How to Tell If Your Liver Is Bad or Failing

A struggling liver often gives no obvious warning signs in its earliest stages. Many people with liver damage feel perfectly fine until the disease has progressed significantly, which is why knowing the subtle and not-so-subtle signals matters. The signs range from persistent fatigue and vague abdominal discomfort to more alarming changes like yellowing skin, swelling, and mental fog.

Early Signs Are Easy to Miss

The liver sits just under your rib cage on the right side of your abdomen, and when it’s inflamed or struggling, the first symptoms tend to be nonspecific. Constant tiredness that doesn’t improve with rest is one of the most common early complaints. You may lose your appetite without explanation or feel a dull ache or fullness in the upper right part of your belly. Mild nausea can come and go.

These symptoms overlap with dozens of other conditions, which is exactly why early liver disease gets overlooked. Most people chalk it up to stress, poor sleep, or a bad diet. If the tiredness and discomfort persist for weeks, especially if you have risk factors like heavy alcohol use, obesity, or a family history of liver disease, blood work can catch what your symptoms alone can’t.

Changes You Can See

As liver function declines, visual changes start appearing on your skin, in the mirror, and in the bathroom.

Jaundice is the most recognizable sign. When the liver can’t properly process bilirubin, a waste product from broken-down red blood cells, the pigment builds up in your bloodstream and deposits in your skin and the whites of your eyes, turning them yellow. Even a faint yellowish tint is worth paying attention to.

Dark urine and pale stools often appear alongside jaundice. Excess bilirubin gets excreted by the kidneys, making urine noticeably darker, sometimes tea or cola-colored. Meanwhile, stools can turn clay-colored or pale because bilirubin is no longer reaching the intestine to give stool its normal brown color. Stools may also become greasy, foul-smelling, and difficult to flush, because bile isn’t entering the gut to help digest fat.

Spider-shaped blood vessels can appear on your skin, particularly on your upper body and face. These small red marks, called spider angiomas, look like a central dot with thin lines radiating outward. Having one or two is normal. If you count more than three, it can signal an underlying liver condition like cirrhosis or alcoholic hepatitis and is worth bringing up with a doctor.

Swelling in the Belly and Legs

Fluid retention is one of the hallmarks of advancing liver disease. It shows up in two main places.

Ascites is fluid buildup inside the abdominal cavity, between the layers of tissue that line your organs. It causes your belly to swell noticeably, sometimes dramatically. Rapid weight gain of two to three pounds per day over several days is a classic pattern. Your clothes may stop fitting around the waist before you realize what’s happening. Cirrhosis is the most common cause of ascites, though heart failure, kidney failure, and certain cancers can also produce it.

Edema, or swelling in the lower legs, ankles, and feet, frequently accompanies ascites. If you press a finger into the swollen area and a dent remains for several seconds, that’s a sign of fluid retention rather than simple weight gain. The combination of a swollen abdomen and puffy ankles together is a strong indicator that the liver (or another major organ) isn’t functioning properly.

Mental and Neurological Changes

A failing liver can affect your brain. When the liver can no longer filter toxins from the blood effectively, those toxins, particularly ammonia, accumulate and reach the brain. This can cause confusion, difficulty thinking clearly, memory problems, personality changes, and disrupted sleep patterns. Some people become unusually irritable or struggle to concentrate on tasks that used to feel automatic.

These cognitive symptoms can develop gradually, making them easy to attribute to aging or stress. Family members sometimes notice changes before the person does. In more severe cases, disorientation and excessive sleepiness can develop quickly and require urgent medical attention.

How Liver Disease Progresses

Liver damage doesn’t happen all at once. It follows a general trajectory, and knowing where things stand makes a significant difference in what can be done about it.

The earliest stage is fat accumulation in the liver, sometimes called fatty liver. At this point, most people have no symptoms at all, and the condition is often reversible with lifestyle changes. If inflammation develops, the liver begins to scar. Scarring is graded on a scale from F0 (no scarring) through F1, F2, and F3 (mild to advanced scarring), up to F4, which is cirrhosis. Fibrosis staging is the single most important factor in predicting outcomes and survival.

The key distinction: early-stage scarring (F1 and F2) can often be slowed or even partially reversed. Once cirrhosis develops, the damage is largely permanent, and management shifts to preventing complications and preserving whatever liver function remains.

Tests That Detect Liver Problems

Blood Tests

A standard liver function panel measures several markers. Two enzymes, ALT and AST, indicate liver cell damage when elevated. Normal ALT runs between 7 and 55 units per liter, while normal AST falls between 8 and 48. Albumin, a protein made by the liver, normally ranges from 3.5 to 5.0 grams per deciliter; low levels suggest the liver isn’t producing proteins efficiently. Bilirubin, the pigment behind jaundice, should fall between 0.1 and 1.2 milligrams per deciliter. These ranges can vary slightly between labs and may differ for women and children.

Elevated liver enzymes don’t automatically mean serious disease. Temporary spikes can result from medications, intense exercise, or a recent illness. Persistently abnormal results, especially combined with symptoms, warrant further investigation.

Imaging

A standard ultrasound can reveal fatty liver, an enlarged liver, or obvious structural problems, but it’s limited in detecting scarring. For that, elastography (sometimes marketed under the brand name FibroScan) measures liver stiffness, which correlates directly with the degree of scarring. Both ultrasound elastography and MRI elastography can be as accurate as a traditional biopsy for diagnosing fibrosis, with the advantage of being painless and noninvasive. These techniques let doctors see real-time images of the liver and target the probe to exactly where damage is suspected.

A liver biopsy, where a small tissue sample is extracted with a needle, remains an option when imaging results are inconclusive, but many patients can now get a clear diagnosis without one.

Common Causes of Liver Damage

Heavy or prolonged alcohol use is the cause most people think of first, and it remains a leading driver of cirrhosis. But the most common liver condition overall is now metabolic-associated fatty liver disease, linked to obesity, type 2 diabetes, and metabolic syndrome. An estimated one in four adults worldwide has some degree of fatty liver.

Viral hepatitis (particularly hepatitis B and C) causes chronic inflammation that can silently progress to cirrhosis over years or decades. Autoimmune conditions, certain medications (including long-term acetaminophen overuse), and genetic disorders like hemochromatosis, where the body stores too much iron, round out the major causes. Some people have overlapping risk factors, which accelerates damage.

What Reversible Looks Like

If you catch liver problems early, the outlook is often surprisingly good. Fatty liver without significant scarring can improve substantially with weight loss of even 5 to 10 percent of body weight. Reducing or eliminating alcohol allows the liver to regenerate in many cases. Treating an underlying hepatitis infection halts ongoing damage.

Once significant scarring is present, the liver’s ability to bounce back diminishes with each stage. That’s why the vague early symptoms, the persistent fatigue, the unexplained appetite loss, the subtle discomfort under your right ribs, matter more than they seem. A simple blood test can distinguish between “probably nothing” and “worth watching closely.”
