Most blisters on the bottom of the foot heal on their own within a week if you protect them from further friction. The key decision is whether to drain the blister or leave it intact, and that depends on how much pain it’s causing when you walk.
An unbroken blister is its own best bandage. The intact skin over the fluid acts as a natural barrier against bacteria, significantly lowering your risk of infection. If the blister is small enough that you can walk without significant pain, leave it alone and let your body reabsorb the fluid over a few days.
When and How to Drain a Blister Safely
A blister on the sole of your foot takes pressure with every step, so painful blisters often need to be drained. The goal is to release the fluid while keeping the overlying skin completely intact. That flap of skin continues to protect the raw tissue underneath and speeds healing considerably compared to exposed skin.
Here’s how to do it safely:
- Clean the area. Wash the blister and surrounding skin with soap and warm water, then pat dry.
- Sterilize a needle. Wipe a sharp sewing needle with rubbing alcohol. Don’t use a flame, which can leave carbon residue on the tip.
- Puncture the edge. Make a small hole near the base of the blister, at the side closest to the ground. A single puncture is usually enough. For larger blisters, you can make two small holes on opposite sides so fluid drains more completely.
- Press gently. Use clean gauze or a tissue to press the fluid out. Let the overlying skin settle flat against the raw skin beneath it.
- Apply ointment. Cover the drained blister with a thin layer of petroleum jelly. This keeps the area moist, prevents the skin flap from drying out and cracking, and creates a barrier against bacteria.
- Bandage it. Cover with an adhesive bandage or a small piece of gauze held in place with medical tape. On the bottom of the foot, a moleskin donut (a ring of moleskin with a hole cut for the blister) helps cushion the area and keeps direct pressure off while you walk.
Daily Care After Draining
Change the bandage at least once a day, or whenever it gets wet or dirty. Each time, gently wash the area, reapply petroleum jelly, and put on a fresh bandage. The skin flap will gradually dry and peel away on its own over several days. Don’t pull it off early. As long as it stays attached, it’s protecting the new skin forming underneath.
If the blister refills with fluid (which is common on the sole of the foot because of repeated pressure), drain it again using the same sterile technique. Refilling doesn’t mean something is wrong. It just means the tissue is still producing fluid as part of the healing process.
Most drained blisters on the foot heal within 7 to 10 days. The new skin underneath will look pink or red at first and may feel tender for a few days after the old skin peels away.
If the Blister Has Already Popped
When a blister tears open on its own, the priority shifts to infection prevention. Gently clean the area with soap and water. If there’s a loose flap of skin still attached, smooth it down over the wound rather than cutting it off. Apply petroleum jelly and cover with a bandage.
If the skin flap has torn away entirely, treat it like an open wound. Keep it clean, moist with petroleum jelly, and covered at all times. Open blisters on the sole are especially vulnerable to infection because of constant contact with socks, shoes, and the ground. Change the bandage more frequently, and avoid going barefoot until new skin has fully formed.
Signs of Infection
A normal healing blister might be mildly red and tender. An infected blister looks distinctly different. Watch for fluid that turns green or yellow (pus), skin around the blister that feels hot to the touch, increasing pain rather than gradual improvement, or red streaks spreading outward from the blister site. On darker skin tones, redness around the blister can be harder to spot, so pay closer attention to heat, swelling, and the color of any fluid.
An infected blister on the foot needs medical treatment, typically a course of oral antibiotics. Don’t try to manage it at home if you see pus or spreading redness.
Reducing Pain While It Heals
The bottom of the foot is one of the worst places to get a blister because you can’t easily avoid putting weight on it. A few strategies help:
- Moleskin padding. Cut a donut shape and place it around the blister so the raised ring absorbs pressure instead of the blister itself.
- Blister-specific bandages. Hydrocolloid bandages (the gel-type patches sold for blisters) cushion the area, stay in place better than regular adhesive bandages, and maintain a moist healing environment.
- Looser shoes. If the blister was caused by tight footwear, switch to a roomier pair while it heals. Sandals or open-backed shoes can take pressure off the sole depending on blister location.
Preventing Blisters on the Sole
Friction blisters happen when layers of skin separate under repeated shearing force, the kind of force your foot experiences with every step when a shoe doesn’t fit right or your skin is damp. Prevention comes down to reducing friction, managing moisture, and wearing properly fitted shoes.
Sock choice matters more than most people realize. Acrylic or polypropylene socks reduce friction and wick moisture away from the skin. A two-layer sock system (a thin polyester liner under a thicker wool or polypropylene outer sock) is especially effective because the layers slide against each other instead of against your skin. Cotton socks absorb sweat and hold it against the foot, which softens the skin and makes it more blister-prone.
Lubricants can also help. Applying a triglyceride-based lubricant (like body glide or even plain petroleum jelly) to friction-prone areas of the sole before activity reduces shearing forces. Foot antiperspirants decrease blister risk by keeping the skin drier, though they can cause irritation in some people with regular use.
Neoprene insoles reduce friction inside the shoe and may help if you’re prone to sole blisters during long walks or runs. Above all, shoes that fit properly are the simplest and most effective prevention. A shoe that’s too tight compresses the foot, and one that’s too loose allows excessive sliding. Either situation generates the friction that causes blisters.
