How to Tell If You Have Pancreatitis: Symptoms

The hallmark sign of pancreatitis is sudden, severe pain in the upper abdomen that often radiates to the back. This pain typically doesn’t respond to changing positions, and it can be intense enough that you can’t sit still or get comfortable. If that description matches what you’re feeling, especially alongside nausea or vomiting, you need medical evaluation quickly.

Pancreatitis is inflammation of the pancreas, the organ behind your stomach that produces digestive enzymes and insulin. It ranges from a brief, painful episode that resolves in days to a chronic condition that permanently changes how your body digests food. Here’s how to recognize it, what doctors look for to confirm it, and what separates a mild case from a dangerous one.

Where the Pain Shows Up and How It Feels

The pain of pancreatitis is centered in the upper abdomen, typically right below the ribcage in the middle or slightly to the left. What distinguishes it from common stomach pain is its intensity and the way it travels. It often spreads to the back, and sometimes to the chest or sides. Many people describe it as a deep, boring pain rather than a sharp, stabbing one. It tends to feel worse after eating, particularly fatty meals.

The pain can be constant or come in waves with periods of relief in between. One characteristic pattern: leaning forward or curling into a fetal position sometimes takes the edge off, while lying flat on your back makes it worse. This postural quality is a useful clue, though not everyone experiences it. If the pain is so severe you can’t find any comfortable position at all, that’s a signal to get to an emergency room.

Other Symptoms Beyond the Pain

Pancreatitis rarely shows up as pain alone. Most people also experience nausea and vomiting, sometimes severe enough that they can’t keep food or water down. Your abdomen may feel tender and swollen, and you might develop a fever. A rapid heartbeat is common during an acute episode as your body responds to the inflammation.

When the pancreas isn’t working properly, it can’t produce enough enzymes to break down fat in your diet. This leads to pale, greasy, foul-smelling stools that may float in the toilet. This is more typical of chronic pancreatitis than a first acute episode, but it’s a telling sign that your pancreas isn’t keeping up with digestion. Unexplained weight loss often follows, since your body isn’t absorbing the calories and nutrients from the food you eat.

In rare but serious cases (fewer than 1% of acute episodes), internal bleeding from the inflamed pancreas causes visible bruising on the skin. This can appear as a bluish-red discoloration around the belly button or along the flanks. These are late signs of severe hemorrhagic pancreatitis and indicate a dangerous situation requiring intensive medical care.

Who Is Most at Risk

Two causes account for the vast majority of pancreatitis cases: gallstones and heavy alcohol use. Gallstones can block the duct where the pancreas empties its digestive enzymes, causing a backup that triggers inflammation. Alcohol damages pancreatic tissue directly, and the risk rises with the amount and duration of drinking. If you have gallstones or drink heavily and develop the pain pattern described above, pancreatitis should be high on the list of possibilities.

Other risk factors include high triglyceride levels (a type of blood fat), certain medications, abdominal injury, and a family history of pancreatic problems. Smoking increases risk independently and makes alcohol-related pancreatitis more likely. Sometimes no clear cause is found, which doctors call idiopathic pancreatitis.

How Doctors Confirm the Diagnosis

You can’t diagnose pancreatitis at home. What you can do is recognize the symptom pattern and get to a doctor, who will use a combination of tools to confirm or rule it out. The international diagnostic standard requires at least two of three criteria: characteristic abdominal pain, elevated pancreatic enzymes in your blood, and imaging findings consistent with pancreatitis.

Blood Tests

The first step is usually a simple blood draw. Your doctor will check levels of lipase and amylase, two digestive enzymes produced by the pancreas. When the pancreas is inflamed, these enzymes leak into the bloodstream at abnormally high levels. A lipase or amylase level at least three times higher than the top of the normal range strongly suggests acute pancreatitis. Lipase is considered the more reliable of the two because it stays elevated longer and is more specific to pancreatic problems.

Blood tests can often confirm a diagnosis within hours. They’ll also check for signs of complications, like elevated white blood cell counts (indicating infection) or changes in kidney and liver function.

Imaging

If blood tests alone aren’t conclusive, or if doctors need to assess how severe the inflammation is, imaging comes next. An abdominal ultrasound is often the first choice because it’s fast, widely available, and good at spotting gallstones as a potential cause. On ultrasound, an inflamed pancreas appears enlarged and darker than normal.

A CT scan with contrast dye gives a more detailed picture. It can reveal swelling of the pancreas, inflammation in the surrounding fat tissue, fluid collections building up around the organ, and areas of tissue death (necrosis) where the pancreas has stopped receiving blood flow. CT is particularly valuable for distinguishing mild pancreatitis from severe cases that may need aggressive treatment. MRI serves a similar role and is sometimes preferred when CT isn’t an option.

Acute vs. Chronic Pancreatitis

Acute pancreatitis comes on suddenly, usually over hours, and the pain peaks quickly. Most mild cases resolve within a week with supportive care: IV fluids, pain management, and a temporary break from eating to let the pancreas rest. You’ll typically be in the hospital for several days. Once the inflammation subsides, the pancreas often recovers fully.

Chronic pancreatitis is a different condition. It develops gradually after repeated episodes of inflammation scar the pancreas permanently. The pain may become constant rather than episodic, though paradoxically, some people with advanced chronic pancreatitis experience less pain as the gland burns out. The bigger problems are functional: the pancreas loses its ability to produce digestive enzymes and eventually insulin. This means chronic pancreatitis can lead to malnutrition and diabetes over time. Greasy stools, weight loss despite eating normal amounts, and new-onset diabetes in someone with a history of pancreatic problems are red flags for chronic disease.

Signs That Need Immediate Attention

Not all pancreatitis is equally dangerous, but certain signs point to a severe episode. Get emergency care if you experience upper abdominal pain so intense you cannot find a comfortable position, a fever above 101°F (38.3°C) alongside abdominal pain, a rapid heartbeat with dizziness or lightheadedness, or persistent vomiting that prevents you from keeping any fluids down.

Severe pancreatitis can lead to organ failure when inflammation triggers a body-wide immune response. This can affect the lungs, kidneys, and heart within the first 48 hours. About 20% of pancreatitis cases are classified as severe, and early hospital care makes a significant difference in outcomes. The bruising patterns around the belly button or flanks mentioned earlier, while rare, are a particularly urgent warning sign.

If your symptoms are milder but persistent, a pain that lingers in your upper abdomen for more than a day or two, especially with nausea or changes in your stool, still warrants a visit to your doctor. A lipase blood test can give a clear answer relatively quickly.
