How to Treat Diabetes in Cats: Insulin, Diet & Remission

Treating diabetes in cats typically involves a combination of insulin injections, a low-carbohydrate diet, and regular blood glucose monitoring at home. The good news: about 30% of diabetic cats in the United States eventually go into remission, meaning they no longer need treatment. Early, aggressive management gives your cat the best shot at that outcome.

Why Diet Is the First Thing to Change

Cats are obligate carnivores, and their bodies aren’t built to process large amounts of carbohydrates. In a diabetic cat, carbohydrates directly contribute to dangerously high blood sugar. The current recommendation is to feed a diet where only about 12% of calories come from carbohydrates and at least 40% come from protein. In practical terms, this usually means switching to a high-protein wet food, since most dry kibble is loaded with starches and grains that spike blood sugar.

A high-protein diet does more than just limit glucose swings. It helps preserve lean muscle mass, keeps your cat feeling full on fewer calories, and reduces the risk of a serious liver condition called hepatic lipidosis, which can develop when overweight cats lose weight too quickly. If your cat is overweight, gradual weight loss is the goal. Crash dieting in cats is dangerous, so work with your vet to set a safe calorie target. Feeding on a consistent schedule, ideally timed around insulin injections, helps keep blood sugar more predictable throughout the day.

Insulin Therapy: The Standard Treatment

Most diabetic cats need insulin injections twice a day, given roughly 12 hours apart. The idea of giving your cat shots sounds daunting, but insulin needles are tiny, and most cats barely react once you and they get used to the routine. Three insulin types are commonly used in cats, each with a different profile.

  • Vetsulin (Lente): A porcine-based insulin approved for cats. It starts working quickly, peaking between 2 and 8 hours, and lasts roughly 8 to 14 hours. Starting doses are low, typically no more than 3 units per injection.
  • ProZinc (PZI): A longer-acting insulin also approved for cats. It peaks between 5 and 7 hours and can last anywhere from 8 to 24 hours. Cats usually start at 1 to 2 units per injection.
  • Glargine: A human insulin used off-label in cats, often favored because of its long, steady action. It peaks around 12 to 14 hours and lasts 12 to 24 hours. Many veterinarians prefer glargine for newly diagnosed cats because its prolonged effect may improve the chances of remission.

Your vet will choose a starting dose based on your cat’s weight and blood sugar levels, then adjust it over the following weeks based on how your cat responds. Dose adjustments are normal and expected. It can take several weeks to find the right amount, and the dose may change over time as your cat’s condition improves or shifts.

Oral Medications: A Newer Option for Some Cats

Two oral medications, bexagliflozin (Bexacat) and velagliflozin (Senvelgo), were recently approved for treating feline diabetes. These are SGLT2 inhibitors, which work by causing the kidneys to flush excess glucose out through urine rather than helping cells use glucose for energy. You give the pill once daily at around the same time, with or without food.

These drugs aren’t for every diabetic cat. They only work in cats that still produce some insulin on their own but don’t respond to it properly, a situation similar to type 2 diabetes in humans. Cats that have ever been treated with insulin, cats with kidney, liver, or pancreatic disease, and cats showing signs of diabetic ketoacidosis (a serious complication covered below) cannot take these medications. Cats that are lethargic, dehydrated, or not eating are also excluded. Your vet can run blood tests to determine whether your cat is a candidate.

Monitoring Blood Sugar at Home

Home monitoring is a central part of managing feline diabetes. In fact, the latest veterinary guidelines from the American Animal Hospital Association recommend against relying on in-hospital blood glucose curves for cats, because the stress of a vet visit can artificially raise blood sugar and give misleading results. Checking at home, where your cat is calm, produces more reliable numbers.

The most commonly recommended glucometer for cats is the AlphaTrak 2, which is calibrated specifically for veterinary patients and tends to be more accurate than human meters. To get a blood sample, you prick your cat’s ear margin, a non-weight-bearing paw pad, or the gum line using a small lancet. Most owners find the ear the easiest spot. A tiny drop of blood on the test strip is all you need.

Beyond the numbers, pay attention to your cat’s behavior. The current guidelines emphasize monitoring the cat, not just the readings. A cat that’s drinking normal amounts of water, maintaining weight, staying active, and using the litter box at a normal frequency is generally well-regulated, even if an individual glucose reading looks a little high.

Diabetic Remission: What It Means and How Likely It Is

Unlike dogs, many diabetic cats can eventually stop needing insulin altogether. This is called diabetic remission. About 30% of treated cats achieve it. Among those that do, roughly a quarter reach remission within 2 to 3 months of diagnosis, and the majority get there within 6 months.

Remission is most likely when treatment starts early and combines tight blood sugar control with a low-carbohydrate diet. The logic is straightforward: high blood sugar is itself toxic to the cells that produce insulin. By bringing glucose levels down quickly, you give those cells a chance to recover. This is why many vets push for aggressive treatment right from the start rather than a wait-and-see approach.

Remission doesn’t always mean the diabetes is gone permanently. Some cats relapse weeks, months, or even years later, so continued monitoring is important even after insulin is stopped.

Recognizing and Handling Low Blood Sugar

Hypoglycemia, or dangerously low blood sugar, is the most immediate risk of insulin therapy. It happens when a cat receives too much insulin relative to what it needs, sometimes because the cat didn’t eat a full meal or because the diabetes is improving and the dose hasn’t been reduced yet. The warning signs escalate in severity:

  • Early signs: unusual hunger, restlessness, shivering
  • Moderate signs: wobbling, loss of coordination, disorientation
  • Severe signs: seizures, convulsions, collapse, coma

If your cat is still conscious and able to swallow, give corn syrup or a glucose solution by mouth, about 1 gram per kilogram of body weight (roughly a tablespoon for an average-sized cat). If your cat is collapsed or seizing, do not pour liquid into the mouth, as it could enter the lungs. Instead, rub a small amount of corn syrup directly onto the gums or under the tongue and get to an emergency vet immediately. Keep a bottle of corn syrup in the house at all times once your cat starts insulin.

Diabetic Ketoacidosis: The Serious Complication

Diabetic ketoacidosis, or DKA, occurs when a cat’s body starts breaking down fat for energy because it can’t use glucose at all. This process produces acidic compounds called ketones that build up in the blood and can become life-threatening. DKA can develop in undiagnosed cats, cats whose insulin dose is too low, or cats dealing with an illness that throws their diabetes out of control.

Signs of DKA include vomiting, extreme lethargy, loss of appetite, rapid breathing, and a distinctive sweet or fruity smell on the breath. If your cat is already on insulin and suddenly becomes very sick, this is an emergency. At-home ketone meters (the same type used by people with diabetes) can detect ketones in a blood sample and may catch the problem earlier than urine test strips. DKA requires hospitalization with intravenous fluids and careful insulin management, and the sooner treatment starts, the better the outcome.

What Daily Life Looks Like

Managing a diabetic cat settles into a routine faster than most people expect. A typical day involves two meals timed with two insulin injections, and periodic blood glucose checks at home. Many owners find that the hardest part is the first week or two of learning the injection technique and getting comfortable with the glucometer. After that, it becomes a quick task that takes a few minutes morning and evening.

Costs vary depending on the type of insulin, how often you monitor, and whether your cat needs a prescription diet. Insulin vials last several weeks for most cats, and test strips are the main recurring expense. Some owners find that the low-carb diet alone, combined with weight management, dramatically improves their cat’s condition, reducing the amount of insulin needed over time and sometimes eliminating the need for it entirely.
