity and pain as the decay deepens. Here’s what to look for at each stage.

The Earliest Sign: White Spots

Before a cavity becomes a hole, it starts as a patch of weakened enamel. These appear as chalky, glossy white spots or lines on the tooth surface, usually along the gum line. They’re areas where minerals have been stripped away by acid-producing bacteria, and they represent the very first clinical sign of decay.

At this point, there’s no hole yet and typically no pain. The surface of the enamel feels rougher than normal, which means these white patches pick up stains easily. So a light brown spot on a tooth can also be an early warning sign, not just a coffee stain. If you catch decay at this stage, it can sometimes be reversed with fluoride and improved oral hygiene before it ever becomes a true cavity.

What a Cavity Looks Like as It Progresses

Most people picture cavities as dark brown or black, but they actually move through a range of colors. Early cavities can be white, light brown, or yellow. As bacteria break through the outer enamel and reach the softer layer underneath (called dentin), the spot may turn yellow or grey because that inner layer is showing through. Over time, the color deepens to dark brown and eventually black.

You might also notice a visible pit or hole in the tooth. Sometimes you can feel it with your tongue as a rough edge or a small gap in the tooth’s surface. Food getting stuck repeatedly in the same spot is another clue, since a developing cavity creates a pocket where food lodges easily.

Sensitivity to Temperature and Sweets

Sharp, brief pain when you eat or drink something hot, cold, or sugary is one of the most common signs of a cavity. This happens because the inner layer of your tooth contains tiny tubes that connect directly to the nerve at the center. When decay eats through the protective enamel, those tubes become exposed, letting temperature changes and sugar reach the nerve and trigger pain.

This type of sensitivity tends to be localized. If one specific tooth stings when you sip ice water or bite into something sweet, that’s more telling than a general sensitivity across several teeth (which is more likely related to enamel erosion or gum recession). The key detail: when a cavity is just beginning, you may not have any symptoms at all. Sensitivity usually starts once the decay has moved past the enamel into the dentin layer, which is softer and far less resistant to acid.

Pain When Biting Down

A sharp jolt of pain when you bite or chew can signal a cavity, but it can also mean a cracked tooth. With a cavity, the pain tends to accompany sensitivity to temperature or sweets as well. A cracked tooth, by contrast, often hurts only under biting pressure without the same temperature sensitivity. If you’re experiencing sharp pain specifically when chewing, pay attention to whether other triggers (cold drinks, sugary food) also affect that same tooth. Multiple triggers pointing to the same tooth strongly suggest decay.

Bad Breath or a Lingering Bad Taste

A cavity itself doesn’t smell. But the bacteria hiding inside it do. As decay creates a hole in your tooth, it becomes a sheltered spot where plaque accumulates and is nearly impossible to clean out with normal brushing. The bacteria in that buildup produce sulfur-like compounds, which can cause persistent bad breath or a sour, bitter taste in your mouth that doesn’t go away even after brushing.

Bad breath has many causes, and tongue bacteria are one of the most common. But if you notice a foul taste or smell that seems to come from one area of your mouth, and it persists despite good oral hygiene, trapped bacteria in a cavity could be the source.

How Pain Tracks With Depth of Decay

The relationship between pain and cavity severity follows a predictable pattern. In the earliest stage, when only the outer enamel is affected, you’ll likely feel nothing. Once decay reaches the dentin, sensitivity begins. This is when hot, cold, and sweet triggers start to register.

If the decay continues deeper and reaches the pulp (the innermost part of the tooth containing nerves and blood vessels), the pain changes character. The pulp becomes swollen and irritated, but because it’s enclosed inside a rigid tooth, there’s nowhere for the swelling to go. The nerve gets compressed, producing a persistent, throbbing ache that can come on without any trigger at all. At this stage, the pain is harder to ignore and often wakes people up at night.

Signs Decay Has Become an Infection

Left untreated long enough, a deep cavity can lead to an abscess, which is a pocket of infection at the root of the tooth. The symptoms are distinct and harder to miss:

  • Severe, constant throbbing pain that can radiate into your jaw, neck, or ear
  • Facial swelling in your cheek, jaw, or neck
  • Fever
  • Swollen, tender lymph nodes under your jaw or along your neck

An abscess is a medical concern that goes beyond a dental appointment. If you develop a fever along with facial swelling, or if swelling makes it difficult to breathe or swallow, that indicates the infection may be spreading into deeper tissues of the jaw, throat, or neck. This warrants an emergency room visit.

Why You Can’t Always Self-Diagnose

The tricky thing about cavities is that many of them form between teeth or in areas you simply can’t see or feel. Dentists use bitewing X-rays to catch decay hidden between teeth or below the gum line, areas that look perfectly fine from the outside. Some offices also use transillumination devices that shine light through the tooth to reveal internal decay without radiation.

Because early cavities produce no pain and may not be visible to you, regular dental exams catch problems that self-checks miss. The symptoms described above are real and worth paying attention to, but the absence of symptoms doesn’t guarantee your teeth are cavity-free. Many people learn they have a cavity only when a dentist finds it on an X-ray during a routine visit.
