Which Cavities Can Actually Be Reversed

Tooth decay isn’t a single event. It’s a tug-of-war between two ongoing processes: demineralization (acid dissolving minerals out of the enamel) and remineralization (minerals from saliva depositing back into the enamel). A cavity forms when demineralization wins that battle over time.

A double-blind crossover study using enamel blocks from primary (baby) teeth found no statistically significant difference in remineralization between 10% hydroxyapatite and 500 ppm fluoride toothpaste. This makes hydroxyapatite toothpaste a reasonable choice for parents concerned about fluoride, particularly for young children who tend to swallow toothpaste. It’s widely available in children’s formulations. Look for products listing hydroxyapatite or nano-hydroxyapatite at 10% concentration, as lower amounts haven’t been studied as thoroughly.

What About Oil Pulling and Other Home Remedies

Oil pulling, typically done with coconut oil, is one of the most commonly searched natural remedies for cavities. The theory is that swishing oil reduces levels of the bacteria most responsible for tooth decay. While some small studies in adults have shown modest antibacterial effects, there is no published clinical data showing oil pulling reverses or arrests cavities in children. Studies investigating coconut oil pulling in pediatric populations are still in the recruitment phase and have not yet produced results.

Other popular natural approaches, including clove oil, neem, and baking soda rinses, face the same evidence gap. They may have mild antimicrobial properties, but “reduces some bacteria in the mouth” is a very different claim from “treats cavities.” No natural antimicrobial has demonstrated the ability to halt active decay in a controlled trial. At 18 months it was 78%, and beyond 30 months it held at 71%. Decay removal before application wasn’t necessary. The major tradeoff is cosmetic: SDF permanently stains the treated area black. For back teeth this is rarely a concern, but it’s worth knowing upfront. No significant complications beyond staining were reported across 19 clinical trials in children.

SDF doesn’t restore the tooth’s shape, so large cavities may still eventually need a crown or filling for structural reasons.
