How to Tell If You’re Insulin Resistant: Signs & Tests

Insulin resistance rarely announces itself with a single obvious symptom. Instead, it shows up as a collection of subtle signals: changes in your bloodwork, shifts in how you feel after meals, and sometimes visible changes on your skin. A fasting blood glucose between 100 and 125 mg/dL or an A1C between 5.7% and 6.4% already places you in the prediabetes range, which is strongly associated with insulin resistance. But those numbers only tell part of the story.

What Insulin Resistance Actually Means

When you eat carbohydrates, your body breaks them into glucose and releases insulin to shuttle that glucose into cells, primarily skeletal muscle. Insulin works by triggering glucose transporters to move from inside the cell to its surface, where they act like doors letting glucose in. In insulin resistance, those transporters don’t respond as well. Your cells essentially ignore insulin’s signal, so glucose stays in the bloodstream longer than it should.

Your pancreas compensates by pumping out more insulin. For years, this extra insulin keeps blood sugar in a normal range, which is why standard glucose tests can look perfectly fine even when resistance is already well established. The problem isn’t just high blood sugar. It’s the chronically elevated insulin your body needs to maintain that normal sugar level.

Physical Signs You Can See

One of the most recognizable markers is a skin condition called acanthosis nigricans: dark, thick, velvety patches that typically appear on the back of the neck, in the armpits, or in the groin. The affected skin can feel slightly rough or textured and sometimes develops an odor or small skin tags. Most people who have these patches also have insulin resistance. If you’ve noticed darkening skin in those areas and it isn’t from sun exposure or friction, it’s worth investigating further.

Skin tags on their own, particularly clusters around the neck or underarms, are also linked to higher circulating insulin levels. They’re not diagnostic by themselves, but combined with other signs, they’re a useful clue.

How It Feels Day to Day

The symptoms of insulin resistance are easy to dismiss because they overlap with everyday tiredness and stress. But a few patterns stand out.

Crashing after meals, especially carb-heavy ones, is a hallmark. When your cells are resistant to insulin, your pancreas overproduces it after you eat. That flood of insulin can drive your blood sugar too low in the hours that follow, a process called reactive hypoglycemia. The result is intense fatigue, brain fog, shakiness, or sudden sugar cravings roughly two to three hours after a meal. Your body then triggers a stress hormone response (including cortisol) to bring blood sugar back up, which over time promotes fat storage around the midsection and deepens the insulin resistance itself. It becomes a self-reinforcing cycle: carbs trigger an insulin spike, blood sugar crashes, you crave more carbs, and the pattern repeats.

Other common day-to-day signs include persistent hunger even shortly after eating, difficulty losing weight despite consistent effort (particularly around the abdomen), and feeling mentally sluggish in the afternoon. None of these prove insulin resistance on their own, but if several show up together, they paint a recognizable picture.

Bloodwork That Reveals the Pattern

Standard blood tests can catch insulin resistance if you know which numbers to look at, and some of the most telling markers aren’t the ones your doctor highlights first.

Fasting Glucose and A1C

The American Diabetes Association defines prediabetes as a fasting blood glucose of 100 to 125 mg/dL or an A1C of 5.7% to 6.4%. These are the thresholds most doctors use. But here’s the catch: your fasting glucose can sit in the 80s or low 90s for years while insulin resistance quietly builds, because your pancreas is working overtime to keep it there. A “normal” fasting glucose doesn’t rule out resistance.

Triglyceride-to-HDL Ratio

Your standard lipid panel contains a surprisingly useful insulin resistance marker hiding in plain sight. Divide your triglycerides by your HDL cholesterol (both in mg/dL). For White European men, a ratio above 3.8 signals likely insulin resistance. For women, the threshold is lower, around 2.0. In South Asian populations, the cutoffs are lower still: 2.8 for men and 2.5 for women. If your triglycerides are high and your HDL is low, that combination reflects the metabolic pattern insulin resistance creates.

Fasting Insulin and HOMA-IR

A fasting insulin level is one of the more direct ways to assess resistance, but many doctors don’t order it routinely. If you can get it measured, your fasting insulin and fasting glucose together can be used to calculate a score called HOMA-IR. There’s no single universal cutoff, but in U.S. clinical settings a score of 2.5 or higher is generally considered indicative of insulin resistance. In Asian populations, thresholds tend to be lower, ranging from 1.4 to 2.5. Ask your doctor to add fasting insulin to your next blood draw if you suspect resistance. It’s inexpensive and far more informative than glucose alone.

Testing That Goes Deeper

A standard two-hour oral glucose tolerance test (OGTT) measures your blood sugar after drinking a glucose solution. Prediabetes is defined as a two-hour result between 140 and 199 mg/dL. This test is better than fasting glucose at catching problems, because it shows how your body handles an actual sugar load rather than just a snapshot at rest.

An even more detailed version, sometimes called a Kraft test, measures insulin (not just glucose) at multiple points over three to five hours. It reveals five distinct patterns of insulin response. A healthy pattern shows a moderate insulin peak at 30 to 60 minutes that drops quickly. Patterns classified as hyperinsulinemic show insulin peaking too high, peaking too late, or staying elevated for hours. These abnormal patterns can appear years before glucose numbers ever leave the normal range. The Kraft test isn’t widely available, but it’s the closest thing to catching insulin resistance at its earliest stage.

The PCOS Connection

For women, insulin resistance and polycystic ovary syndrome (PCOS) are deeply intertwined. Roughly 70 to 80% of women with PCOS have insulin resistance. The elevated insulin drives the ovaries and adrenal glands to produce excess androgens, which in turn cause symptoms like irregular periods, acne, thinning hair on the scalp, and excess hair growth on the face or body.

Reactive hypoglycemia is especially common in this group. When women with PCOS eat simple carbohydrates, the resulting insulin surge and subsequent blood sugar crash stimulates cortisol and adrenal androgen secretion. Cortisol promotes central fat deposition, which worsens insulin resistance. The adrenal androgens convert to testosterone, worsening hirsutism. Research shows that about one-third of women with PCOS experience physiologically significant reactive hypoglycemia, and those women tend to be more obese and more insulin resistant than those who don’t. If you have PCOS symptoms and also experience post-meal crashes and sugar cravings, insulin resistance is very likely a driving factor.

Risk Factors That Raise Your Odds

Some things that increase the likelihood of insulin resistance are within your control, and some aren’t. A family history of type 2 diabetes is one of the strongest predictors. Carrying excess weight, particularly around the waist, is closely linked because fat tissue (especially visceral fat) actively impairs insulin signaling. A sedentary lifestyle matters too: exercise directly increases the number of glucose transporters on muscle cells, while muscle atrophy and inactivity decrease them. Age, ethnicity (South Asian, Hispanic, Black, and Native American populations face higher risk), and a history of gestational diabetes all shift the odds as well.

Sleep deprivation and chronic stress both worsen insulin sensitivity through cortisol elevation. If you’re doing everything right with diet and exercise but sleeping five hours a night, your metabolic picture may not improve the way you expect.

Putting the Clues Together

No single symptom or test result confirms insulin resistance in isolation. The most reliable approach is to look at the full pattern. Dark skin patches on the neck or underarms, persistent fatigue after meals, stubborn belly fat, a triglyceride-to-HDL ratio that’s creeping up, and a fasting glucose nudging toward 100: together, these tell a clear story. If you recognize several of these signs, requesting a fasting insulin level and calculating your HOMA-IR gives you a much sharper picture than glucose-only testing. Catching insulin resistance early, before it progresses to prediabetes or type 2 diabetes, opens the widest window for reversing it through changes in diet, movement, and sleep.
