How to Tell If Your Blood Pressure Is Too High

The honest answer is that you probably can’t tell from how you feel. High blood pressure almost never causes noticeable symptoms, which is why nearly half of American adults who have it don’t know it. The only reliable way to find out is to measure it. A normal reading is below 120/80 mm Hg, and anything at or above 130/80 is considered high blood pressure.

Why You Can’t Feel It

High blood pressure is often called the “silent killer” because it damages your heart, blood vessels, and kidneys gradually over years without producing obvious warning signs. There’s no headache, dizziness, or flushing that reliably tells you your numbers are up. Some people assume they’d feel stressed or flushed if their blood pressure were high, but that’s not how it works. You can feel perfectly fine with a reading of 150/95.

The exception is a hypertensive crisis, when blood pressure spikes to 180/120 or higher. At that level, you may experience chest pain, blurred vision, severe headache, confusion, nausea, shortness of breath, or stroke symptoms like numbness on one side of the body or trouble speaking. That’s a 911 situation. But anything short of a crisis typically produces no symptoms at all.

What the Numbers Mean

Blood pressure is recorded as two numbers. The top number (systolic) measures the pressure when your heart beats. The bottom number (diastolic) measures the pressure between beats. Both matter, and if your two numbers fall into different categories, the higher category is the one that applies to you.

  • Normal: below 120/80
  • Elevated: 120 to 129 systolic and below 80 diastolic
  • Stage 1 hypertension: 130 to 139 systolic or 80 to 89 diastolic
  • Stage 2 hypertension: 140 or higher systolic, or 90 or higher diastolic

A single high reading doesn’t mean you have hypertension. A formal diagnosis is typically based on the average of two or more readings taken on separate occasions. Blood pressure fluctuates throughout the day based on stress, activity, caffeine, and even how full your bladder is. That’s why patterns over time matter more than any single number.

How to Check at Home

A home blood pressure monitor is the most practical way to track your numbers between doctor visits. The American Heart Association recommends an automatic, cuff-style monitor that wraps around your upper arm. Wrist and finger monitors give less reliable readings. When you’re shopping for one, make sure it’s been validated for accuracy, and measure around your upper arm to pick the right cuff size. A cuff that’s too small or too large will give you inaccurate results.

Getting an accurate reading depends heavily on technique. The CDC recommends this routine:

  • Don’t eat, drink, or smoke for 30 minutes beforehand.
  • Empty your bladder first.
  • Sit in a chair with your back supported for at least 5 minutes before measuring.
  • Keep both feet flat on the floor, legs uncrossed.
  • Rest your arm on a table so the cuff is at chest height.
  • Place the cuff on bare skin, not over clothing.
  • Don’t talk during the reading.

Small details make a surprising difference. Crossing your legs or letting your arm hang at your side instead of resting it on a table can push your reading higher than it actually is. If you’ve ever gotten a worryingly high number and then a normal one five minutes later, positioning is often the reason.

White Coat and Masked Hypertension

Some people consistently show high readings at the doctor’s office but normal readings at home. This is called white coat hypertension, and it affects roughly 20 to 25% of people diagnosed with high blood pressure in a clinical setting. The anxiety of a medical visit is enough to temporarily raise your numbers.

The reverse is more dangerous. Masked hypertension means your readings look normal at the doctor’s office but are actually elevated during daily life. Studies across multiple continents have found this affects around 10 to 20% of people with normal office readings. These individuals may go untreated because their condition never shows up in a clinical visit. Home monitoring catches what office visits miss, which is one of the strongest reasons to own a monitor and check regularly.

Factors That Raise Your Risk

Certain things make high blood pressure more likely, and knowing your risk profile helps you decide how closely to monitor. Family history is one of the strongest predictors. If your parents or close relatives have hypertension, your chances go up significantly. Age plays a role too: blood vessels gradually lose elasticity over time, which increases pressure. Men are more likely to develop high blood pressure before age 64, while women’s risk rises after 65.

Lifestyle factors carry significant weight. A diet high in sodium, physical inactivity, smoking, and heavy alcohol use all contribute. Chronic stress doesn’t just raise blood pressure directly; it also drives the behaviors that raise it, like eating poorly, skipping exercise, and drinking more. Over half of people with high cholesterol also have high blood pressure, and most people with diabetes eventually develop it as well. Chronic kidney disease both causes and results from sustained high pressure, creating a cycle that’s hard to break without treatment.

Black, Hispanic, and Asian adults, particularly men, have higher rates of hypertension. These disparities reflect a combination of genetic predisposition, systemic health inequities, dietary patterns, and differences in access to care.

How Often to Check

If your blood pressure is normal and you have no major risk factors, getting it checked at your annual physical is generally sufficient. If your readings are in the elevated range (120 to 129 systolic), more frequent monitoring at home helps you catch a trend before it becomes a diagnosis. For anyone already diagnosed with hypertension or taking medication for it, regular home monitoring gives you and your provider a much clearer picture than occasional office visits alone.

When you do check at home, take two or three readings one minute apart and record all of them. Morning readings before medication or food tend to be the most consistent baseline. Keep a log with dates and times so your provider can see the full pattern rather than a single snapshot.
