How to Treat a Sprained Wrist: Relief and Recovery

Most sprained wrists are mild, involving stretched but intact ligaments, and heal well with a few simple steps: rest the joint, manage swelling, and gradually rebuild strength. A mild sprain typically resolves in two to four weeks, while a moderate or severe sprain can take two to three months or longer. The key is matching your treatment to the severity of the injury and knowing the signs that suggest something more serious than a sprain.

How to Tell How Serious Your Sprain Is

Wrist sprains are graded on a three-tier scale. A Grade 1 (mild) sprain means the ligaments are stretched but not torn. You’ll have some pain and mild swelling, but you can still move the wrist. A Grade 2 (moderate) sprain involves a partial tear of the ligament, sometimes pulling a small chip of bone away with it. Pain is more intense, swelling is noticeable, and gripping feels weak or unstable. A Grade 3 (severe) sprain is a complete ligament tear, often producing significant bruising, instability, and sharp pain with any movement.

One injury that commonly masquerades as a wrist sprain is a scaphoid fracture, a break in one of the small bones at the base of the thumb. Scaphoid fractures cause pain and swelling in the hollow between the two tendons on the back of your thumb, a spot called the anatomical snuffbox. If pressing on that area hurts, or if tilting your wrist toward your pinky side reproduces the pain, you should get an X-ray. These fractures are frequently missed on initial exams and can cause lasting problems if untreated. Visible deformity, inability to move the wrist at all, or numbness in the fingers are also reasons to seek imaging rather than treating at home.

Immediate Treatment in the First 72 Hours

The first one to three days after a sprain are about controlling swelling and preventing further damage. A protocol published in the British Journal of Sports Medicine recommends two phases: an immediate protection phase and an active recovery phase. During the first phase, the priorities are straightforward.

Protect the joint. Restrict movement for one to three days to minimize bleeding inside the tissue and prevent the stretched ligament fibers from tearing further. A simple wrist splint or brace works well. You don’t need a rigid cast for a mild sprain, just enough support to keep you from bending the wrist under load.

Elevate your hand. Keep your wrist above the level of your heart whenever you can. This helps fluid drain away from the injured area and reduces swelling faster than resting with your hand at your side.

Compress the area. An elastic bandage wrapped snugly (not tightly) around the wrist limits swelling and provides gentle support. If your fingers start tingling, turn white, or feel cold, loosen the wrap.

Apply ice. Use a cold pack wrapped in a thin cloth for 15 to 20 minutes at a time, several times a day. Avoid placing ice directly on the skin.

Pain Relief With Over-the-Counter Medication

Anti-inflammatory painkillers help with both pain and swelling. Ibuprofen can be taken as one to two 200 mg tablets every four to six hours, up to 1,200 mg per day. Naproxen sodium is another option: one to two 220 mg tablets every eight to twelve hours, up to 660 mg per day. Take either with food to reduce stomach irritation, and stick to the lowest effective dose for the shortest time you need it.

Some clinicians suggest avoiding anti-inflammatories in the very first 24 hours because a small amount of inflammation is part of the body’s natural healing response. If you’re unsure, acetaminophen (Tylenol) handles pain without affecting inflammation and is a safe alternative for those initial hours.

When to Start Moving Again

After the initial protection window of one to three days, gentle movement becomes important. Keeping the wrist completely immobilized for too long can lead to stiffness, muscle weakening, and a slower overall recovery. The goal is to reintroduce pain-free motion gradually.

Start with basic range-of-motion exercises. Slowly bend your wrist forward and backward, then side to side, stopping at the point where you feel resistance but not sharp pain. Rotate your forearm so your palm faces up, then down. Perform these movements several times a day in sets of 10 to 15 repetitions. Mild discomfort is expected. Sharp or worsening pain means you’re pushing too far.

Rebuilding Strength

Once you can move the wrist through its full range without significant pain, typically after one to two weeks for a mild sprain, you can begin strengthening exercises. Resistance bands are a practical and inexpensive tool for this phase.

For wrist extension: sit down and rest your forearm on your thigh with your hand hanging past your knee, palm facing down. Hold one end of a resistance band and step on the other end. Slowly bend your wrist upward over two seconds, then lower it back down over five seconds. The slow lowering phase is what builds strength most effectively. Repeat eight to twelve times.

For wrist flexion: use the same seated position, but flip your hand so your palm faces up. Curl the wrist upward against the band’s resistance using the same two-seconds-up, five-seconds-down tempo. Again, aim for eight to twelve repetitions.

Grip strengthening matters too. Squeezing a soft ball or rolled towel and holding for five seconds helps restore the hand strength you lose during the rest phase. Progress from a soft object to a firmer one as comfort allows.

Recovery Timeline by Severity

A Grade 1 sprain generally heals in two to four weeks with consistent home treatment. You can usually return to light activity within a week and full activity, including sports, by the three- to four-week mark as long as there’s no pain with gripping or loading the wrist.

Grade 2 sprains take longer, often six to eight weeks. Because these involve partial ligament tears and sometimes small bone chips (avulsion fractures), they may require a more rigid splint and formal physical therapy. Your doctor might order imaging to rule out fracture fragments that need specific treatment.

Grade 3 sprains, with fully torn ligaments, can require eight to twelve weeks of recovery or more. Some need surgical repair, particularly if the wrist joint is unstable. Post-surgical rehab follows a similar progression of rest, range-of-motion work, and strengthening, but on a slower timeline guided by a therapist.

Signs Your Sprain Isn’t Healing Normally

Most mild sprains improve noticeably within the first week. If your pain hasn’t decreased at all after seven to ten days, or if swelling is getting worse instead of better, the injury may be more severe than you initially thought. Persistent tenderness at the base of the thumb (that anatomical snuffbox area) is particularly concerning for a missed scaphoid fracture, which sometimes doesn’t show up on the first X-ray and may need a follow-up scan.

A wrist that feels loose or unstable when you try to use it, clicks painfully during movement, or can’t support any weight after two to three weeks of rest warrants a closer look. Ligament injuries in the wrist, especially to the ligament connecting the two bones in the center of the wrist (the scapholunate ligament), can cause long-term instability if left untreated.
