How to Test for Insulin Resistance the Right Way

Insulin resistance is typically detected through blood tests that measure your fasting insulin and glucose levels, then calculate a score called HOMA-IR. A HOMA-IR score of 2.5 or higher is the most widely used threshold in U.S. clinical settings. The catch is that many standard checkups don’t include a fasting insulin test, so you may need to specifically request one.

Why Standard Blood Work Often Misses It

Most routine metabolic panels check your fasting blood sugar and HbA1c (a three-month average of blood sugar levels), but neither of these catches insulin resistance early. Your body compensates for insulin resistance by producing more and more insulin to keep blood sugar in the normal range. By the time fasting glucose or HbA1c starts climbing, you’ve likely already developed significant insulin resistance and your pancreas is struggling to keep up. Fasting insulin levels can identify the problem long before glucose-based markers budge.

This is the core frustration for many people: you can have insulin resistance for years while your blood sugar looks perfectly fine on paper.

Fasting Insulin and the HOMA-IR Score

The most practical test for insulin resistance combines two blood draws taken after an overnight fast: fasting glucose and fasting insulin. These two numbers are plugged into a formula called HOMA-IR (Homeostatic Model Assessment for Insulin Resistance), which estimates how effectively your cells respond to insulin.

There’s no single universal cutoff, but a HOMA-IR of 2.5 or above is the threshold used in the National Health and Nutrition Examination Survey (NHANES) to flag insulin resistance. In practice, U.S. clinicians typically use cutoffs between 2.0 and 3.0. For context, a large study of U.S. adults without diabetes found a median HOMA-IR of 2.2 and a mean of 2.8. In adolescents, normal-weight individuals averaged 2.3, while those with obesity averaged 4.9.

Cutoffs also vary by ethnicity. Asian populations generally use lower thresholds, ranging from 1.4 to 2.5, to identify metabolic risk.

What Fasting Insulin Levels Look Like

Standard lab reference ranges for fasting insulin run from about 2 to 25 mcIU/mL, which is a wide window. Many clinicians who focus on metabolic health consider levels under 15 mcIU/mL more optimal, and some prefer to see values even lower. A landmark analysis of 1,000 people found a mean fasting insulin of 14 mcIU/mL with a standard deviation of 5. If your fasting insulin comes back at 20, your lab report might mark it “normal,” but it could still indicate your body is working harder than it should to manage blood sugar.

How to Prepare for the Test

You’ll need to fast for 8 to 12 hours before the blood draw, meaning no food or drinks other than water. If you take biotin supplements (vitamin B7), or any multivitamin that contains biotin, stop taking them at least 24 hours before the test, as biotin can interfere with certain lab assays. Don’t stop any prescription medications without talking to your provider first.

The Triglyceride-to-HDL Ratio

If you already have a standard lipid panel on file, you can get a rough sense of your insulin resistance risk without any additional testing. Divide your triglyceride number by your HDL cholesterol (both in mg/dL). A ratio above about 2.8 for men or 2.5 for women suggests insulin resistance is worth investigating further.

These thresholds vary by race. For African Americans, the ratio is less accurate overall, and lower cutoffs apply: above 2.0 for men and above 1.5 for women. The ratio works best as a screening signal for Caucasians, Asians, and Hispanics. It’s not a diagnosis on its own, but a high ratio is a good reason to push for fasting insulin testing.

The Oral Glucose Tolerance Test

A more detailed option is the oral glucose tolerance test (OGTT), where you drink a standardized glucose solution and have your blood drawn at intervals over two hours. The standard version only measures glucose at each time point, but an enhanced version that also tracks insulin levels at each draw can reveal how your body’s insulin response behaves in real time.

This matters because some people have normal fasting numbers but produce enormous amounts of insulin after eating to keep glucose in range. The insulin level at the 120-minute mark has shown excellent diagnostic accuracy for identifying insulin resistance in research settings. An OGTT with insulin measurements gives a more complete picture than fasting labs alone, especially for people whose fasting results are borderline. Not every lab or clinic offers the insulin-inclusive version, so you may need to ask for it specifically.

The Gold Standard (and Why You Won’t Get It)

The most precise measurement of insulin resistance is a procedure called the hyperinsulinemic-euglycemic clamp, developed in 1979. During this test, insulin is infused at a constant rate through an IV while a separate glucose infusion is carefully adjusted to keep blood sugar steady. The amount of glucose your body uses during this process directly measures how sensitive your tissues are to insulin.

It’s considered the gold standard because it eliminates the guesswork involved in indirect calculations. At the insulin levels used during the procedure, the liver stops releasing its own glucose, making the measurement of tissue glucose uptake highly accurate. But the clamp takes about two hours, requires continuous IV monitoring and blood sampling, and is almost exclusively used in research. You won’t encounter it in a typical clinical visit, and you don’t need to. HOMA-IR and glucose tolerance testing are reliable enough for real-world diagnosis and monitoring.

Physical Signs Worth Noting

Before or alongside lab testing, some physical markers can point toward insulin resistance. Waist circumference is one of the simplest: measurements above 40 inches for men or 35 inches for women are part of the diagnostic criteria for metabolic syndrome, which is closely linked to insulin resistance. Darkened patches of skin on the neck, armpits, or groin (called acanthosis nigricans) are another visible signal. Skin tags, particularly around the neck and underarms, also correlate with elevated insulin levels.

These signs don’t confirm insulin resistance on their own, but they’re useful context when deciding whether to pursue blood work.

Which Tests to Ask For

If you suspect insulin resistance, the most useful starting request is a fasting insulin level alongside your fasting glucose. Many providers will order these without hesitation, especially if you have risk factors like excess weight around the midsection, a family history of type 2 diabetes, polycystic ovary syndrome, or a history of gestational diabetes. If your results are borderline, an oral glucose tolerance test with insulin measurements at each time point gives a clearer answer. And if you already have recent lipid panel results, calculate your triglyceride-to-HDL ratio to see if the numbers align.

The key thing to know is that insulin resistance is detectable years before diabetes develops. The tests exist and are straightforward. The gap is usually that they aren’t ordered as part of routine screening unless you ask.
