How to Tell If You Have High Blood Pressure

Most people with high blood pressure feel completely normal. It rarely causes noticeable symptoms, which is why it’s often called the “silent killer.” The only reliable way to know if you have it is to measure it. The good news: you can do that at home with an inexpensive monitor, and knowing your numbers puts you in a much stronger position to protect your health.

Why You Can’t Feel High Blood Pressure

High blood pressure damages blood vessels, the heart, kidneys, and brain gradually over years. But this internal damage doesn’t produce symptoms until it becomes severe. There’s no headache pattern, no facial flushing, and no reliable physical sensation that signals your blood pressure is creeping up. The vast majority of people with hypertension have no idea anything is wrong until a reading catches it or a complication develops.

This is what makes it dangerous. You can walk around for a decade with elevated pressure, feeling fine, while the extra force on your artery walls quietly accelerates damage. Waiting for symptoms is not a strategy that works here.

What the Numbers Mean

Blood pressure is recorded as two numbers: systolic (the pressure when your heart beats) over diastolic (the pressure between beats). Both are measured in millimeters of mercury, written as something like 120/80. The American Heart Association breaks readings into four categories:

  • Normal: below 120/80
  • Elevated: 120 to 129 systolic, with diastolic still under 80
  • Stage 1 hypertension: 130 to 139 systolic, or 80 to 89 diastolic
  • Stage 2 hypertension: 140 or higher systolic, or 90 or higher diastolic

Only one of those two numbers needs to be in the higher range for the reading to count. So a reading of 142/76 qualifies as Stage 2 hypertension even though the bottom number looks fine. A single high reading doesn’t mean you have hypertension, though. The 2025 guidelines from the American Heart Association and American College of Cardiology recommend basing a diagnosis on the average of at least two careful readings taken on at least two separate occasions. Blood pressure fluctuates throughout the day, so multiple measurements over time give a much more accurate picture.

How to Check at Home

A home blood pressure monitor is the simplest way to find out where you stand. Upper-arm cuff monitors are more reliable than wrist models. Look for a device listed on ValidateBP.org, a registry maintained with input from the American Medical Association that independently verifies clinical accuracy. Validated monitors from major brands typically cost $30 to $70.

The technique matters as much as the device. The CDC recommends this process:

  • Don’t eat, drink, or exercise for 30 minutes before measuring.
  • Empty your bladder first.
  • Sit in a chair with your back supported and both feet flat on the floor, legs uncrossed, for at least five minutes before you start.
  • Place the cuff on bare skin (not over a sleeve) and rest that arm on a table so the cuff sits at chest height.
  • Stay still and don’t talk during the reading.
  • Take at least two readings, one to two minutes apart, and use the average.

Small details can throw off your numbers. A full bladder can add 10 to 15 points to your systolic reading. Crossed legs, an unsupported back, or an arm hanging at your side can inflate results too. If you skip the five-minute rest period and measure right after climbing stairs, you’ll get a number that doesn’t reflect your resting blood pressure.

When Office and Home Readings Don’t Match

Some people consistently get high readings at the doctor’s office but normal readings at home. This is called white-coat hypertension, and it happens because the stress of a medical visit temporarily raises blood pressure. The reverse also exists: masked hypertension, where your numbers look fine in the clinic but run high the rest of the time. Masked hypertension is more dangerous because it often goes undetected and untreated.

If your office and home readings tell different stories, your doctor may recommend ambulatory monitoring. You wear a small cuff for 24 hours while it takes automatic readings every 15 to 30 minutes, including while you sleep. This provides the most complete picture of your blood pressure patterns and catches both white-coat and masked hypertension that standard office visits miss.

What Raises Your Risk

About 90 to 95% of high blood pressure cases are “primary” hypertension, meaning there’s no single identifiable cause. Instead, it develops from a combination of genetics, aging, diet, physical activity level, and body weight. Carrying extra weight, eating a high-sodium diet, drinking heavily, and being sedentary all push blood pressure upward over time. A family history of hypertension significantly increases your odds.

The remaining cases are “secondary” hypertension, where another medical condition is driving the numbers up. Kidney disease is one of the most common culprits. Hormone-producing conditions affecting the adrenal glands account for 10 to 20% of secondary cases. Sleep apnea is another frequent contributor. Even certain over-the-counter pain relievers, particularly anti-inflammatory drugs like ibuprofen, can raise blood pressure by causing your body to retain sodium. If your blood pressure is unusually high, hard to control, or developed suddenly, your doctor may investigate these underlying causes.

Warning Signs of a Blood Pressure Emergency

Day-to-day hypertension is silent, but a sudden spike to extreme levels can produce symptoms. A hypertensive crisis occurs when blood pressure reaches 180/120 or higher. At that level, you may experience chest pain, shortness of breath, severe headache, blurred vision, confusion, nausea, or anxiety. Stroke symptoms can also appear: sudden numbness or weakness on one side of the body, difficulty speaking, trouble walking, or sudden vision changes.

This is a medical emergency. If you get a reading of 180/120 or above and have any of these symptoms, call 911. If you get that reading but feel fine, wait five minutes, re-measure, and contact your doctor promptly if it stays that high.

Building a Useful Record

A single blood pressure reading is a snapshot. What matters is the pattern. If you’re checking at home, measure at the same times each day, ideally morning and evening, for at least a week before drawing conclusions. Write down or log every reading with the date and time. This record is far more useful to a doctor than a one-off number from a pharmacy kiosk.

Consistently seeing numbers at or above 130/80 across multiple days is a strong signal that your blood pressure is elevated. Readings that are sometimes high and sometimes normal still deserve attention, especially if the high readings cluster around stressful moments, poor sleep, or high-sodium meals. The pattern tells you what your cardiovascular system is dealing with on a regular basis, and that’s exactly what determines your long-term risk.
